    Notes: Tussometry involves a continuous measurement of airflow produced by a cough manoeuvre displayed as an airflow-time wave. There is a rapid rise to its peak (cough peak flow rate) and the time taken to reach the peak (peak velocity time) is determined by the laryngeal opening at the onset of cough. Cough peak flow and peak velocity time were measured in 10 healthy volunteers before and after topical anaesthesia of the larynx with lignocaine 100 mg sprayed under indirect laryngoscopy. Adequacy of anaesthesia was established by touching the cords deliberately with a fibreoptic laryngoscope. All subjects had excellent anaesthesia of the larynx. However, cough peak flow rate and peak velocity time remained unchanged following topical anaesthesia. We conclude that topical anaesthesia of the larynx does not impair the motor performance of the vocal cords during a voluntary cough.

    Notes: One hundred patients (69 female) undergoing surgical excision of three or more wisdom teeth were randomly allocated to receive either thiopentone or propofol for induction of anaesthesia. Other than the induction agent, the anaesthetic regimen was standardised for all cases. All patients were observed for 15 min after entry into the recovery area to assess the presence and intensity of shivering. Twenty-five patients in the thiopentone group (n = 50) and 11 patients in the propofol group shivered postoperatively (p 〈 0.005). There was no statistically significant difference in axillary temperature between shivering and non-shivering patients. The use of propofol as an induction agent is associated with a lower incidence of postoperative shivering than thiopentone.

    Notes: A significant reflex fall in mean arterial blood pressure from baseline values was observed during extra-ocular muscle traction in rabbits. This effect could be abolished only by retrobulbar block (afferent pathway block) and not by vagotomy, intravenous atropine or glvcopyrronium, suggesting that it is distinct from, and independent of, the oculocardiac reflex. We suggest that these reflex changes in blood pressure be known as the ‘oculodepressor reflex’.

    Notes: Circannual variation in plasma levels of testosterone (T), oestradiol-17β(E2) and 17α-hydroxy-progesterone (17α-OHPg) were measured in female, Heteropneustes fossilis. T and E2 levels increased during the preparatory phase, reached their peak in the early prespawning phase and fell during the late prespawning phase to reach their lowest levels post-spawning. 17a-OHPg was detected from the late preparatory to the late spawning phase showing its peak during the early spawning phase. The levels of free fatty acids (FFA), monoglycerides (MG), diglycerides (DG), triglycerides (TG), phospholipids (PL), free cholesterol (CF)and esterified cholesterol (CE) were estimated in liver, plasma and ovaries. The preparatory phase showed hepatic lipogenic activity while during the prespawning phase TG lipolysis was increased by FFA. Ovarian CF was depleted by enhancing plasma E, levels during the prespawning phase. The gonadosomatic index reached its peak during spawning.

    Notes: The effect of the addition of a single dose of 7 μg.kg−1 of alfentanil to a propofol/enflurane anaesthetic on the quality of anaesthesia and recovery was assessed. A total of 53 ASA grade 1 or 2 patients who underwent daycase dental surgery were allocated randomly to receive either alfentanil or saline. The study was blinded so that neither the anaesthetist nor the assessor was aware of which solution had been given. Patients in the alfentanil group took signficantly longer to recommence spontaneous ventilation (p = 0.035). Both techniques provided good quality of anaesthesia. Postoperative morbidity was common (45% of patients), but there was no difference between the groups. The rate of recovery was similar in the two groups and no patient required hospital admission after the final assessment at 3 h. Drowsiness was only mild to moderate in both groups at 24 h. A single dose of alfentanil can be administered safely as part of a daycase anaesthetic without increasing morbidity, although there appears to be little advantage in doing so.

    Notes: ‘BJ 104’ was the most widely grown pearl millet hybrid in India until it became susceptible to downy mildew (DM) in 1984—85. Residual variability for resistance was found in both parental lines, 5141 B (maintainer of 5141 A) and J 104, and through four generations of pedigree selection under intense disease pressure in the DM nursery, two lines, IC-MA841 (from 5141 B) and ICMP 84814 (from J 104), were selected resulting in a reconstituted DM resistant hybrid (‘ICMH 84814’) which was equal in yield to the original ‘BJ 104’. The reconstituted hybrid, though phenotypically similar, can be distinguished from “BJ 104” being slightly taller, flowers later, has heavier heads, and 1000-seed weight, but tillers less.A similar exercise was attempted on Tifton 23 B, the female parent of the first widely grown hybrid (‘HB 3’— the male parent was also J 104), but no variability for resistance to DM was found.

    Notes: A number of percutaneous dilational tracheostomy devices are now available for clinical use. Recently, a new percutaneous dilational tracheostomy device, the ‘T-Dagger™’ (Criticure Invasives, India), has been introduced for rapid bedside percutaneous tracheostomy. In a prospective preliminary study, we have performed percutaneous dilational tracheostomy (PDT) using the T-Dagger™ in 20 adult ventilated patients in order to evaluate the safety and efficacy of the new device. The T-Dagger™ facilitated bedside PDT in about 3 min with no untoward incidents. There was no significant bleeding, pneumothorax, pneumomediastinum, tracheal wall injuries or difficulty in ventilation in any of the patients. We conclude that the T-Dagger™ shows early promise in bedside percutaneous dilational tracheostomy. However, controlled studies are required in a larger patient population before it can be recommended for routine use.

    Notes: The K-D2 point is the Korean hand acupressure point in Koryo Hand Therapy for prevention of postoperative nausea and vomiting. We evaluated the efficacy of capsaicin ointment at the K-D2 point in 186 patients undergoing laparoscopic cholecystectomy in a randomised, prospective, double-blind and placebo-controlled study. Patients were randomised to have either placebo ointment or capsaicin ointment applied to the K-D2 point of both hands 1 h before surgery under standardised anaesthesia. The ointment was removed 8 h later. Postoperative nausea and vomiting was evaluated 6 and 24 h following surgery. The incidence of postoperative nausea and vomiting was lower in the capsaicin group, with an absolute risk reduction (ARR) of 21%, a relative risk reduction (RRR) of 50% and a number-needed-to-treat (NNT) of 5 at 0–6 h (p = 0.001), and an ARR of 11%, a RRR of 85% and a NNT of 9 at 6–24 h (p = 0.003). The need for rescue anti-emetic treatment was also lower at 0–6 h (3 (3%) vs 11 (12%); p = 0.04) and at 6–24 h (5 (5%) vs 0; p = 0.02).
