Zusammenfassung Die Defektdeckung bei ausgedehnten Tumorresektionen, Weichteildefekten nach Strahlentherapie, Malignomen auf dem Boden von Strahlenveränderungen und bei Decubitalulcera stellt den Chirurgen noch immer vor erhebliche Probleme. Konservative Therapieversuche sind in jedem Fall langwierig und in seltenen Fällen erfolgversprechend. Muskelplastiken und insbesondere musculocutane Lappenplastiken stellen heute die Therapie der Wahl dar. Ein befriedigendes ästhetisches und funktionelles Ergebnis setzt jedoch eine entsprechende Patientenselektion, eine sorgfältige Lappenwahl und subtile Kenntnisse in der Gefäßanatomie voraus.

Summary The treatment of difficult defects is a surgical problem, whether these defects are the result of planned extirpative surgery, of tumors secondary to radiation therapy, of tissue necrosis secondary to radiation therapy or the result of pressure sores. Conservative treatment is of long duration and is promising in only a few cases. Muscle flaps and musculocutaneous flaps are today the method of choice to reconstruct these defects. Careful selection of the right patient and flap, as well as a subtle knowledge of vascular anatomy, guarantees a satisfying aesthetic and functional result.
