Für jede Fragestellung werden Beispiele angeführt.
    Notes: Summary Since the first clinical prospective trial of J. Lind in 1753, the controlled study has proved to be an essential instrument in clinical research. Possible topics include: pharmacologic prophylaxis of complications such as stress ulcer, thrombosis, etc., indications for operation, preoperative preparation, choice of surgical procedure, and techniques or postoperative treatment. Examples for each topic are given.

    Description / Table of Contents: Zusammenfassung Aus dem Zeitraum der letzten 15 Jahre werden 448 Patienten mit schwerer Blutung aus einem Ulcus ventriculi sive duodeni vorgestellt. In 3 Zeitperioden änderten sich Indikation, Organisation und Methodenwahl. Insbesondere kamen die endoskopische Diagnostik und Therapie mit guten Ergebnissen hinzu. Durch die Endoskopie erfolgte eine negative Auswahl für diese chirurgische Blutstillung, so daß die chirurgischen Leistungszahlen deprimiert wurden. Nach wie vor gilt die Forderung nach der Frühindikation und dies auch bei zusätzlicher Belastung.
    Notes: Summary 448 patients with severe haemorrhage from gastroduodenal ulcera during the last 15 years are presented. Within this period indication, organisation and the choice of method have changed in 3 phases. Included are the endoscopic diagnosis and treatment with very good results. As a result of endoscopy there is a negative selection of patients for surgery causing a depression of the results after surgical treatment. The propagation for early indication even in high risk patients still has validity.

Die Verletzlichkeit des Mesothels bei intraoperativer mechanischer Traumatisierung wird lichtmikroskopisch und rasterelektronenmikroskopisch dargestellt. Zu erheblichen Parenchymschäden des großen Netzes kommt es durch das Abschieben von Mesothellappen, das Zerreißen von Bindegewebsstrukturen und das Zerquetschen von Fettzellen; dabei können Lipidmassen in das Interstitium zwischen den Fettzellen und in die submesotheliale Bindegewebsschicht gedrückt werden. Die sog. Aktionsfläche des Netzes scheint zunächst die Unterfläche zu sein.
    Notes: Summary The study deals with clinically relevant morphological structures of the greater omentum. The vulnerability of the mesothelioma during intraoperative mechanical traumatization is shown with light-microscopic and grid electron-microscopic pictures. Important damage to the parenchyma of the greater omentum happens through shoving off the mesothelial layer, rupture of connective tissue structures, and the crushing of fat cells. In this way, the fat mass is pressed into the interstice between the fat cells and into the submesothelial connective tissue. The first action area of the omentum seems to be the dorsal layer.

    Description / Table of Contents: Zusammenfassung Das Recht verlangt vom Arzt die „erforderliche Sorgfalt” bei der Behandlung. Die Judikatur verweist dafür auf den dem Arzt bei zumutbaren Anstrengungen verfügbaren Stand der medizinischen Wissenschaft. Abzustellen ist auf den durchschnittlich sorgfältigen Arzt. Der Standard enthält normative und tatsächliche Elemente: „die gute ärztliche Übung”. Es stellt sich aber die Frage, wer den Standard für die Felder der chirurgischen Tätigkeit bestimmt. Das Recht verweist auf die medizinische Wissenschaft, setzt aber Mindesterfordernisse. Der bisherige Standard hindert nicht die Erprobung eines neuen Verfahrens mit zumutbaren Risiken.
    Notes: Summary German law requires “due diligence” of the physician. The courts refer to the state of medical science that is available to a physician who makes every reasonable effort to obtain it. The relevant qualification is to be the skill of an “average” physician. The standard contains normative and factual elements: “good medical practice” The question, however, arises, who establishes thestandards for the fields of surgery. The law refers to medical science but lays down minimum requirements. The prevailing standard does not prevent new techniques from being tested, but when they become the standard, cannot be determined in general.

    Description / Table of Contents: Zusammenfassung Beim Makrocarcinom sind die Behandlungsergebnisse sei über 10 Jahren weitgehend konstant. Die absoluten 5-Jahres-Heilungsergebnisse bewegen sich zwischen 10 und 15 %. Anders die Ergebnisse beim Frühcarcinom. Hier gibt es 5-Jahres-Heilungsquoten bis 90 % und mehr. Daraus resultiert die systematische Suche nach dem Frühcarcinom. Praktisch geschieht dies durch regelmäßige Kontrolle der gefährdeten Gruppen. Das Behandlungsverfahren der Wahl ist für Makro- und Frühcarcinom die subtotale Resektion. Die Kardiaresektion sollte durch die Gastrektomie ersetzt werden und die Gastrektomie definierten Befunden vorbehalten bleiben.
    Notes: Summary For over a decade, therapeutic results in cases of macrocarcinoma have remained almost constant. The absolute 5-year survival rate is 10-15 %. However, in cases of early detection, the 5-year survival rate is over 90 %. Systematic screening to detect the cancers early has resulted. The practical results is the systematic observation of high-risk groups. The therapy of choice for macrocarcinoma and cancer that is detected early is subtotal resection. Resection of the cardia should be replaced by gastrectomy, which should also be performed only when defined diagnostic criteria are met.

    Notes: Pyrolysis of polymer wastes appears to be a suitable means of dealing with an increasingly important environmental problem. To provide background on the pyrolytic behavior of wastes, model mixtures of polyethylene and polystyrene have been pyrolyzed in various gas phases including nitrogen and carbon monoxide, at atmospheric and at reduced pressures and at temperatures from about 300-900°C. When the pyrolyzing temperature is attained by gradual heating, and when pyrolysis occurs at pressures in excess of about 200 torr, the process occurs as if each component were present alone - that is, no evidence for component interaction was noted. In these cases, the known mechanisms for pyrolysis of the pure polymers can be applied to the behavior of mixtures. Complicating effects due to interactions between the starting polymers, or among the pyrolysis products, were observed in low pressure pyrolysis (≲20 torr); further, it was shown that the balance of pyrolysis products depends on rates of attaining the pyrolyzing temperature, more rapid rates favoring the production of carbon-rich solid residues of possible use as carbon blacks. More detailed understanding of pyrolytic processes in polymer mixtures is needed to permit selection of conditions producing the optimum balance of useful products.
