Résumé Les auteurs rapportent l'étude morphologique et ultrastructurale d'un hémangioendothélio-sarcome splénique avec diffusion rapide du processus tumoral au niveau de la moëlle osseuse et du foie. Il existe au sein de la tumeur splénique étudiée une importante activité phagocytaire et surtout érythrophagocytaire des cellules tumorales ainsi que des thromboses multiples. La phase de généralisation est marquée par l'apparition d'une anémie hémolytique, de globules rouges fragmentés, d'une thrombopénie et d'un déficit en facteurs de coagulation. Les thromboses sont responsables de la coagulopathie de consommation. L'anémie hémolytique peut s'expliquer par deux mécanismes associés: l'hémolyse intravasculaire (anémie hémolytique microangiopathique), et, l'érythrophagocytose par les cellules tumorales et le système réticuloendothélial, de globules rouges fragilisés par leur passage dans la tumeur.

Summary The authors studied the light and ultrastructural morphology of a malignant hemangioendothelioma of the spleen, which rapidly spread to the liver and bone marrow. An important phagocytic activity, especially erythrophagocytic, of the tumor cells was observed as well as multiple capillary thromboses. The stage of general spread was marked by the appearance of hemolytic anemia, thrombocytopenia, fragmented red blood cells, and a deficit in coagulation factors. The thrombosis was considered to be responsible for the consumption coagulopathy. The hemolytic anemia may be explained by two associated mechanisms: intravascular hemolysis (microangiopathic hemolytic anemia), and erythrophagocytosis by the tumor cells and by the reticuloendothelial system of the red blood cells made fragile by their passage through the tumor.
