    Notes: Summary The chimney sweep is professionally exposed to air pollution and carcinogenetic substances more than others. Within the scope of an occupational-medicine study begun in 1974, 121 chimney sweeps were examined with regard to history, physical status, pulmonary function including X-rays. The concentrations of different noxious substances at the place of work were measured. At the same time they underwent an ENT-medical examination including X-ray studies of the paranasal sinuses. In 1976 the follow-up examination was performed. The larynx and pharynx of all subjects were endoscopically inspected. More than 60% showed an inflammation or irritation of the pharyngeal and laryngeal mucosa. In24 of the pathological X-ray findings in the paranasal sinuses mostly cysts, solitary or multiple, were found by endoscopic inspection. This follow-up examination contributes to the evaluation of the etiological role of chronic inflammatory diseases of the upper respiratory mucosa in chronic bronchitis.

    Notes: Summary Endoscopic methods as the zoom-epipharyngoscopy and the transnasal inspection of the epipharynx enable an exact examination and diagnosis in the organic or functional lesions of the pharyngeal tube. The physiological and pathological movements of the tubal torus, the pharyngeal tubal orifice and the soft palate are demonstrated.

    Notes: Summary The kinetic behavior of heterogenous microbial populations in biotechnological processes can be described with the aid of the Monod equation extended by a term kd for endogenous metabolism. The modified Monod equation is examined in detail to demonstrate the influence of the kd-term on the values of the Monod parameters μm and KS. The principles of kinetic analysis are also shortly considered in connection with fermenter design. The prerequisites for kinetic analysis together with parameter determination are demonstrated.

    Description / Table of Contents: Summary A daily dose of 6 g cefoxitin was administered i. v. in two short infusions for six days. Treatment was successful or there was noticeable improvement in 17 of 20 patients. Cefoxitin was tolerated well locally. Changes in the haemopoetic system were only temporary. Cefoxitin would appear to be a safe and successful antibiotic for the treatment of infections of the upper female genitalia.
    Notes: Zusammenfassung Cefoxitin wurde i. v. verabreicht, die tägliche Dosis betrug 6 g und wurde in zwei täglichen Kurzinfusionen während sechs Tagen verabfolgt. 17 von 20 Fällen wurden geheilt bzw. deutlich gebessert. Die lokale Toleranz war gut. Die Veränderungen im hämopoetischen System traten nur transitorisch auf. Cefoxitin scheint ein sicheres und erfolgreiches Medikament für die Behandlung von Infektionen im oberen Abschnitt des weiblichen Genitale zu sein.

    Notes: Summary Surgical speech rehabilitation after total laryngectomy by the three procedures of Prof. Staffieri is based on the principle of establishing a short fistula lined by pharyngeal mucosa between trachea and hypopharynx. It permits voluntary air pressure control for voice production while preventing aspiration during deglutition. In a single-stage procedure the “Neoglottis phonatoria” is established on the occasion of the laryngectomy at the top of the trachea. In cases of previous laryngectomy a shunt is established between the trachea dorsal wall and the Oesophagus, either cranially (direct internal shunt) or more caudally (retrograde internal shunt). The three methods are described, and the one-way function of the different fistulas are demonstrated. The percentage of successful voice restorations that remain free from swallowing difficulties in 700 patients, subjected to these techniques in Europe and Overseas is between 60 and 90%.

    Notes: Summary Chronic and diffuse contact between inhalation noxae and the mucosa plays an important role in the pathogenesis of cancer of the larynx and lead us to expect a multilocular development of carcinoma and precancerous changes. This supposition was examined in some 100 larynx resection preparations. After standardized stratigraphic sub-divisioning of the larynx, it was lamellated and the individual zones studied histologically for epithelial dysplasias, varying degrees of differentation up to an including carcinoma, and for inflammatory changes. Certain relationships were found to exist between certain characteristics of growth of the carcinoma and the site, extent and histological classification of the tumour. The fact that, frequently, a macroscopically non-verifiable coexistance of precancer and carcinoma can be found, is taken account of in the modified conception for the taking of biopsies.

    Notes: Summary Different tumorous alterations in the root of the tongue, the epiglottic vallecula and of the posterior wall of the oro-hypopharynx are shown. The different genesis and dignity of macroscopic similar findings in the anterior wall of the oropharynx make a biopsy with histology necessary. Only this enables a safe diagnosis. Protuberances of the oro-hypopharyngeal posterior wall can be clarified mostly by X-ray examination, as they are caused often by cervical vertebrae. In a few cases the biopsy with histology is prerequisite for a diagnosis in this region too.
