Zusammenfassung Der Bronchialschleim von insgesamt 25 Patienten mit chronisch obstruktiver Bronchitis und teilweise emphysematösen Lungenveränderungen wurde über einen 15tägigen Klinikaufenthalt untersucht. Versuchsgruppe I (15 Patienten) wurde schon über längere Zeit mit Glucocorticoiden behandelt; Versuchsgruppe II (10 Patienten) kam unvorbehandelt in unsere Klinik. Durch Ultrazentrifugation, Ultraschallbehandlung und anschließende Gefriertrocknung konnten 3 Fraktionen aus dem Sputum gewonnen werden. Als Meßgrößen bestimmten wir die expektorierte Sputummenge, das Trockengewicht der einzelnen Fraktionen, die Gesamteiweißmenge, die Zusammensetzung der Serumproteine Albumin, γ A-, G-, M-Globulin und α1-Antitrypsin sowie das Vorhandensein proteolytischer Fermente mit Fermentinhibitoren. Als Maß für die Obstruktion wurden die totale Resistance und das intrathorakale Gasvolumen gewertet.

Abstract The bronchial mucus of 25 patients afflicted with chronic obstructive bronchitis and partly emphysematous lung changes has been investigated during a hospitalization of 15 days. Test group I (15 patients) had already been treated with glucocorticoids for a longer period of time; test group II (10 patients) consisted of untreated subjects. By means of ultracentrifugation, ultrasonic treatment, and subsequent freeze-drying, three fractions could be gaind from the sputum. The expectorated amount of sputum, dry weight of the single fractions, total amount of protein, composition of the serum proteins: albumin, γA-, G-, M-globulins and α1-antitrypsin were taken as measuring parameters as well as the presence of proteolytic ferments and ferment inhibitors. As measuring values for obstruction we admitted total resistance and intrathoracic gas volume. Immunoglobulins A and M were found to be augmented in the sputum during the observation time in contrast to serum values. The quotient IGG/IGA in the sputum was exceptionally shifted to the IGA-portion. In nearly all the sputa proteolytic ferments have been found. In addition to α1-antitrypsin, which probably plays a part in inhibiting proteolytic ferments in the sputum, further ferment inhibitors have been found, the nature and origin of which are still unknown.
