    Notes: A multidisciplinary group of experts was convened by the National Institute of Mental Health (NIMH) to discuss new opportunities in psychotherapy research. Participants agreed that more research is needed to assess stakeholders’ needs, to better understand techniques commonly used in clinical practice, and to test efficacious treatments in community settings. These steps will require increased collaboration across different disciplines, development of new research methods, and a willingness for investigators to ask novel research questions. Participants outlined obstacles to conducting public health-oriented research such as the current dearth of supportive infrastructure (e.g., lack of training opportunities for new investigators), concerns about how applications for conducting new or hybrid methodologies will fare when evaluated by NIMH initial review groups, and the obvious differences in terminology used by relevant stakeholders.

    Notes: Generalization of localized perturbation theory, which results with a method able to span the spin space correctly, is presented. This generalization is achieved by using a multiconfigurational (MC) wave function as the reference. This is the most comprehensive expansion used within MC–LMP2 approach to date, with, however, low computational cost [computational scaling with system size (N) of the new method is O(N3)]. Recently, we have reported the successful Jaguar2 (J2) model for calculating atomization energies. Within the MC–LMP2 framework, the J2 model for calculating heats of formation is based on the generalized valence bond–perfect pairing (GVB–PP) wave function. The J2 model was applied only to closed shell cases because of the perfect pairing (PP) restriction in the reference function. In order to describe other systems, the PP restriction needs to be lifted. This work describes efforts in that direction. The PP restriction can be lifted by a restricted configuration interaction (RCI) procedure applied to the GVB–PP wave function. In this paper, the equations describing the application of LMP2 theory to self-consistent RCI wave function are derived and explained. The RCI wave function is a "true" MC expansion as opposed to the GVB–PP, which uses only a single spin eigenfunction (SEF). We also present the self-consistent (SC) optimization of the RCI wave function. The SC–RCI–LMP2 is the first MC–LMP2 method where the spin space is spanned in the reference. This is important for describing the nondynamical correlation (near degeneracy) effects associated, for example, with bond breaking processes. The SC–RCI–LMP2 is an efficient method applicable to large systems; it is shown to reproduce the potential energy surfaces calculated by the complete active space–second order perturbation (CAS–SCF–PT2) method. This is demonstrated, for the first time, on some widely used test cases. © 2001 American Institute of Physics.

    Notes: While the abuse potential of cocaine stems mainly from its ability to increase dopaminergic transmission in limbic regions, drug actions on other monoamine-innervated circuits may contribute to the development and maintenance of cocaine addiction. Previous extracellular recordings in anaesthetized rats revealed a facilitatory influence of cocaine on primary sensory pathways, which could influence the processing of drug-related stimuli during the development of cocaine addiction. We further analysed these sensory effects of cocaine in freely behaving rats (n = 9). Using an array of eight microelectrodes chronically implanted in infragranular layers of primary somatosensory cortex, we recorded the basal activity of 40 single- and 64 multiunits and their response to electrical stimulation of the whisker pad before and after incremental doses of cocaine (0.25–2 mg/kg i.v.). Both spontaneous and cocaine-induced explorations were associated with elevated basal firing of the cortical neurons and suppression of their short-latency excitation and postexcitatory inhibition in response to the whisker-pad stimulation. In addition to exploration-related alterations, the administration of cocaine enhanced the long-latency rebound excitation induced by the whisker-pad stimulation. This component of the sensory response, which is more labile and does not seem to convey information about the physical characteristics of the stimulus, may participate in the processing of drug-related sensory stimuli.

    Notes: : To provide a basis for regional hydroclimatic forecasting, New England (NE) precipitation and streamflow are compared with indices for the El Niño/Southern Oscillation, the Pacific North American (PNA) pattern, and the North Atlantic Oscillation (NAO). Significant positive correlations are found between the NAO index and monthly streamflow at western inland locations, with the strongest seasonal correlations occurring in winter. Smoothed records for the winter NAO and winter streamflow are highly correlated at some sites, suggesting that interrelationships are most significant in the low frequency spectrum. However, correlations between the NAO and precipitation are not significant, so further examination of other factors is needed to explain the relationship between the NAO and streamflow. NAO related regional air temperature, sea surface temperature (SST), storm tracking, and snowfall variability are possible mechanisms for the observed teleconnection. Exceptionally cool regional air temperatures, and SSTs, and unique regional storm track patterns characterized NE's climate during the famous 1960s drought, suggesting that concurrent (persistent) negative NAO conditions may have contributed to the severity of that event. Monthly and winter averaged regional streamflow variability are also significantly correlated with the PNA index. This, along with results from previous studies, suggests that tropospheric wave character and associated North Pacific SST anomalies are also related to NE regional drought conditions.

    Notes: Compression of the paediatric airway is a relatively common and often unrecognized complication of congenital cardiac and aortic arch anomalies. Airway obstruction may be the result of an anomalous relationship between the tracheobronchial tree and vascular structures (producing a vascular ring) or the result of extrinsic compression caused by dilated pulmonary arteries, left atrial enlargement, massive cardiomegaly, or intraluminal bronchial obstruction. A high index of suspicion of mechanical airway compression should be maintained in infants and children with recurrent respiratory difficulties, stridor, wheezing, dysphagia, or apnoea unexplained by other causes. Prompt diagnosis is required to avoid death and minimize airway damage. In addition to plain chest radiography and echocardiography, diagnostic investigations may consist of barium oesophagography, magnetic resonance imaging (MRI), computed tomography, cardiac catheterization and bronchoscopy. The most important recent advance is MRI, which can produce high quality three-dimensional reconstruction of all anatomic elements allowing for precise anatomic delineation and improved surgical planning. Anaesthetic technique will depend on the type of vascular ring and the presence of any congenital heart disease or intrinsic lesions of the tracheobronchial tree. Vascular rings may be repaired through a conventional posterolateral thoracotomy, or utilizing video-assisted thoracoscopic surgery (VATS) or robotic endoscopic surgery. Persistent airway obstruction following surgical repair may be due to residual compression, secondary airway wall instability (malacia), or intrinsic lesions of the airway. Simultaneous repair of cardiac defects and vascular tracheobronchial compression carries a higher risk of morbidity and mortality.

    Notes: Background: We evaluated the relationship of the bispectral index (BIS) to commonly used indices of depth of anaesthesia in 19 infants enrolled in a prospective study of the stress response to hypothermic cardiopulmonary bypass. Methods: Group 1 (n=8) received high-dose fentanyl by bolus technique; group 2 (n=6) received high-dose fentanyl by continuous infusion; and group 3 (n=5) received a fentanyl–midazolam infusion. Blood pressure (BP), heart rate (HR) and plasma epinephrine, norepinephrine, cortisol, ACTH, glucose, lactate and fentanyl were analysed 15 min postinduction, 15 min poststernotomy, 15 min on CPB during cooling and during skin closure. Results: Mean BIS (SD) values for all 19 patients were 45.3 (12.3), 40.4 (14.5), 24.4 (12.4) and 47.9 (13.9), at the successive time points. No significant differences were observed in changes in BIS over time between the groups. A significant correlation was found 15 min postinduction between BIS and BP (systolic r=0.51, mean r=0.56) in all groups, but not between BIS and HR. BIS did not correlate with BP or HR at any other time point. There was no significant correlation between BIS and hormonal, biochemical or plasma fentanyl levels for any group at any time point. Conclusions: We were unable to demonstrate a relationship between the BIS and haemodynamic, metabolic or hormonal indices of anaesthetic depth. Further evaluation of the BIS algorithm is required in neonates and infants.
