Zusammenfassung Die Wirkung von prä- und intraoperativ appliziertenα-bzw.β-Receptoren-blockern auf die perioperative Komplikationsrate beim Phäochromocytom wurde an Hand von 21 eigenen Fällen untersucht. Die Vorbehandlung mitα- bzw.β-Blockern führte nur zu einer subtotalen Blockade der Catecholaminwirkung. Die intraoperativen Kreislaufdysregulationen waren aber abgeschwächt und nach Gabe vonα- bzw.β-Blockern rasch reversibel. Durch gezielte intra- und postoperative Volumenzufuhr gelang es, postoperative hypotone Phasen weitgehend zu verhindern. Die perioperative Letalität konnte durch dieses Behandlungskonzept auf 0% gesenkt werden.

Summary The effect of pre- and intraoperativeα- andβ-adrenergic blockade on cardiovascular disorders in patients with pheochromocytoma was studied. Catecholamine responses during surgery were not totally eliminated by pretreatment with a- and ß-blocking drugs. Intraoperative changes in blood pressure and cardiac rhythm, however, were attenuated and could be easily controlled byα- andβ-blocking drugs or sodium nitroprussid and xylocain. Severe postoperative hypotension was efficiently prevented by judicious intra- and postoperative volume replacement. Postoperative mortality could be reduced to 0% by this perioperative treatment.
