Zusammenfassung Die Grenzbereiche der konservativen Therapie frakturierter Gelenkkörper werden vorgestellt. Dazu gehören allgemein die Gelenkfrakturen des Kahnbeins, des Humeruskopfes, des Fersenbeines sowie geeignete intraartikuläre Frakturen der distalen Speiche und des Tibiakopfes. Kaum, nicht verschobene und nicht zur Dislokation neigende Gelenkbrüche werden oft frühfunktionell behandelt. Bei alten Menschen ist die konservative Therapie vieler Gelenkfrakturen vorteilhaft, zumal eine mögliche posttraumatische Arthrose nicht mehr erlebt wird. Zu warnen ist vor der internen Osteosynthese von Gelenkfrakturen bei arteriellen Durchblutungsstörungen und bei Gelenkfrakturen mit nachhaltiger Weichteiltraumatisierung. Regellose, trümmerhafte Zerstörungen der Gelenkkörper sind operationstechnisch nicht operabel. Zweifellos gibt es auch iatrogene Gründe, operationstechnisch schwierige Gelenkfrakturen wegen mangelnder Erfahrung nicht zu versorgen.

Summary The borderline cases of conservative therapy of joint fractures are described. They include in general joint fractures of the scaphoid, the head of the humerus, the calcaneum and appropiate intraarticular fracture of the distal radius and head of the tibia. Little or nondisplaced joint fractures and joint fractures which tend to dislocate are frequently treated functionally at an early stage. The conservative therapy of many joint fractures is advantageous for older people since a potential posttraumatic arthrosis is no longer experienced. The external osteosynthesis of joint fractures in care of arterial blood circulation disturbances and in care of severe soft tissue trauma is associated with risks. Irregular, decade joint distractions are technically inoperable. Doubtlessly, there are some joint fractures which should not be operated because of lack of experience of the surgeon.
