Zusammenfassung Die Primärbehandlung der profusen Oesophagusvaricenblutung mit Kompressionssonden unter gleichzeitiger Schockbekämpfung und Prophylaxe gegenüber eines Leberausfallkomas führt zunächst in ca. 70–75% zur Blutstillung. Geeignete Fälle sollten in diesem Stadium einer druckableitenden Not-Shuntoperation zugeführt werden. Bei der Vielzahl der angegebenen Palliativoperationen ist die Wahl des einzuschlagenden Weges nicht leicht. Zu empfehlen ist ein programmiertes, teilweise auch standardisiertes, jedoch der Notsituation angepaßtes Vorgehen. Bei erfolgloser konservativer Blutstillung hat sich uns bei nicht anwendbarem Not-Shunt die transthorakale Oesophagusdissektion am besten bewährt.

Summary In 70–75% of cases, bleeding from ruptured esophageal varices can be controlled by means of Sengstaken-Blakemore tubes, while shock is treated at the same time. Preventive measures to avoid hepatic-failure coma are initiated at the same time. After successful primary treatment, an emergency porto-systemic shunt operation should be performed. Many palliative operations have been proposed in the past, so that it is quite difficult to select the one most suitable for the individual patient. For best results we suggest a programmed, mainly standardized, but also flexible procedure. In cases where conservative management has not arrested the massive bleeding we have found transthoracic esophagus dissection to be the method of choice.
