Zusammenfassung Anhand einer Nachuntersuchung von 18 Patienten mit Milzzysten werden Beschwerdebild, Diagnostik und Indikation zur Operation Bowie die zur Diskussion stehenden operativen Maßnahmen diskutiert. Bei heterogenem Beschwerdebild erfolgt die Diagnostik im wesentlichen durch Sonographie mit eventuell ergänzender Computertomographie. Die Indikation ergibt sich aus dem Beschwerdebild, der Größe innerhalb des Organs sowie ganz entscheidend der Dignitat der Zyste. Wir bevorzugen die Zystenwandresektion bei Pseudozysten als technisch einfaches und sicheres Verfahren. Bei unklarer Dignitat jedoch ist die Splenektomie heute noch eine adäquate Methode.

Summary Based on a late physical examination of 18 patients with splenic cysts, we discuss symptoms, diagnostics and indication to operate and the procedure of operation. We found no specific symptoms. The ultrasound is the most important diagnostic method and may be supplemented by computerized axial, tomography. The indication depends on the symptoms, the diameter within the organ and, most important, the dignity of the cyst. For pseudocysts we prefer resection of the cyst wall, because it is a simple and safe procedure. Splenectomy is the treatment of choice for cysts with unknown dignity.
