    Description / Table of Contents: Zusammenfassung Anhand des eigenen Krankenguts, das in 7 1/4 Jahren 514 operativ behandelte Coloncarcinome (Radikaloperationen in 58,5%) umfaßt, wird erneut das Problem der einzeitigen Colonresektion ohne entlastende Darmfistel beleuchtet. 194 einzeitige Resektionen am Quer- und linksseitigen Colon ohne entlastende Fistel gliedern sich in 101 Sigma-, 39 Rectosigmoid-, 21 Quercolonresektionen und 33 linksseitige Hemicolektomien. In 10,3%, mußten zur Wahrung der Radikalität erweiterte Colonresektionen erfolgen. Ergebnisse: Unter, diesen 194 Resektionen kam es in 3,6% zur partiellen Anastomoseninsuffizienz; davon führten 2 Fälle zur kotigen Peritonitis, 5 Kotfisteln heilten unter Drainagebehandlung spontan. Nach den Rectosigmoidresektionen waren klinisch keine Anastomosendehiszenzen nachweisbar. Tödliche postoperative Atonien traten nicht ein. Wundinfektionen (Abdeckung mit Aeroplast-Wundschutzfolien) entstanden bei Sigma- und Rectosigmoidresektionen in 5,2%, nach linksseitigen Hemikolektomien in 8,5%, nach Quercolonresektionen kein Wundinfekt. Die post-operative Wundruptur sahen wir in 1%. In 1% kam es zu einem postoperativen mechanischen Ileus. Die primäre Operationsmortalität unter 194 Resektionen betrug 5,6%, nach Ausschluß palliativer Resektionen 5%. In 1% entstand eine kotige, letal endende Peritonitis. Die durchschnittliche postoperative Verweildauer war nach Sigma- und Rectosigmoidresektionen 18,3 Tage, nach linksseitigen Hemicolektomien 20,1 Tage, nach Quercolonresektionen 16,9 Tage.
    Notes: Summary On the basis of our own clinical material which comprises 514 surgically treated carcinomas of the colon (radical operations in 58.5%) and covers 71/4 years, we again approach the problem of the one-stage operation without a relieving intestinal fistula. 194 one-stage resections of the transverse and left colon without relieving fistula can be divided into 101 sigmoid, 39 recto-sigmoid and 21 transverse colon resections and 33 left-sided hemicolectomies. In 10.3% of the cases, a further colon resection was necessary, to complete the radical surgery. Results: In these 194 resections, partial insufficiency of the anastomosis occurred in 3.6%; of these, 2 cases developed faecal peritonitis. 5 faecal fistulas healed spontaneously after drainage. No dehiscence of the anastomosis was clinically observed after the recto-sigmoid resections. No fatal adynamic ileus occurred post-operatively. Wound infection (Aeroplast wound-foils were used) occurred in 5.2%. of the cases after sigmoid and recto-sigmoid resections, in 8.5% after left-sided hemicolectomies and in none of the patients after resections of the transverse colon. Post-operative rupture of the wound occurred in 1% of the cases. Post-operativemechanical ileus was observed in 1% of the cases. The primary operative mortality rate in the 194 resections was 5.6% and after excluding the palliative resections. it was 5%. In 1% of the cases faecal peritonitis developed, and proved fatal. The average post-operative hospitalization was, after sigmoid and recto-sigmoid resections, 18.3 days, after left-sided hemicolectomies, 20.1 days and after resections of the transverse colon, 16.9 days.
