    Notes: Abstract Objective To compare subcutaneous (PscO2) and transcutaneous (PtcO2) oxygen tension measurements in relation to hemodynamic variables at different levels of PEEP, and to evaluate the usefulness of these measurements as monitors of peripheral tissue perfusion. Design Prospective trial. Setting Intensive care unit in a university hospital. Patients Seven patients with gastric cancer who where undergoing total gastrectomy. Interventions Silicone catheter was placed in the upper arm and transcutaneous oxygen monitor was placed on the upper part of the chest. A pulmonary artery catheter was placed in the right pulmonary artery. Measurements and results PscO2 and PtcO2 together with hemodynamic variables were measured at different levels of PEEP. Progressive increase of PEEP reduced cardiac index (CI) (p〈0.05) with a concomitant decrease of PscO2 (p〈0.05) and oxygen delivery (DO2) (p〈0.05). Changes in PtcO2 parallelled changes in arterial oxygen tension (PaO2), but no correlation was found between PtcO2, CI and DO2. Conclusion PscO2 is a sensitive indicator of subcutaneous tissue perfusion, which can be used to identify the PEEP level, with optimum peripheral perfusion. PscO2 seems to be a more reliable indicafor of tissue perfusion than PtcO2.

    Description / Table of Contents: Abstract Background: The purpose of the study was to examine the pain related health care utilization in patients with fibromyalgia syndrome. Methods: In a multicenter study six rheumatological hospitals associated in the rheumatological center of Heidelberg participated. All consecutive patients which fulfilled the ACR-criteria of fibromyalgia were assessed. Data about pain and related treatments were available from n=259 patients. Results: Patients suffered from chronic generalized pain on average for 5 years before being diagnosed as fibromyalgia. In most of the cases longlasting localized pain symptoms preceeded generalized pain. The spectrum of pain related treatments seems to be wide and arbitrary: 87% took questionable or excessive medication. 86% had some form of physiotherapy and 41% already experienced psychological treatment. 28% of the patients consulted dental surgeons including teeth extraction and 20% underwent multiple orthopaedic surgery. A mean of five different health care professionals were consulted regarding the pain problem. Treatment statisfaction rated by the patients was lowest with pharmacological and highest with psychological treatment and physiotherapy. Conclusions: The health care-seeking behavior of fibromyalgia-patients seems to be characterized by a multitude of different treatment attempts. Treatment was often invasive and focused on localized symptoms rather than on the generalized pain syndrome. An early confirmation of the diagnosis and a more evidence based treatment is strongly indicated.
    Notes: Zusammenfassung Fragestellung: Ziel der vorliegenden Studie ist die Analyse der Inanspruchnahme medizinischer Versorgungsleistungen von Fibromyalgiepatienten. Methode: Im Rahmen einer multizentrischen Erhebung in 6 Kliniken des Rheumazentrums Heidelberg wurden alle Patienten mit Fibromyalgiesyndrom (gemäß ACR 1990) hinsichtlich ihrer bisherigen Behandlungen befragt. Es liegen Daten von insgesamt n=259 Patienten vor. Ergebnisse: Im Durchschnitt hatten die Patienten bereits 5,4 Jahre vor der Diagnosestellung chronisch generalisierte Schmerzen (Diagnoselatenz). Dem Stadium der Schmerzgeneralisierung ging dabei ein über 10-jähriges Stadium lokalisierter Schmerzen voraus. Zahlreiche therapeutische Maßnahmen gegen die Schmerzsymptomatik wurden durchgeführt: 87% nahmen Medikamente – häufig eine fragwürdige Polymedikation. 86% erhielten aktive oder passive Physiotherapie und 41% wurden psychologisch behandelt. Bei 28% der Patienten wurde wegen der Schmerzen eine Zahnbehandlung durchgeführt. 20% wurden z. T. mehrfach orthopädisch operiert, nach der Schmerzgeneralisierung sogar signifikant mehr als vorher. Insgesamt wurden im Schnitt von den Patienten Ärzte in 5 verschiedenen Fachrichtungen konsultiert. Die Patienten selber beurteilten die medikamentöse Behandlung als am wenigsten hilfreich und passive Physiotherapie neben psychologischen Interventionen als am effektivsten. Schlussfolgerungen: Die Versorgungssituation der Patienten besteht aus einer Vielzahl unterschiedlicher Behandlungsversuche, welche jedoch eher auf lokalisierte Probleme als auf die generalisierte Schmerzsymptomatik abzielen. Eine frühzeitige Diagnosestellung und ein evidenzbasiertes Behandlungskonzept erscheinen dringend erforderlich.

    Notes: The binding of iodinated basic fibroblast growth factor (bFGF) to low-density heparan sulfate proteoglycan purified from the Engelbreth Holm Swarm (EHS) sarcoma was investigated using different techniques. The tumor clearly contained bFGF, the level being comparable to that found in other tissues such as human or bovine brain. 125I bFGF strongly bound to the basement membrane-like matrix of EHS frozen sections as revealed by autoradiography. Iodinated bFGF bound to purified heparan sulfate proteoglycan but not to laminin or collagen type IV, three components isolated from the same tumor. In contrast, acidic fibroblast growth factor (aFGF) displayed negligible binding to heparan sulfate proteoglycan. Binding of bFGF to frozen sections and to purified proteoglycan could be strongly inhibited by heparin and was displaced by an excess of unlabeled factor and completely suppressed after heparitinase and heparinase treatments. Binding was a function of the salt concentration and was abolished at 0.6 M NaCl. Scatchard analysis indicated the affinity site had a Kd of about 30 nM, a value 10-15 higher than that recently reported by Moscatelli (J. Cell. Physiol., 131:123-130, 1987) in the case of the low-affinity binding sites present on the surface of baby hamster kidney (BHK) cells.

    Notes: The human omentum contains a potent, not yet identified angiogenic activity. The omentum is very vascularized. Therefore, we investigated whether human omental microvascular endothelial cells (HOME cells) express the angiogenic peptide basic fibroblast growth factor (bFGF). Cytosol prepared from HOME cells stimulated DNA synthesis in bovine epithelial lens cells (BEL cells). The mitogenic activity could be neutralized by an anti-bFGF antibody. Basic FGF-like material from the HOME cell cytosol was bound onto a heparin-Sepharose column at 0.6 M and was eluted at 3 M NaCl. The 3 M NaCl eluted material reacted with the specific anti-bFGF antibody in an ELISA and stimulated DNA synthesis. It did not react with a specific anti-acidic fibroblast growth factor (aFGF) antibody. Western blotting experiments using the same bFGF antibody showed the presence of a major band of 17 Kd and a doublet of 20-22 Kd. Northern blotting of nonstimulated HOME cells using a specific 1.4 kb bFGF probe showed the presence of 5 molecular species of 6.6, 3.7, 2.2, 2.0, and 1.0kb. No aFGF mRNA was detected with a specific previously characterized 4.04 kb probe. 12-O-tetradecanoylphorbol 13-acetate (TPA) did not influence significantly the expression of bFGF at the protein and mRNA level in HOME cells. Thus, protein kinase C activation by TPA did not appear to modulate significantly the expression of bFGF for that cell type. Contrastingly, human umbilical vein endothelial cells (HUVE cells), which expressed no bFGF and aFGF mRNA at a basal level, were induced to express bFGF but not aFGF mRNA when stimulated by TPA. These results suggest that the described angiogenic activity could be the bFGF-like mitogen contained in HOME cells and that these cells are different from endothelial cells derived from large vessels (HUVE cells) regarding the expression of bFGF.
