    Notes: Summary We report a series of 41 patients with multiple intracranial aneurysms, analyzing them by type of surgical treatment and prognosis. We compare our findings with those of the largest published series and draw conclusions regarding the most appropriate treatment. The upshot would seem to be that morbidity and mortality depend chiefly on whether the aneurysms are bilateral or unilateral and on the patient's pre-operative status.

    Description / Table of Contents: Zusammenfassung Die Autoren verwendeten bei 16 Fällen von Myelopathie infolge zervikaler Spondylose Kalbsknochen (Kieler Knochenspan) zur ventralen Wirbelkörperfusion. Die Patienten wurden bis zu drei Jahren nach der Operation beobachtet. Die Ergebnisse waren voll zufriedenstellend.
    Abstract: Résumé Les auteurs utilisent de l'os de veau („Kiel bone splint”) dans 16 cas de myélopathies par spondyloses cervicales qui furent traitées par approche chirurgicale antérieure. Les malades furent surveillés pendant 3 ans après l'opération. Les résultats furent un succès total.
    Notes: Summary The Authors employed calf bone (“Kiel bone splint”) in 16 cases of myelopathy from cervical spondylosis, which were treated by anterior surgical approach. The patients were observed for as long as 3 years following operation. Results proved to be fully successful.

    Notes: Summary Acute traumatic subdural haematoma suspected of extending over an entire hemisphere must be treated with all speed. To obviate unnecessary skin incisions and subsequent craniectomies and craniotomies during the operation, we think it advantageous to plan a series of burrholes at set points in order to: a) begin to clear the haematoma; b) assess its extent and possible origin; c) decide whether a hemicranial or smaller craniotomy is called for.

    Notes: Summary Of the 510 patients with pituitary adenoma treated surgically at our hospital in the period 1956–1984 319 were treated by the microsurgical technique, in the period 1973–1984, 235 by transsphenoidal approach and 84 by subfrontal-pterional approach. The transsphenoidal route was used almost exclusively in microadenomas, in intrasellar adenomas, in suprasellar adenomas with midline development, in adenomas invading the sphenoidal sinus and in haemorrhagic adenomas with considerable suprasellar development. In some giant adenomas the transsphenoidal route was used in a first stage operation for debulking the tumour, later removed by transcranial route. The latter route was preferred in large adenomas and especially in adenomas with laterosellar development. In some patients with PRL secreting adenomas post-operative treatment with bromocriptine proved useful when the hormone levels failed to noramlize. Post-operative radiotherapy was of value in invasive adenomas and in cases in which tumour removal was not radical.

    Notes: Summary 23 patients with unilateral internal carotid artery stenosis (〉70%) and contralateral internal carotid artery occlusion in the neck are reported. The symptoms are referable to the side of the occlusion in 13 cases (57%), to the side of stenosis in 7 cases (30%) and non-localizing in 3 cases (13%). All 23 patients had a carotid endarterectomy performed on the side of the stenotic lesion. There was no operative mortality. Late neurological symptomatology after surgery was referable to the side of stenosis in 13% and to the side of occlusion in 9%. The authors consider that, in cases of significant stenosis (greater than 70%) of an internal carotid artery with a contralateral occlusion, preference should always be given to endarterectomy of the stenotic side, reserving extra-intracranial by-pass of the occluded side for patients who remain symptomatic after endarterectomy of the stenotic side.

    Notes: Summary Magnetic resonance imaging (MRI) and/or computer-assisted cisternography (CAc) assessment of latent late recurrences in longterm asymptomatic patients surgically treated for intracranial epidermoid cyst is here presented. MRI was exclusively utilized in one patient; CAc was exclusively employed in three patients with metalic operative clips; both CAc and MRI were employed in another four patients. CAc appears to be more reliable than MRI in detecting cholesterin fragments in asymptomatic patients operated on for intracranial epidermoid cyst.

    Notes: Summary With the aim of understanding better the behaviour of an uncommon lesion such as the choroid plexus papilloma, we have analysed a surgical series of 33 cases operated on at the Neurosurgical Department, University of Rome “La Sapienza”, between 1955 and 1992 and have reviewed the relevant literature. Our results support the view that a radical resection of these lesions, which if removed entirely, carry a very good prognosis. The hydro-cephalus should be treated only if persistent. Adjuvant treatment such as radiotherapy and chemotherapy should be reserved for specific cases.
