    Notes: Introduction: Previous studies have demonstrated gender differences in risk of sudden death in patients with ischemic heart disease. The Multicenter UnSustained Tachycardia Trial (MUSTT) evaluated the ability of therapy guided by electrophysiologic (EP) testing to reduce mortality in patients with coronary disease, ejection fraction ≤40%, and spontaneous nonsustained ventricular tachycardia. Methods and Results: We analyzed the influence of gender on results of EP testing and outcome of patients enrolled in MUSTT. Women made up 14% of the overall MUSTT population and were less likely than men to have inducible sustained randomizable ventricular arrhythmias (24% vs 36%, P 〈 0.001). Baseline characteristics differed between men and women. In randomized patients, women were older, more likely to have had an infarction within 6 months, more likely to have a history of heart failure, and more likely to have recent angina prior to enrollment than men (P 〈 0.05). In the EP-guided therapy group, there was no difference in implantable cardioverter defibrillator implantation rate in men and women (45% vs 53%, P = 0.38). There also were no significant gender influences on risk of arrhythmic death or cardiac arrest (2-year event rate 9% in women and 12% in men, adjusted hazard ratio 0.88) or overall mortality (2-year event rate 32% in women vs 21% in men, adjusted hazard ratio 1.51). Conclusion: The outcome and benefit of EP-guided therapy in this trial did not appear to be influenced by gender. However, due to the small numbers of women in the trial, small differences in outcome may not be apparent. Plans for future primary prevention trials should include careful risk stratification of women who less often have inducible sustained ventricular arrhythmias and better left ventricular function despite more frequent heart failure.

    Notes: Abstract. Nitrogen (N) loss by leaching poses great challenges for N availability to crops as well as nitrate pollution of groundwater. Few studies address this issue with respect to the role of the subsoil in the deep and highly weathered savanna soils of the tropics, which exhibit different adsorption and drainage patterns to soils in temperate environments. In an Anionic Acrustox of the Brazilian savanna, the Cerrado, dynamics and budgets of applied N were studied in organic and inorganic soil pools of two maize (Zea mays L.) – soybean (Glycine max (L.) Merr.) rotations using 15N tracing. Labelled ammonium sulphate was applied at 10 kg N ha−1 (with 10 atom%15N excess) to both maize and soybean at the beginning of the cropping season. Amounts and isotopic composition of N were determined in above-ground biomass, soil, adsorbed mineral N, and in soil solution at 0.15, 0.3, 0.8, 1.2 and 2 m depths using suction lysimeters throughout one cropping season. The applied ammonium was rapidly nitrified or immobilized in soil organic matter, and recovery of applied ammonium in soil 2 weeks after application was negligible. Large amounts of nitrate were adsorbed in the subsoil (150–300 kg NO3−-N ha−1 per 2 m) matching total N uptake by the crops (130–400 kg N ha−1). Throughout one cropping season, more applied N (49–77%; determined by 15N tracers) was immobilized in soil organic matter than was present as adsorbed nitrate (2–3%). Most of the applied N (71–96% of 15N recovery) was found in the subsoil at 0.15–2 m depth. This coincided with an increase with depth of dissolved organic N as a proportion of total dissolved N (39–63%). Hydrophilic organic N was the dominant fraction of dissolved organic N and was, together with nitrate, the most important carrier for applied N. Most of this N (〉80%) was leached from the topsoil (0–0.15 m) during the first 30 days after application. Subsoil N retention as both adsorbed inorganic N, and especially soil organic N, was found to be of great importance in determining N losses, soil N depletion and the potential of nitrate contamination of groundwater.

    Notes: The use of anti-PrP antibodies represents one of the most promising strategies for the treatment of prion diseases. In the present study, we screened various anti-PrP antibodies with the aim of identifying those that would block PrPSc replication in prion-infected cell culture. Two antibodies, SAF34 recognizing the flexible octarepeats region on HuPrP protein, and SAF61 directed against PrP amino acid residues (144–152), not only inhibited PrPSc formation in prion-infected neuroblastoma cells but also decreased the PrPC levels in non-infected N2a cells. In addition, treatment with both SAF34 and SAF61 antibodies decreased PrPC and PrPSc levels in the cells synergistically. In the presence of both antibodies, our results showed that the mode of action which leads to the disappearance of PrPSc in cells is directly coupled to PrPC degradation by reducing the half-life of the PrPC protein.

    Notes: Abstract In the present article, we report, for the first time, a prenatal diagnosis of a congenital mesoblastic nephroma in combination with a post-partum hyperreninemia with hypertension. A newborn was delivered at 35 weeks gestation who had an intrauterine diagnosis of a renal mass as early as 32 weeks gestational age by ultrasound examination. Tumor nephrectomy was performed on day 11 after delivery when an increase in hypertension was observed in the newborn.

    Notes: It has been hypothesized that cesarean delivery might have an impact on the development of atopic diseases because of its gut flora modulating properties. In the present study, we analysed the association between cesarean delivery and atopic diseases using data of 2500 infants enrolled in the LISA-Study, a German prospective multicenter birth cohort study. Data on symptoms and physician-diagnosed atopic diseases were gathered by questionnaires shortly after birth and at infant's age 6, 12, 18, and 24 months. In addition, sensitization to common food and inhalant allergens was assessed by measuring specific immunoglobulin E (IgE) using the CAP-RAST FEIA method at the age of 2 yr. Confounder-adjusted odds ratios (aOR) with 95% confidence intervals (CI) were calculated by multiple logistic regression. We found a positive association between cesarean delivery and occurrence of at least one episode of wheezing [aOR 1.31 (95% CI 1.02–1.68)] and of recurrent wheezing [1.41 (1.02–1.96)] during the first 2 yr of life. Furthermore, effect estimates for allergic sensitization defined as at least one specific IgE ≥0.70 kU/l against any allergen [1.48 (0.98–2.24)], against food allergens [1.64 (1.03–2.63)], and against inhalant allergens [1.75 (0.98–3.12)] were increased. Symptoms of atopic dermatitis [1.21 (0.92–1.59)], physician-diagnosed atopic dermatitis [1.04 (0.79–1.39)], and symptoms of allergic rhinoconjunctivitis [1.40 (0.80–2.44)] were only marginally increased in children delivered by cesarean section. In conclusion, our results suggest that cesarean delivery may be an additional risk factor for wheezing and allergic sensitization at least to food allergens up to the age of 2 yr. This should be considered when cesarean section is done for other than medical reasons.

    Notes: Skin photodamage due to excessive ultraviolet (UV) light exposure is followed by a series of biochemical and immunologic events that cause inflammation including the release of prostaglandins, lipoxygenase products, cytokines (e.g. tumour necrosis factor-α), adhesion molecule, reactive oxygen radicals and mast cell-derived mediators, such as histamine and tryptase. In various inflammatory pain conditions, including UVB sunburn, nociceptors are sensitized to mechanical and thermal stimuli at the site of inflammation. This phenomenon, which leads to hyperalgesia, can be used to investigate pharmacological effects of different drugs and drug formulations in the model of UVB-induced skin inflammation. Two additional models are useful for evaluating the direct, prostaglandin-independent, modulatory effects on neuronal excitability of nociceptors (chemosensitivity): the chemically induced pain (capsaicin test) and itch (histamine test). IDEA-070 is a novel carrier-based topical dosage form of an analgesic drug that acts by inhibiting cyclooxygenase (COX-1 and COX-2) and lipoxygenase, thereby reducing prostaglandin- and leukotriene-mediated inflammatory reactions. We used all three described models in the framework of a phase I clinical study to assess efficacy of IDEA-070. The results of an interim analysis of the UVB model from the first 12 healthy volunteers indicate that IDEA-070 as well as hydrocortisone-21-acetate (HC) reduces the UVB-induced heat hyperalgesia if applied immediately after UV exposure. In contrast, only IDEA-070 is effective when used 6 h after exposure to UVB. In addition, IDEA-070, but not HC, suppressed erythema development if used immediately after UVB exposure as well as 6 h after UVB irradiation.

    Notes: Abstract:  It has been shown that epidermal keratinocytes have the capacity for the UVB-induced photochemical conversion of 7-dehydrocholesterol to vitamin D3, and also for the enzymatically controlled hydroxylation of the photolysis product. This metabolic loop results in the formation of the biologically active final product 1α,25-dihydroxyvitamin D3 (1α,25(OH)2D3, calcitriol). The epidermal synthesis of calcitriol is of fundamental relevance because calcitriol regulates important cellular functions in keratinocytes and immunocompetent cells. Because of their anti-proliferative and prodifferentiating effects, calcitriol and other vitamin D analogs are highly efficient in the treatment of psoriasis vulgaris. In addition, the known therapeutic effect of UVB light therapy in the treatment of psoriasis may, at least in part, be mediated via UVB-induced synthesis of calcitriol. Increasing evidence now indicates that cutaneous vitamin D synthesis is of great importance for the prevention of a broad variety of diseases, including various malignancies. It has been postulated that cancer mortality could be reduced via careful UV exposure or, more safely, via oral substitution with vitamin D. These new findings must be taken into account when establishing new sun protection guidelines for the prevention of skin cancer. In addition, better understanding of the metabolism of vitamin D in the skin has opened up new perspectives for the therapeutic application of vitamin D analogs, e.g. in inflammatory skin diseases.

    Notes: Germall 115 and Germall II are well known cosmetic preservatives, widely used as such since the early 1970’s and 1980’s, respectively. This investigation reveals that they have mistakenly been regarded as pure chemical entities with specific structures by the majority of the chemical society. In fact Germall 115 and Germall II both consist of complex mixtures of numerous and fast degradable allantoin-formaldehyde condensation products and may not even contain the conventionally assigned chemical structures of imidazolidinyl urea (Germall 115) and diazolidinyl urea (Germall II). Equilibrium between different structures may be formed in different cosmetic formulations/test materials. This may have led to the discrepancies in the literature: Are the Germalls themselves sensitizers or are the allergic reactions caused by formaldehyde release? The identification and structure assignment of (4-hydroxymethyl-2,5-dioxo-imidazolidin-4-yl)-urea (HU) will be presented, as it is the major and chemically stable degradation product of both Diazolidinyl urea and Imidazolidinyl urea. A guinea pig maximization test has been performed to determine the potential of HU to induce delayed contact hypersensibility in guinea pigs. Separate chemically stable TRUE Test patches containing the mixtures have shown clinical efficacy.

    Notes: TRUE Test® is the only standardized patch test system for the diagnosis of allergic contact dermatitis based on 24 fully stable and reproducible formulations. New standardized patches are in process of development for TRUE Test® Panel 3. The allergens chosen are well-known sensitizers and include preservatives, steroids, metals and others. 14 new patches are included in an Experimental panel which is tested in several European clinics. 11 of the patches contain new allergens while 3 patches consist of improved TRUE Test® formulations with Neomycine sulphate, Fragrance mix and Potassium dichromate. A number of the patches from the Experimental panel will be selected for TRUE Test® Panel 3. The overall purpose with pharmaceutical development of patch test materials for TRUE Test® is to obtain a precise dose, bioavailability and stability in every single patch. The patches have to be chemically stable for at least 2 years at refrigerated temperature. The selected preservatives on the Experimental panel are Methyldibromoglutaronitrile, Germall 115, Germall II and Bronopol. All 4 patches have proven to be difficult to stabilize because of the natural reactivity of the allergenic substances. The steroids chosen are Budesonide, Tixocortol-21-pivalate and Hydrocortisone-17-butyrate. All 3 steroids are stable in TRUE Test® formulations. Other allergens on the Experimental panel are Ammoniated Mercury, Goldsodiumthiosulphate, Parthenolide and Disperse blue 106, where the latter consists of a complex mixture. Chemical stabilization of the patches as well as clinical studies will be presented.

    Notes: This paper has three objectives. First, we develop an equilibrium pricing model in which consumers have incomplete information about both product qualities and prices. Specifically, manufacturers can use high prices to signal high quality to uninformed consumers. Furthermore, prices of any given brand can vary geographically across retail outlets. We show that previous models are special cases of our model. Specifically, the hedonic regression model assumes that consumers have full information about all product qualities and prices. Second, we propose a methodology for testing price-signaling models. Third, we test our model using data from consumer reports for several consumer durable and nondurable products. The results show that firms use prices to signal quality, regardless of whether they market durable or nondurable products. The results do not support the popular theory that markets for experience goods are more efficient than those for search goods. Finally, our model outperforms the standard hedonic regression model for four of the five product categories analyzed.
