Die Untersuchung zeigt, daß die Pulsoximetrie zur Qualitätskontrolle der durchgeführten Wiederbelebungsmaßnahmen in einem hohen Prozentsatz geeignet erscheint. Es werden die beeinflußbaren physikalischen Parameter der Reanimation aufgeführt. Die Pulsoximetrie stellt eine sinnvolle Überwachungsmaßnahme unter der Reanimation dar und sollte so früh wie möglich eingesetzt werden.

    Description / Table of Contents: Zusammenfassung. In einem 2,5 jährigen Zeitraum wurden 55 Patienten mit einer frischen Achillessehnenruptur (ASR) nach definierten sonographischen Kriterien operativ und funktionell konservativ behandelt. Durchschnittlich 2,4 Jahre später konnten von 51 Patienten (operative Gruppe = 28 Patienten, konservative Gruppe = 23 Patienten) erneut klinische, funktionelle sowie isokinetische Daten der eingelenkigen Plantarflexoren erfaßt werden. Während in der operativen Gruppe insgesamt bei 10,6 % Wundheilungsstörungen mit spontanem Heilungsverlauf auftraten, betrug die Rerupturrate in der konservativen Gruppe 13 %. Die Incidenz an Achillodynien war zum Nachuntersuchungszeitpunkt in der nicht operativen Gruppe deutlich höher (p = 0,019). Durch die Cybextestung wurden durchschnittliche Kraftdefizite in der operativen Gruppe von 13,7 % und nach funktionell konservativer Behandlung von 24,7 % (p = 0,012) festgestellt. Während die Adaptationsnaht i. d. R. zum Erfolg führt, ist die Indikation zur konservativen Therapie der ASR von der Sehnendiastase, dem Aktivitätswunsch und der Kooperation abhängig.
    Notes: Summary. According to sonographic criteria, 55 patients underwent surgical treatment with mobilisation in a lower leg plaster or conservative treatment with early mobilisation in heel pad (3 cm) shoes when an acute Achilles tendon rupture (ATR) was diagnosed. The follow-up period in 51 patients was 2.4 years (operated group = 28 patients, conservative group = 23 patients) with clinical examination and testing of isokinetic muscle strength in knee joint flexion. After surgical treatment, minor wound infections occurred in 10.6 %. Reruptures occurred in 13 % of the conservative group. Following conservative treatment, the rate of stress-related achillodynia was significantly higher (P = 0.019). In the operated group mean isokinetic muscle strength was 13.7 % lower than in the uninvolved leg and decreased significantly with non-operative treatment to 75.3 % (P = 0.012). We recommend surgical treatment of acute ATR. The indications for conservative treatment depend on the extent of the rupture (measured by ultrasound in the equinus position), the desired level of daily activity and the patient's degree of compliance.

    Description / Table of Contents: Sixty-three patients with rigid equinovarus contractures of the foot following ischemic episodes in the lower leg were treated at our institute from 1983 to 1994 by lengthening the Achilles tendon and the tendon of the m. tibialis posterior, release of the tendons of the m. flexor digitorum longus and the m. flexor hallucis longus and release of the dorsal capsule of the ankle joint. Patients with an equinus deformity greater than 20°, with an additional hind foot varus deformity of more than 5° and/or malrotation of the midfoot were not eligible for this procedure. The initial equinus deformity ranged from 7° to 20° (mean 14°). The clinical and radiological results of 41 patients were evaluated retrospectively with a minimum follow-up of 1 year (mean 3.4 years). The overall results were evaluated according to a modified score of Angus and Cowell. Results were good in 60.9%, fair in 29.3% and poor in 9.8%. The range of motion of the ankle joint and the subtalar and midtarsal joints could not be improved. Postoperative complications were observed in 8 patients, one intraoperative lesion of the posterior tibial artery occurred, one avulsion fracture of the anterior tibial metaphysis and one compression syndrome of the tibial nerve. One patient had an initially incomplete correction with a remaining equinus deformity of 10°, and two recurrences of the foot deformity after initially correct position were observed. Furthermore, two hematomas and two soft-tissue infections required surgical revision. These complications may have been due to the preoperatively scarred soft tissue at the medial aspect of the hind foot and a residual postoperative soft-tissue defect after the correction of the foot deformity had been achieved. In conclusion, the technique described is effective in correcting mild pes equinovarus deformities after ischemic episodes in the lower leg. If the pes equinus deformity is greater than 20°, corrective osteo-tomies of the hind foot should be performed instead.
    Notes: Im Zeitraum von 1983 – 1994 wurden am ,,Bergmannsheil`` Bochum 63 Patienten wegen eines ,,Kurzfußsyndroms`` nach pathologischer Druckerhöhung im Unterschenkelkompartment durch Verlängerung der Achillessehne und der Sehne des M. tibialis posterior sowie Durchtrennung der Sehnen der Mm. flexor hallucis longus und flexor digitorum longus mit dorsaler Kapselotomie des oberen Sprunggelenks therapiert. Die Indikation zu diesem Eingriff war eine Spitzfußstellung bis zu 20°, ein Rückfußvarus bis 5°. Als Kontraindikation wurde eine Supinationsfehlstellung in Höhe des Tarsus oder Metatarsus gesehen. 41 Patienten standen mindestens 1 Jahr nach dem Korrektureingriff (im Mittel 3,4 Jahre) zu einer klinischen und radiologischen Beurteilung zur Verfügung. Die durchschnittliche Fehlstellung betrug 14° vor Operation bei einer Spanne von 7°-20°. Nach modifizierten Kriterien von Angus und Cowell wurden insgesamt bei 60,9% ein gutes und bei 29,3% der Patienten ein befriedigendes Resultat erzielt. Lediglich bei 4 Patienten (9,8%) war das Ergebnis unbefriedigend. Ursache war in 3 Fällen ein Rezidiv der Fehlstellung, bei einem weiteren eine initial unzureichende Korrektur mit verbliebener Spitzfußstellung von 10°. Bei insgesamt 8 postoperativen Komplikationen überwogen Wundheilungsstörungen. Diese sind auf die häufig schlechten präoperativen Weichteilverhältnisse und den problematischen Weichteilverschluß nach Aufdehnung des Fußes in die korrekte Stellung zurückzuführen. Aufgrund unserer Ergebnisse kann daher die ischämieinduzierte, isolierte Fußfehlstellung mit einer weichteilverlängernden Operationstechnik nur dann erfolgreich angewandt werden, wenn die initiale Spitzfußstellung 20° nicht überschreitet und keine weiteren entscheidenden Fußdeformitäten vorliegen. In allen anderen Situationen empfiehlt sich die Korrekturosteotomie in Höhe des Rückfußes.

    Description / Table of Contents: The use of pulse oximetry during pre-hospital cardiopulmonary resuscitation was prospectively investigated in 23 cases. The question was whether the measurement of oxygen saturation during pre-clinical resuscitation is useful and what conclusions are possible. We were able to obtain confidential data for oxygen saturation in 82.6% (n = 19) of cases. Our results show that pulse oximetry could be used as a means of quality control during resuscitation. It is a useful part of pre-hospital monitoring and should be used during resuscitation as early as possible.
    Notes: Die Anwendung der Pulsoximetrie im Rahmen der kardiopulmonalen Reanimation wurde unter 23 präklinischen Reanimationen prospektiv untersucht. Es wurde den Fragen nachgegangen, welche Rückschlüsse die unter der Reanimation gewonnenen O2-Sättigungswerte zulassen und welche Konsequenzen sich aus diesen Erkenntnissen ergeben. Die Untersuchung zeigt, daß die Pulsoximetrie zur Qualitätskontrolle der durchgeführten Wiederbelebungsmaßnahmen in einem hohen Prozentsatz geeignet erscheint. Es werden die beeinflußbaren physikalischen Parameter der Reanimation aufgeführt.     Notes: Eighty-one patients (83 shoulder joints) underwent surgical procedures for recurrent shoulder joint dislocations. All patients had an extra-articular osteotomy with subcapital derotation according to Weber's technique. In all, 46% of the patients underwent additional intra-articular reconstructions. After a mean follow-up of 4.6 years, redislocations (6%) were observed only when a singular derotation technique had been used. Complications were minor (2.4%). External rotation of the operated shoulder joints was normal in most patients with a mean of 48.5°. Improvement in sporting activities was seen in 80% and in working activities in 70%. The mean Constantscore was 89.2. Subacromial pain was rare and correlated with reduced scores in functional strain tests postoperatively. There was no postoperative arthritis owing to the derotation technique. Because of unrestricted rotation and early functional therapy, we observed a good indication for an additional derotation technique when a Hill-Sachs defect is evident.

    Notes: Abstract The clinical course of renal cell carcinoma (RCC) is highly variable. Overexpression of the p53 protein has been suggested as a possible prognostic parameter in RCC. Overexpression of the mdm-2 oncogene product has been shown to interact with the p53 function. To investigate the immunohistochemical overexpression of mdm-2 protein in comparison with that of p53 protein in RCC, 50 nonpapillary pT3 RCCs were immunostained for p53 protein (DO-7) and mdm-2 (1172). Tumor growth fraction (Ki-67 labeling index; MIB-1) was determined by immunohistochemistry. p53 positivity was detected in 16% of tumors. mdm-2 overexpression was seen in 30% of RCCs. There was a significant association between p53 and mdm-2 immunostaining (P=0.0006), suggesting that mdm-2 protein may contribute to p53 protein stabilization in RCC. p53 overexpression was associated with a high Ki-67 LI (P=0.0002), suggesting that p53 overexpression is involved in growth control in RCC. Survival analysis showed that Ki-67 LI (P=0.04) and p53 overexpression were associated with poor prognosis (P=0.0021), whereas mdm-2 overexpression was not related to patient outcome (P=0.73). A Cox regression analysis revealed tumor stage (P〈0.001) and p53 overexpression (P〈0.05) to be independent prognostic parameters. It is concluded that p53 but not mdm-2 may be of practical relevance in predicting patient prognosis in RCC.

    Notes: Abstract Detection of molecular alterations is of potential significance for diagnosis and prognosis in bladder cancer. Fluorescence in situ hybridization (FISH) allows visualization and quantitation of genes and chromosomes on a cell by cell level and can easily be applied to urinary cells. To evaluate the sensitivity of FISH for detection of DNA aberrations in bladder cancer, formalin-fixed tissues of 293 tumors were examined by FISH and flow cytometry (FCM). Centromere probes for the chromosomes X, Y, 1, 7, 9, and 17 were used for FISH analysis. FISH was more sensitive for detection of quantitative DNA aberrations than FCM. An aberration of at least one chromosome was found in 107 of 108 tumors (99%), which were tetraploid, aneuploid, or multiploid, and in 29 of 49 tumors (59%), which were diploid, by FCM. The frequency of FISH aberrations showed greater differences between pTa (47%) and pT1 tumors (85%;P〈0.0001) than between stages pT1 and pT2-4 (98%). The marked genetic difference between pTa and pT1 tumors argues against the concept of grouping pTa and pT1 tumors together as “superficial bladder cancer.” The frequency of tumors with chromosomal aberrations detected by FISH increased with the number of chromosomes examined. Aneusomy was seen in 68% of grade 1 tumors examined for ≥4 chromosomes, suggesting that the cytological diagnosis of bladder cancer recurrences could be substantially improved by FISH.

    Notes: Abstract Three maize storage systems, combined with insecticide treatment, were tested in three trials at the same location in southern Togo. Trials were carried out in the storage season 1989/90, 1990/91 and 1992/93. Maize was stored in traditional granaries as cobs with husks, in cribs as dehusked cobs in bulk and as shelled grain in sacks. After 8 months of storage damage and losses of stored maize and insect infestation were evaluated. Storage of loose grain in sacks gave the best results with lowest insect infestation and smallest damage and losses. Highest damage and losses occurred in cribs, whereas intermediate results were obtained in granaries. The treatment with an insecticide was most effective in traditional granaries. In cribs the insecticide treatment was less effective. No differences were found in treated sack variants because of very low pest infestation.

    Notes: Nanometre-period nickel-carbon multilayers were used as a writing medium for nanostructure fabrication induced by gap voltage manipulations in a scanning tunnelling microscope. The choice of immiscible sample materials provided high-stability written metallic structures. Two well-distinguished hillock-like nanostructure types were observed, depending on the tip-sample separation, polarity and interaction time. Relatively slow local annealing under positive sample potential without a direct tip-sample contact resulted in the formation of nanostructures of 〈20 nm wide and a few nanometres high. Rapid melting followed by metal melt extrusion was observed if the tip contacted the sample during the nanostructure formation, independent of the polarity of the applied voltage. These metallic-like structures were tens of nanometres high and had a good electronic contrast to the initial carbon-coated surface. No traces of tip material were found in the processed areas. The shape of the nanostructures and the threshold voltage of their formation were strongly dependent on the tip condition. Possible courses of nanostructure formation are discussed.© 1997 John Wiley & Sons, Ltd.
