    Notes: Abstract The objective of this study was to determine the role of serum KL-6 levels as a marker for the activity of interstitial pneumonia in patients with connective tissue diseases. The serum concentrations of KL-6, a glycoprotein produced mainly by pulmonary type II epithelial cells, were measured in 21 patients with connective tissue disease. The activity of interstitial pneumonia was compared with the associated serum KL-6 concentrations. Serum KL-6 concentrations in patients with interstitial pneumonia were significantly higher than those in the controls. Among patients with active interstitial pneumonia, serum KL-6 concentrations following the treatment (after improvement) were significantly lower than the pretreatment values. The extent of the pulmonary fibrosis correlated positively with the serum KL-6 concentrations during the inactive phase of the interstitial pneumonia. These results suggest that sequential measurement of serum KL-6 levels is a new and useful means for the evaluation of interstitial pneumonia in patients with connective tissue diseases.

    Description / Table of Contents: Zusammenfassung Mittels Hochleistungs-Flüssigkeitschromatographie wurden durch Analyse der Fettsäuren in Sphingomyelien Adrenoleukodystrophien und Adrenomyeloneuropathien untersucht. Die oben erwähnte analytische Methode erwies sich als einfach, empfindlich und exakt. Bei der Adrenoleukodystrophie und bei Patienten mit Adrenomyeloneuropathie wurden sehr lange Fettsäureketten (C26:0, C25:0 und C24:0 auf der Basis von C22:0) sowohl in Plasma und den Erythrozytenmembranen als auch in Gesamtblutproben als vermehrt erkannt. Auch in Kulturen von Hautfibroblasten von Sphingomyelien wurde eine Zunahme der langkettigen Fettsäuren nachgewiesen. In einem Fall eines heterozygoten Adrenoleukodystrophiepatienten wurden Werte die zwischen jenen der homozygoten und der Kontrollfälle lagen gefunden. Somit ist die Hochleistungs-Flüssigkeitschromatographie eine brauchbare Methode, um eine Zunahme langkettiger Fettsäuren zwecks Diagnose einer Adrenoleukodystrophie oder einer Adrenomyeloneuropathie festzustellen.
    Notes: Summary Using high-performance liquid chromatography, adrenoleukodystrophy (ALD) and adrenomyeloneuropathy (AMN) were diagnosed by the analysis of fatty acids in sphingomyelin. The analytical method was simple, sensitive and accurate. In ALD and AMN patients, very long chain fatty acids (C26:0, C25:0 and C24:0 on the base of C22:0) were elevated not only in plasma and erythrocyte membranes but also in whole blood samples. An increase of long chain fatty acids was also shown in sphingomyelin of cultured skin fibroblasts. One heterozygote for ALD showed intermediate values between homozygotes and controls. Thus, high-performance liquid chromatography is a valuable method to detect increased long chain fatty acids for the diagnosis of ALD or AMN.

    Description / Table of Contents: Résumé Le traitement optimal des cas associant des lésions du ligament croisé antérieur (ACL) à des lésions médiales collatérales (MCL) est controversé. Nous avons réalisé une étude sur le suivi du traitement de telles lésions dans le but d’évaluer si le traitement opératoire des déchirures de MCL peut apporter une amélioration des résultats fonctionnels pour des patients choisissant de ne pas subir une reconstruction de ACL. Notre étude portait sur 14 patients ayant subi une reconstruction du MCL et 11 patients traités sans opération. Nous n’avons pas observé de différences significatives entre les deux groupes pour les tests de relâchement manuel, ni pour les mesures KT-1000, ni pour les niveaux d’activité de Tegner. En revanche les patients du groupe opératoire ont obtenu un meilleur score fonctionnel de Lysholm.
    Notes: Abstract  The management of patients with combined medial collateral (MCL) and anterior cruciate (ACL) rupture remains controversial. We studied 25 such patients who elected to have the ACL lesion treated conservatively; 14 underwent MCL repair with early mobilization and 11 were treated with immobilization for two weeks. The mean follow up was 5.9 years (2 to 11). There was no difference in the clinical assessment of ligamentous laxity, KT-1000 measurements or Tegner activity scores between the two groups but there were significantly higher Lysholm function scores in the operated group.
