    Notes: To investigate the differentiation and activation of monocytes, the combined effects of 1,25-dihydroxyvitamin D3 (D3) and IL-4 on human blood monocytes were examined with respect to expression of MHC class-II antigens, accessory activity, and phagocytic capacity. IL-4 was reported lo upregulate the expression of MHC class-II antigens and accessory activity of monocytes. The experiments described here demonstrate that D3 inhibits the expression of all three subtypes of MHC class-II antigens (HLA-DR, -DP and -DQ) as well as the accessory activity of monocytes. both in a dose- and time-dependent manner. However, D3 enhances the immunoglobulin- and complement-dependent phagocytosis by monocytes in a dose- and time-dependent manner. When monocytes are treated with both IL-4 and D3, the effects of D3 are reverted by IL-4. suggesting that IL-4 induces the development of monocytes into accessory cells, whereas D3 stimulates differentiation of monocytes into classical macrophages. These findings provide further evidence for the contention that, depending on defined stimuli, monocytes may develop either into accessory cells or into classical macrophages.

    Notes: Summary The impact of improved glycaemic control on renal function in newly-presenting Type 2 (non-insulin-dependent) diabetic patients has not been adequately researched. Consequently, glomerular filtration rate and effective renal plasma flow and urinary albumin excretion rates were determined in 76 subjects (age (mean (SD)): 54 (9.5) years; 50 male) of an original cohort of 110 newly-presenting normotensive non-proteinuric Type 2 diabetic patients following 6 months treatment with diet alone (n=42) or with oral hypoglycaemic agents (n=34). Significant reductions were observed in (presentation vs 6 months): body mass index (p〈0.01); fasting plasma glucose (p〈0.001); glycated haemoglobin (HbA1) (p〈0.001); systolic blood pressure (p〈0.01); and diastolic blood pressure (p〈0.001). Glomerular filtration rate declined from 117 (22) to 112 (21) ml·min−1 (p〈0.01), with unchanged effective renal plasma flow (534 (123) vs 523 (113) ml·min−1) and filtration fraction (22.4 (3.0) vs 21.8 (3.4)%). Albumin excretion rate (median (range)) declined from 1.1 (0.1–34.7) to 0.5 (0.1–29.9) μg·min−1 (p〈0.01). Changes in glomerular filtration rate (Δ values) were inversely correlated with presentation values (p〈0.001), and positive relationships were observed with Δ effective renal plasma flow (p〈0.01), and Δ glycated haemoglobin (p〈0.05). Type 2 diabetic patients with glomerular filtration rate values at presentation over 120 ml·min−1 demonstrated significant reduction in glomerular filtration rate (n=31; p〈0.001), whilst those with original values less than 120 ml·min−1 remained unchanged (n=45). Glomerular filtration rate, effective renal plasma flow and filtration fraction for the Type 2 diabetic patients remained elevated compared with age-controlled normal subjects (p〈0.01-0.001). Albumin excretion rate at presentation and 6 months were positively correlated with fasting plasma glucose levels (p〈0.05) but not renal haemodynamics. Thus, glomerular filtration rate and albumin excretion rate in newly-presenting Type 2 diabetic patients are influenced by metabolic control. Improved glycaemia for 6 months produces a reduction in glomerular filtration rate, mainly in the younger patients with values greater than 120 ml·min−1 at diagnosis of diabetes. Despite these changes, renal haemodynamic parameters remain elevated compared with age-matched normal subjects.

    Notes: Summary Stones are sometimes spilled at the time of cholecystectomy. Retrieval may be difficult, especially during laparoscopic cholecystectomy. Little is known about the natural history of missed stones which are left behind in the peritoneal cavity. We present a case in which a patient developed an intraabdominal abscess around such a stone. The abscess recurred after drainage and removal of the stone was needed for resolution. This case suggests that care should be taken to avoid stone spillage, and that stones which are spilled into the abdomen should be retrieved.

    Notes: Abstract Dutch integral water management aims to restore and maintain healthy aquatic ecosystems, which can sustain most human uses. Clear ecological objectives are necessary. Sustainability of fundamental ecological values (production and yield, species diversity, and self-regulation) is assumed to be best preserved in the reference situation. This is the situation, in which human influence is minimal. A general method to present ecological information, the Amoeba approach is discussed. This approach uses selected species as representatives for the ecosystem. Criteria for the selection of species are discussed. For each species (target variable) the current situation, the reference situation, and estimated effects of measures can be found and plotted in an amoeba-like figure. The presentation is a simplification and is therefore easily understood by managers, politicians, and the public. It's application for some Dutch inland waters is discussed.

    Notes: Summary In healthy humans, the increase in arterial blood pressure seen in patients with autonomic dysfunction in response to exogenous vasopressin (AVP) is abolished. We tested the hypothesis that redistribution of blood from the intra- to the extrathoracic vascular compartment might contribute to this buffer response. Regional distribution of99mTc labeled autologous red cells was assessed in healthy supine volunteers (n=7) during arginine-vasopressin administration (1 ng·kg−1 bolus i.v. followed by a 14-min infusion of 3 ng·kg−1·min−1), along with arterial and central venous pressures, and heart rate. Exogenous vasopressin increased plasma vasopressin concentration from 4.0±1.4 SEM to 91 pg·ml−1±12. Thoracic counts increased slightly but significantly by 2.2%±0.9, while global abdominal counts remained unchanged. Most surprisingly, counts in the liver markedly increased (+8.1%±1.8, p=0.02), but significantly decreased in the spleen (−3.1%±1.4). Intestinal (−2.5%±2.4) and limb counts did not change significantly. Consistent with the increase in thoracic counts centralvenous pressure increased from 3.6 mm Hg±1 to 4.7±1 (p=0.02), while arterial pressure and heart rate did not change. All changes reversed towards baseline when vasopressin administration ceased. Thus, in humans with an intact autonomic system, vasopressin, at concentrations observed during hypotension, increases liver and, albeit to a small extent, also thoracic blood volume, but decreases splenic blood content. These results 1) are incompatible with the hypothesis that AVP induces a shift of blood from intra- to extrathoracic capacitance vessels, and 2) show that AVP increases rather than decreases central blood volume.

    Notes: Summary It has long been recognized that fabric, i.e., the grain configuration in a domain, plays a significant role in the constitutive response of granular media. Here we establish that this effect has bearing on the suitability of non-associative plasticity laws that describe the observed deviation of the plastic strain rate, from the normal to the yield surface. It is known that when non-associated plasticity laws are used, uniqueness of the solution of the initial value problem cannot be proved. Here it is shown that the introduction of configurational entropy and temperature into the theory, gives rise to a thermodynamic framework, specifically athermoplastic potential, with respect to which the plasticity law is associative. However, the inelastic strain is the sum of a plastic and a configurational strain. While the plastic strain rate is still normal to the potential (yield) surface, the inelastic strain rate,which is what is measured in the laboratory, is not. It is further shown that despite the non-normality of the inelastic strain rate to the plastic potential surface, the solution of the coupled initial value — configurational diffusion problem is unique. In the process of development of the underlying configurational thermodynamics, the concept of configurational flux is introduced and a time indifferent configurational diffusion law is inferred on the basis of uniqueness requirements. Thus the foundation is laid for the mode of propagation of chaos in granular media.
