Zusammenfassung Blutungen aus dem Dünn- und Dickdarm haben vielgestaltige morphologische Ursachen: Isolierte bis diffuse Polypen, Rectocolitis, neoplastische Läsionen und gelegentlich auch die Divertikelkrankheit. Immer sollte unter dem Symptom Melaena zunächst auch eine postbulbäre Ulcusblutung durch Gastroskopie ausgeschlossen warden. Das diagnoatisch-taktische Vorgehen unter besonderer Berücksichtigung der prä- und intraoperativen Coloskopie und Angiographie sowie Problemfälle wie Blutungen aus Anastomosen, Colostomien und nach traumatischer Läsionen des Anorectalkanals werden besprochen.

Summary There are various morphological causes of bleeding from the small and large intestine: isolated or diffuse polyps, rectocolitis, paraneoplastic lesions, and occasionally diverticular disease. The symptom melaena demands gastroscopy to exclude bleeding from a gastroduodenal ulcer. Diagnostic technique is discussed with special reference to the diagnostic value of preand intraoperative endoscopy, and problems such as bleeding from anastomoses after colostomy or bleeding following traumatic lesions in the anorectal region are also dealt with.
