Zusammenfassung In der vorliegenden Arbeit sollte vor allem untersucht werden, ob die Zusammensetzung des Bronchialschleims Rückschlüsse auf den Krankheitsverlauf der chronisch obstruktiven Bronchitis zulassen könnte. Es wurde das Sputum von 50 Patienten mit schwerer chronisch obstruktiver Bronchitis, teilweise mit emphysematösen Lungenveränderungen, untersucht. Durch Ultrazentrifugation und Ultraschallbehandlung und anschließende Gefriertrocknung konnten 3 Fraktionen aus dem Sputum gewonnen werden. Als Meßgrößen bestimmten wir die expektorierte Sputummenge, das Trockengewicht der einzelnen Fraktionen, die Gesamteiweißmenge, die Zusammensetzung der Serumproteine Albumin, Gamma A, G, M-Globulin und Alpha1-Antitrypsin, das Vorhandensein proteolytischer Fermente und Fermentinhibitoren und die gebundene Neuraminsäure als Maß für den Glucoproteinanteil. Als Maß für die Obstruktion wurden Atemwegswiderstand und intrathorakales Gasvolumen gewertet. Der Eiweißverlust durch Sputumexpektoration lag im Patienten-Mittel bei ca. 130 mg in 12 Std. Die Immunglobuline A und M waren, im Gegensatz zum Gamma G-Globulin im Verhältnis zum Serum erhöht. Der Quotient IgG/IgA war im Sputum ausnahmslos zum IgA verschoben. Alpha1-Antitrypsin war im Sputum im Verhältnis zum Serum verringert. In fast allen Sputen wurden proteolytische Fermentaktivitäten ermittelt. Zusätzlich wurden Fermentinhibitoren gefunden, die unabhängig vom Alpha1-Antitrypsin effektiv waren. Die Neuraminsäurebestimmung im Sputum unserer Patienten ergab eine Verringerung gegenüber den in der Literatur angegebenen Werten für normalen Bronchialschleim. Eine Korrelation der gemessenen Werte mit dem Krankheitsbild konnte nicht gefunden werden.

Abstract The object of the present paper was primarily to investigate whether the composition of bronchial mucus would correlate with the progress of illness of chronic obstructive bronchitis. Sputa were assayed from 50 patients suffering from severe chronic obstructive bronchitis, some of which were associated with emphysematous lung changes. Subjecting the sputa to ultra centrifugation and ultrasonics, followed by freeze drying, we obtained three fractions. We determined the amount of expectorated sputum, the dry weight of the single fractions, the total protein, the composition of the serum proteins: albumin, gammaglobulins A, G, M and alpha1-antitrypsin, the presence of proteolytic ferments and ferment inhibitors, and the bound siliac acid as measures of the glucoprotein portion. Airway resistance and intrathoracic gas volume were taken as measures of obstruction. Total protein loss by sputum expectoration was about 130 mg in 12 h, found to be the mean value of all patients. The immunoglobulins A and M were elevated in proportion to the serum, in contrast to the gamma-globulin G. The quotient IgG/IgA was without exception shifted to IgA in the sputum. Alpha1-antitrypsin in the sputum was reduced in proportion to the serum. In nearly all the sputa proteolytic ferment activities were detected. Additionally, ferment inhibitors were found effective, independent of alpha1-antitrypsin. Siliac acid assay in the sputum of our patients showed a reduction as compared to the values given in the literature for normal bronchial mucus. No correlation of the measured values with the syndrome of this illness has been found.
