Zusammenfassung Ist beim Rezidiv eines Dünndarmileus eine Operation erforderlich, wird deren Vorgehen entscheidend davon abhängen, ob es sich um einen mechanischen, paralytischen oder gemischt mechanisch paralytischen Ileus handelt. Beim mechanischen Ileus sind neben der Beseitigung der Obstruktion die Darmabsaugung über eine Enterotomie und zur Rezidivprophylaxe die Darmschienung durch eine Sonde probate Operationsverfahren. Beim peritonitisbedingten, paralytischen Dünndarmileusrezidiv jedoch sollte die Indikation zur Stomaanlage großzügig gestellt werden.

Summary In case of an operation in recurrent ileus of the small intestine its operative proceeding is dependent on the cause of ileus being mechanical, paralytic or mixed mechanoparalytic. In mechanical ileus, beside removal of the obstruction, decompression by an enterotomy or using a splint with a Miller-Abbot-tube is mostly sufficient. The recurrent paralytic ileus caused by peritonitis however warrants in most cases the construction of a stoma, as this method is able to relieve advanced intestinal paralysis safest and fastest.
