    Notes: Oral aspirin challenge (OAC) is used to confirm aspirin hypersensitivity (AHs) but there is no consensus on a standardized protocol. As a prior clinical history of adverse reactions to aspirin is poorly predictive of a positive result from formal aspirin challenge, many patients have an OAC performed.We retrospectively identified and prospectively validated how a 1-day OAC protocol could be modified, and patient selection improved, to deliver a safe and more efficient service.In a retrospective audit of 45 OACs using a 2 h dose interval, all reactions occurred within 90 min of the threshold dose. Forty OACs were then performed using a 90-min dose interval. This reduced the mean duration of a positive and negative OAC from 6 to 5 h and from 8 to 6 h, respectively.Histories of multiple manifestations of AHs were found in 91.6% (11) of those with asthma, 87.5% (7) with angiooedema, 70.6% (12) with rhinosinusitis, 63.6% (7) with chronic non-vasculitic urticaria and all with anaphylaxis, who developed a positive OAC. None of those with anaphylaxis, 8.3% (1) with asthma and 12.5% (1) with angiooedema, with a positive OAC, had a history of a single manifestation of AHs.The efficiency of an OAC service can safely be improved by reduction of the dose interval from 2 to 1 ½ h, and more targeted patient selection, as the likelihood of a positive OAC increases among patients with a history of asthma, angiooedoema or anaphylaxis with multiple manifestations of AHs.

    Notes: Background To determine which diary card variables are the most predictive for administration of additional courses of corticosteroids using the TRUST (The Regular Use of Salbutamol Trial) data set.Methods Logistic regression models were used to identify the extent to which a change in diary card variable affected the odds ratio (OR) for administering a course of oral or increased inhaled corticosteroids. The complete TRUST diary card data were used with over 200 000 days of diary card observations from 983 mild to moderate asthmatic subjects.Results An increase in daytime symptoms of 1–5 U over baseline was associated with an increase in the OR for starting all types of corticosteroids from two- to 60-fold.Conclusions These results indicate that an increase in daytime symptoms of two or more over baseline strongly predicts the administration of additional corticosteroids. The results have significant implications for both clinical practice and design of clinical trials in asthma.

    Notes: Background Although epidemiological studies have found an association between Chlamydia pneumoniae infection and severe asthma, the causality and underlying mechanism are largely unknown. We hypothesized that C. pneumoniae infection increases the proliferation and enhances the survival of immune and inflammatory cells, resulting in reduced responsiveness to corticosteroids and suggesting that the underlying mechanism is related to a TNF-α-dependent pathway.Methods Human peripheral blood mononuclear cells (PBMCs) were cultured in vitro in the presence or absence of C. pneumoniae infection. Responsiveness to corticosteroids was assayed by adding dexamethasone, and the underlying mechanism was investigated by treating cells with infliximab that is a chimeric anti-TNF-α monoclonal antibody. Cellular proliferation and apoptosis was assessed by thymidine uptake and counting apoptotic cells using flow cytometry.Results Cellular proliferation was significantly higher in C. pneumoniae-infected PBMCs than in uninfected PBMCs, which is more prominent in Th2-dominant microenvironment. The anti-proliferative and pro-apoptotic effect of corticosteroid were significantly reduced in C. pneumoniae-infected PBMCs compared with uninfected PBMCs. The proliferative effect of C. pneumoniae infection and the reduced response to corticosteroid were effectively reversed by blocking the TNF-α pathway at least partially.Conclusion C. pneumoniae infection enhanced the proliferation and survival of immune and inflammatory cells, resulting in steroid resistance. The reversal of these phenomena by the TNF-α inhibitor suggests that TNF-α may play an important role in the induction of steroid dependence or resistance. A TNF-α inhibitor may therefore be a candidate agent for managing steroid-dependent or -resistant severe asthma.

    Notes: Habitat segregation has been recognized as an important means of resource partitioning in river fish assemblages. Association between river fishes and habitat features were identified through principal component analysis (PCA). Fish and habitat data were collected in the Kaoping, Tsengwen, Choshui, and Tatu rivers from July 1997 to June 1999 in western Taiwan. Electrofishing was used to collect fishes in grids, and environment variables in the sampled areas were measured immediately after sampling. The first four PCs of depth, velocity, cover, substrate and water temperature variables explained 92.3% of the total variance in habitat characteristics. Varicorhinus barbatulus, Acrossocheilus formosanus and Tilapia mossambica in the upper right quadrant, indicated preference for deep-water habitat. Species usually associated with shallow water habitat include several minnow species. Rhinogobius brunneus, Hemimyzon formosanum, Sinogastromyzon puliensis and Crossostoma lacustre distribution is centred in the upper and lower left, indicating preference for shallow water. Principal component (PC) scores on Zacco pachycephalus, Abbottina brevirostris and Hemiculter leucisculus appeared to have no special preference. Differences in habitat use exhibited by these species may be related to success in finding food and avoiding predators.

    Notes: summary  The purpose of this study was to investigate the influence of chemical composition of Ni-Cr dental casting alloys on the bonding behaviour between porcelain and metal. A three-point bending test was used to measure the fracture load of alloy after porcelain firing. A scanning electron microscope, accompanied by an energy dispersion spectrometer, was used to analyse the morphology and chemical composition of the fracture surface. An X-ray photoelectron spectrometer and glow discharge spectrometer were used to identify the structure and cross-sectional chemical composition, respectively, of oxide layers on Ni-Cr alloys after heat treatment at 990 °C for 5 min. Results showed that the oxide layers formed on all Ni-Cr alloys contained mainly Cr2O3, NiO, and trace MoO3. The Ni-Cr alloy with a higher Cr content had a thicker oxide layer, as well as a weaker bonding behaviour of porcelain/metal interface. The presence of Al (as Al2O3) and Be (as BeO) on the oxide layer suppressed the growth of the oxide layer, leading to a better porcelain/metal bonding behaviour. However, the presence of a small amount of Ti (as TiO2) on the oxide layer did not have any influence on the bonding behaviour. The fracture propagated along the interface between the opaque porcelain and metal, and exhibited an adhesive type of fracture morphology.

    Notes: Reviewed the achievements and advances in the vibratory metal machining, analyzed thefeatures and mechanics in microstructure with SEM. The dynamic stress intensity factor is derivedon the basis of fracture mechanics and stress-wave theory with the Griffith- II type sliding-opencrack.. Comparison between the vibratory machining and the conventional machining has beenmade with brass on lathe CW6150B, with computer controlled piezoelectric ceramic micro-drivesystem and cutting forces and surface texture of the specimen were observed. The dynamic stressintensity factor was discussed and it was found that the value of dynamic stress intensity factor maybe high with response to the Heaviside impulsive loading, indicating the reason why vibratorymachining has better comprehensive results than the conventional one does
