    Notes: Between October 1988 and March 1991, 23 ovarian cancer patients with progressive disease whilst receiving second- or third-line polychemotherapy received subcutaneously 3.6 mg Goserelin, a GnRH analogue, at monthly intervals until further tumor progression. Four patients (17.4%) achieved partial response, 7 patients (30.4%) had stable disease and 12 patients (52.2%) showed further tumor progression. Median time to tumor progression was 8.5, 5.3 and 2.1 months, respectively (Mantel test, P = 0.0003). Ten out of 11 patients who showed partial response or no change had grade 2 or grade 3 tumors. We conclude that Goserelin shows evidence of antitumor activity also in grade 2 and grade 3 ovarian carcinoma. It offers a therapeutic alternative to a group of patients, in whom we usually terminate cytotoxic treatment.

    Notes: Mirogrex terrae-sanctae is a commercial, sardine-like cyprinid fish in the Sea of Galilee. The fish spawns from December to the end of March. Females collected at the quiescent phase of the cycle (June) were exposed to four combinations of photoperiod and temperature for 3 months. The highest GSI (2.44 ± 0.22) was found in females exposed to simulated winter conditions (16°C, 9L) while the lowest GSI (0.65 ± 0.04) was found in fish exposed to simulated summer conditions (27°C, 14L). The GSI of fish maintained at the unnatural combination of 16°C, 14L (1.30 ± 0.09) did not differ from that of fish maintained at the other unnatural combination of 27°C, 9L (1.07 ± 0.11). However, histological examinations revealed that vitellogenic oocytes were present only in fish maintained at low temperature regimes, which implies that the growth of the ovarian mass in the 27°C, 9L group was due mainly to oogonial proliferation and that vitellogenesis was inhibited in Mirogrex at a high temperature.Both protein and calcium concentrations increased in the serum of fish following injections of oestradiol-17β. The increase of calcium was evident only in the non-filtrable (bound) fraction, suggesting that the total serum calcium above 13 mg% could be used as an indirect parameter for circulating vitelloprotein.Fish collected at the quiescent phase were injected with oestradiol (50 μg/fish c. 15 g body weight) and subsequently transferred to tanks maintained at either 12°, 20° or 27°C. The total serum calcium increased to 23.0 ± 1.75 mg% in the fish maintained for 8 days at 20°C. This was significantly higher than the concentration of calcium in the serum of fish maintained at either 12°C (7.8 ± 0.52) or 27°C(l1.8 ± 0.77). This implies that the temperature may affect ovarian recrudescence by modulating the response of the liver to oestrogen, allowing the oestrogen-induced vitellogenesis to be relatively high in the cool environment during winter.

    Notes: Summary A second-look operation was performed on 151 patients with stage III and IV epithelial ovarian carcinoma who had responded to primary surgery and chemotherapy. 19% of the 79 patients who appeared clinically to be free of disease had microscopic recurrences and 23% had macroscopic residual disease at a second-look operation. The 5-year survival rate for patients with no histological and for those with microscopic secondaries at second-look operation were 55% and 35% respectively (P=0.45). Only patients with well or moderately well differentiated tumors and a small residual tumor mass at first operation had a good prognosis after a second-look operation even without further chemotherapy. Median survival after secondary debulking was 15 to 17 months and was independent in the radicality of the second-look procedure. Outside of clinical trials second-look laparotomy should therefore only be performed as a diagnostic procedured as a diagnostic procedure in patients with well or moderately well differentiated tumors who are left with a small residual tumor mass at the time of the first operation. Because this is a group of patients in whom chemotherapy can be discontinued after a negative second-look operation.

    Notes: Summary On the basis of the results of earlier studies, 30 departments of gynecology have been cooperating nationwide in Austria since 1980 to promote the use of adjuvant chemotherapy after surgery for cure of ovarian carcinoma in early stages and the role of lymph node dissection and of second-look operation. Results recorded in more than 160 patients treated with adjuvant chemotherapy after so-called radical surgery performed in disease stages I and II demonstrate that only highly differentiated tumours in stage Ia can be cured by surgery only with no further adjuvant treatment. This underlines the necessity for staging. More than 200 patients with TNM stages III and IV were randomized after debulking surgery to receive treatment with different kinds of drug combinations to compare the therapeutic efficacy of a sequential alternating drug regimen consisting of Adriamycin-cisplatin+vincristine-cyclophosphamide+high-dose methotrexate with that of the combination of Adriamycin-cyclophosphamide and that of Adriamycin-cisplatin. High-dosed ifosfamide was also used in pilot studies.

    Notes: Summary We present pharmacokinetic data of prolonged, intra-arterial indomethacin treatment (i.e. induction plus maintenance dose) for symptomatic patent ductus arteriosus (sPDA) in 26 ventilated premature infants. sPDA was assessed by two-dimensional and pulsed Doppler echocardiography. Permanent ductal closure occurred in 20 (76%) infants. Plasma levels of indomethacin were 1.18±0.74; 1.8±1.0; 1.51±0.93 and 1.25±0.98 μg/ml (mean±SD) at 12, 24, 48 and 72 h after initial dose administration. All except one patient who responded with ductal closure, showed plasma levels above 0.25 μg/ml throughout the 3 day treatment period and no case of sPDA reopening was noted. Although target concentrations over time were not defined, the data indicate that the maintenance levels measured were within the therapeutic range. A negative correlation was found for plasma drug levels and postnatal age (r=0.52;P〈0.01). Volume of drug distribution was 0.23±0.18l/kg, total clearance 0.1±0.11 ml/min and elimination constant 0.06±0.05h−1 (mean±SD). The great variation in pharmacokinetic data reflects the heterogeneity of the population studied with respect to extracellular fluid space, cardiovascular status, serum protein and other parameters.

    Notes: Abstract Serum angiotensin converting enzyme (ACE) activity was determined in 46 children with congenital heart disease with normal and abnormal lung perfusion: (1) congenital heart disease with normal pulmonary blood flow (12 patients); (2) congenital heart disease with increased pulmonary blood flow (18 patients); (3) congenital heart disease with decreased pulmonary blood flow (16 patients). There was no significant difference in serum ACE activity between the three groups. In group 2 serum ACE activity had a tendency to correlate inversely with both mean pulmonary arterial pressure (r=−0.43;P≤0.05) and pulmonary vascular resistance (r=−0.48;P=0.05). No further correlations between serum ACE activity and age, serum electrolytes, creatinine nor other haemodynamic data could be established.
