Zusammenfassung Seit 1972 benutzen wir eine an unserer Klinik entwickelte neue Methode des Magenersatzes. Die Passagerekonstruktion erfolgt durch iso-anisoperistaltische Jejunuminterposition zwischen Oesophagus und Duodenom, mit einem Längenverhältnis von 30 cm zu 10 cm. Das gegengeschaltete Segment fungiert als Pylorusersatz und bewirkt eine deutlich verzögerte, intermittierende Entleerung des Interponats. Bislang haben wir das Verfahren bei 64 Patienten angewandt, die Letalität betrug 10,9%. Digestion und Resorption, insbesondere von Fett, sind gegenüber anderen Rekonstruktionsverfahren verbessert.

Summary Since 1972 we have used a new method of gastric replacement developed at our hospital. Digestive tract continuity is reestablished by isoanisoperistaltic interposition of two jejunal segments between the esophagus and duodenum (ratio 30:10 cm). The reversed segment acts as a substitute for the pylorus, causing delayed and intermittent emptying of the interponate. To date we have used this method in 64 patients; mortality was 10.9%. Compared with other methods of reconstruction, digestion and absorption, particularly of fat, are improved.
