    Notes: Summary. While studying the lung function of pregnant women at term in four different postures, we were surprised to note marked cyclic accelerations in the heart rate in two-thirds of the women when in a standing position. The mean cycle length was 105 s (range: 1–4 min) and the amplitude had a mean of 27 beats/min (range: 9–51). Blood flow velocity measurements with ultrasound Doppler over the femoral vein showed that there was an intermittent reduction of flow during quiet standing. When the venous return ceased, maternal heart rate increased, cardiac output decreased and blood pressure fell. After the venous blood flow was restored, maternal heart rate, cardiac output and blood pressure returned to normal until the cycle started again. Concomitant with these maternal heart rate changes, different patterns of fetal heart rate were observed. About 70% of the fetuses showed reduction in the long-term variability, increase in fetal heart rate or periodic accelerations. Although no woman fainted during quiet standing, the maternal circulatory changes were consistent with those seen in the classical vena cava syndrome.

    Notes: Abstract: Sea otters may give birth in any month of the year, so obtaining reproductive rates by observation is difficult. Reproductive rates may be estimated directly (births per otter-year observed) or by determining the time interval between births. Both methods give the same result for long sequences of observations, but field data are limited to shorter periods. Monte Carlo simulations were conducted to compare the two approaches, and showed that the interval method overestimates true reproductive rates.

    Description / Table of Contents: Abstract In our study we tested 2 different rapid assays for cardiac troponin t (TropT®-rapid assay, diagnostic cut-off 0,2 ng/ml and TropTsensitive®-rapid assay, diagnostic cut-off 0,1 ng/ml) in 75 chest pain patients. Additional to the emergency ECG (3-lead) we acquired a 12-lead ECG. The TropT®-rapid assay showed a sensitivity of 11% (specifity 94,3%). The TropT sensitive®-rapid assay showed a sensitivity of 62% (specifity 100%). In patients with acute myocardial infarction 12-lead ECG showed a sensitivity of 82% (3-lead ECG 24%). An risk-orientated check-up for thrombolysis contraindications should be done to identify risk-patients. Further education of emergency physicians in 12-lead ECG should be done.
    Notes: Zusammenfassung In der vorgestellten Untersuchung wurden 2 Troponin-T-Schnelltests –, der TropT®-Test (Schwellenwert 0,2 ng/ml) und der TropT sensitive®-Test (Schwellenwert 0,1 ng/ml) – im präklinischen Einsatz bei 75 Patienten mit “akutem Koronarsyndrom” getestet. Zusätzlich wurde bei allen Patienten ein 12-Kanal-EKG abgeleitet. Der Troponin-T-Schnelltest zeigte in unserer Untersuchung eine Sensitivität von 11% bzw. 62%. Die Spezifität lag bei 94,3% bzw. 100%. Die Sensitivität des 12-Kanal-EKG lag bei 82%. Der Troponin-T-Schnelltest kann in einzelnen Fällen einen Benefit zeigen, kann jedoch nicht für den präklinischen Routineeinsatz empfohlen werden. Eine präklinische 12-Kanal-EKG-Ableitung lässt sich sicher und einfach durchführen und führt zu einer Reduktion der “door-to-needle-time”. Im “Fachkundenachweis Rettungsdienst” sollte vor diesem Hintergrund die differenzierte EKG-Auswertung akzentuiert werden.

    Description / Table of Contents: Zusammenfassung Mittels eines Fragebogens wurden 660 Diabetiker interviewt über Krankheits-Vorgeschichte und Krankheits-Verlauf. Internistische und ophthalmologische Befunde wurden erfaßt, die anläßlich eines Klinikaufenthaltes zur Stoffwechselkontrolle erhoben worden waren. Eine HNO-ärztliche Untersuchung einschließlich Gleichgewichts- und Hörtest wurde durchgeführt. Die Auswertung zeigte, daß der mittlere Hörverlust (median) für die einzelnen Typen des Diabetes nicht deutlich über die Altersnorm Gesunder (Spoor) hinausgeht (Abb. 1). Die Hypothese, das Ohr des Zuckerkranken altere vor, konnte statistisch nicht belegt werden.     Notes: Summary It should be examined by this study, if there exists a form of hearing loss, which is typical for diabetes mellitus. 660 patients were interviewed by means of a questionaire with regard to case history and course of disease. Internal and ophthalmological findings have been registered, which were compiled during a clinical treatment for metabolic stabilisation. An ENT-examination was performed including vestibular and auditory tests. The evaluation of data showed that there was no median hearing loss, which could be attributed to the diabetic metabolic disorder. The hypothesis, that diabetes leads to a premature old age of hearing, could not be confirmed on a statistical base. The statistical analysis of separate factors of the diabetic disease, of vascular risks and of noise or ear infections leads till now to no results.

    Notes: Abstract We show that the existence of wave trains with high velocity of generalized reaction-diffusion equations can be easily established by using a theorem of D. V. Anosov on the existence of periodic solutions of singularly perturbed differential systems.
