    Notes: Coronary flow reserve (CFR) and fractional flow reserve (FFRmyo) are two guidewire-based methods currently used to assess the functional severity of coronary artery lesions. Acquiring both measurements simultaneously may provide complementary information, but would require the passage of two different guidewires and complex instrumentation for their calculation. This study assessed the procedural safety and performance of a novel personal computer-based algorithm, the SmartFlow Intravascular Processor (SFIP), which utilizes a single conventional pressure wire for obtaining simultaneous CFR and FFRmyo measurements for the assessment of coronary artery lesion severity. In 20 consecutive patients with 21 lesions, pressure-derived CFR, FFRmyo and SFIP-FFRmyo measurements were obtained during adenosine-induced hyperemia. Intravascular ultrasound and quantitative coronary angiography lesion analysis was done off-line at the Washington Core Laboratory. Mean FFRmyo was 0.83 ± 0.11, SFIP-FFRmyo was 0.86 ± 0.06, and CFR was 1.74 ± 0.46. Pairwise correlation analysis showed excellent correlation between the FFRmyo and the FFRmyo-SFIP (r2= 0.83, P 〈 0.0001) and a fair degree of relationship between the CFR and minimal lumen diameter (r2= 0.43, P = 0.07). We could not find a correlation between CFR and FFRmyo (r2= 0.22, P = 0.37), between CFR and FFR-SFIP (r2= 0.05, P = 0.86), or between the hemodynamic measurements and any of the IVUS-derived measurements. In conclusion, the SFIP is u novel algorithm for obtaining simultaneous CFR and FFRmyo that may provide valuable information for the assessment of lesion severity and clinical decision making.

    Notes: A 44-year-old Caucasian man was admitted in December 1995 for the treatment of generalized skin pruritus. The patient had a history of cirrhosis felt to be caused by chronic ethanol abuse and renal insufficiency. He had undergone orthotopic liver transplantation in January 1991. Physical examination revealed numerous, fleshy, nontender nodules protruding from the face ( 〈link href="#f5-1"〉Fig. 1), trunk, and extremities, as well as axillary freckling. The patient was certain that the lesions had appeared 2–-3 months after his liver transplant. Wood's lamp examination revealed several café-au-lait macules. Ophthalmic examination was remarkable for Lisch nodules ( 〈link href="#f5-2"〉Fig. 2).〈figure xml:id="f5-1"〉1〈mediaResource alt="image" href="urn:x-wiley:00119059:IJD841-5:IJD_841_f5-1"/〉A representative clinical lesion as a fleshy, non-tender nodule on the neck. Also a few very small lesions are barely visible on the mandible line〈figure xml:id="f5-2"〉2〈mediaResource alt="image" href="urn:x-wiley:00119059:IJD841-5:IJD_841_f5-2"/〉Iris hamartomas (Lisch nodules) in our patientTwo skin nodules on contralateral sides of the body were biopsied. Histologic examination of the nodules revealed a normal epidermis and a nonencapsulated dermal mass composed of thin, wavy eosinophilic fibers lying in loosely textured strands. Interdispersed among these strands were cells with spindle-shaped or oval nuclei and an increased number of mast cells. There was no family history of neurofibromatosis.The patient's pruritus resolved with UVB light therapy and hemodialysis. Currently, the patient is on hemodialysis with no clinical progression of his neurofibromatosis.

    Notes: Calmodulin is a ubiquitous highly conserved calcium binding protein involved in cell signalling. Previous studies in our laboratory suggested a role for calmodulin in prepenetration morphogenesis in Colletotrichum trifolii, the causal agent of alfalfa anthracnose. In this report, we describe the cloning, sequencing and partial characterization of the calmodulin gene from C. trifolii. The gene is present as a single copy in the genome of C. trifolii and its predicted amino acid sequence shows considerable homology to other fungal calmodulins. The gene is most highly expressed during conidial germination and appressorial development. Using a Neurospora crassa inducible promoter driving the calmodulin gene in antisense orientation, transformants were obtained with constitutive levels of antisense calmodulin expression. Upon induction, transformants did not develop appressoria and were not pathogenic on alfalfa plants.

    Notes: Abstract. We have identified S60, a new Cryptosporidium parvum sporozoite surface glycoprotein. S60 was cleaved into two subunits, S16 and S45, approximately 16–18 and 45–47 kDa respectively, with cleavage occurring at an SRSRR motif likely to be sensitive to trypsin in vivo. Analysis by surface biotinylation, lectins and monoclonal antibodies suggests S60 is an abundant sporozoite surface glycoprotein that is shed by migrating sporozoites. The major glycosylation on S60 was identified as single O-linked N-acetylgalactosamine sugars on threonine and serine residues. The gene encoding the S60 protein was identified and revealed a C-terminal consensus sequence for the addition of a glycosylphosphatidyl inositol anchor. Antisera raised against recombinant S60 produced in Escherichia coli reacted with the surface of sporozoites and the inner wall of excysted oocysts.

The purpose of this study was to evaluate changes in muscular strength and endurance, work capacity, and subjective fatigue following surgical treatment of primary hyperparathyroidism (pHPT), and to assess whether changes in muscular function were due to changes in activation of the muscles. A prospective consecutive study design was used, and patients surgically treated for nontoxic goiter served as controls. Nineteen female patients with mild to moderate pHPT and 20 controls were included. Maximal isometric handgrip and quadriceps strength, quadriceps endurance (intermittent stimulation), and quadriceps activation (superimposed twitch technique) were used for evaluation of muscular function. All patients were operated on successfully. Knee extension strength increased by 17 ± 17% (mean ± SD; p= 0.0004) in the patients, whereas no change was observed in the controls. The relative strength increase correlated positively to patient age at operation (r= 0.52, p= 0.02). Handgrip strength, quadriceps endurance, and general work capacity did not change in any group after operation. Subjective fatigue was preoperatively higher in patients than in controls (p= 0.01), and decreased postoperatively to the level of controls. In conclusion, women with pHPT increase knee extension force after parathyroidectomy as a result of increased force generation capacity of the muscle. If change in physical performance is a determinant for change in subjective fatigue in pHPT after operation, then change in strength of the quadriceps muscle seems to be of primary importance, whereas handgrip strength, muscular endurance, and work capacity do not seem to be important. The cause of the increasing strength benefit with increasing age at operation as found in this study needs further investigation.

    Notes: ABSTRACT¶We tested the hypothesis that Bis1, a gene involved in seizure regulation in mice which has been localized to the distal part of chromosome 4, was the same as the gene Kcnab2, encoding the β2-subunit for voltage-dependent K+ channels. Two facts suggested this hypothesis: Kcnab2 is located in the 3.1-cM confidence interval containing Bis1 and many studies have shown an involvement of K+ channels in the genesis of seizures. DNA sequence analysis of the coding sequence for Kcnab2 from JE/Le mice revealed no structural alterations which might affect Kcnab2 function. However, several nucleotide changes were observed.

    Notes: Paspalum notatum Flugge) can be increased by burning the sward and applying N fertilizer at the proper time during the growing season. Our objective was to determine the optimum calendar month (photoperiod) for managing `Argentine' bahiagrass pasture for seed production. Triplicate paddocks of a 10-yr-old Argentine bahiagrass pasture established on a Pomona fine sand (sandy, siliceous, hyperthermic, Ultic Alaquods) were grazed or not grazed (main plots) from October–February, 1988–89 and 1989–90, in a randomized complete-block design. Cattle were removed from the area on 25 February. Then, on the 25th of February, March, April, and May (subplots) each year, portions of residue (sub-subplots) were burned or mowed in both grazed and ungrazed fields. The residue removal dates corresponded with day lengths of 11.5, 12.3, 13.1, and 13.7 h, respectively. Immediately after residue removal, sub-sub-subplots (2.4 by 6.1 m) were fertilized with 0, 100, or 200 kg N ha−1. Mature raceme density, seed yield, and seed germination (1989 only) were determined in 1989 and 1990. Grazing did not affect any measured trait. Burning increased seed yield only for the 25 May 1990 treatment. Date of residue removal and N rate influenced (P 〈 0.0001) the number of mature racemes and seed yield interactively (P 〈 0.05). Nitrogen rate increased (P 〈 0.05) all measured traits in a quadratic manner when applied only during late-April to late-May. Results suggest that seed production from Argentine bahiagrass pasture in the subtropics could be enhanced with management implemented at day lengths 〉13.0 h.
