    Notes: Abstract  Thrombotic thrombocytopenic purpura (TTP) is a rare disease which, together with hemolytic uremic syndrome, is subsumed under thrombotic microangiopathy. After stem cell transplantation (SCT), this syndrome represents a possibly fatal complication with a higher incidence in allogeneic SCT than in autologous SCT. Although plasmapheresis offers an encouraging treatment modality in classic TTP, this seems less effective in bone marrow transplant-associated microangiopathy. This is probably due to a different etiology. We present a case of transplant-associated TTP with a fatal outcome despite multiple courses of plasmapheresis.

    Notes: Zum Thema In dieser Arbeit wird eingehend über die pathophysiologischen Grundlagen berichtet, die zur chronischen Herzinsuffizienz führen. Wie als grundsätzlich bekannt vorausgesetzt werden darf, haben sich auf diesem Gebiet in der letzten Dekade geradezu bahnbrechende Entwicklungen mit der Erforschung von Prozessen auf zellulärer Ebene ergeben: die neurohumorale Aktivierung und die Aktivierung des Renin-Angiotensin-Aldosteron-Systems. Die Darstellung dieser Vorgänge, die schließlich zu den pathologisch-anatomischen Veränderungen und den pathophysiologisch faßbaren Funktionsstörungen und somit zu den klinisch dominierenden Symptomen der chronischen Herzinsuffizienz mit ihren charakteristischen Formen führen, werden ausführlich dargestellt. Sie stellen auch die Voraussetzungen für einen tiefgreifenden Wandel in der Therapie der chronischen Herzinsuffuzienz dar. Die Arbeit über die Pathophysiologie steht mit Bedacht am Anfang dieser 2 Schwerpunkthefte des INTERNIST über Herzinsuffizienz. Wieder einmal wird ersichtlich, daß Grundlagen-, klinisch-experimentelle und pharmakologische Forschung, klinische Therapiestudien und Metaanalysen untrennbar zusammengehören, um neue Behandlungskonzepte durchzusetzen.

    Notes: Summary. Previous studies have shown the inhibitory effects of thiopentone on polymorphonuclear leucocyte (PML) function. However, major biochemical mechanisms which have been involved are still unknown. The aim of this study was therefore to investigate thiopentone's effects on intracellular amino acid metabolism in PML using both advanced PML separation – and HPLC techniques, especially developed for this purpose and precisely validated in our institute. Overall, our study indicates important dose-dependent alterations of free intracellular amino acid metabolism following thiopentone treatment and draw attention to the biochemical mechanisms which may be involved in both thiopentone-induced modulation in PML function and cellular immunocompetence.

    Description / Table of Contents: Summary We report on two cases of acute knee trauma with rupture of the synovial tissue of the anterior cruciate ligament. The femoral part of the synovia had retracted and was locked between the femoral attachment of the anterior cruciate ligament and the femur condyle. Both patients showed clinical signs of an acute lesion of the medial meniscus with loss of extension and tenderness on palpation over the medial joint cavity. After complete arthroscopic resection of the interfered soft tissue, both patients had full range of motion and no signs of any knee discomfort.
    Notes: Zusammenfassung Wir berichten über 2 Fälle eines traumatischen Einrisses des Synoviaüberzugs über dem vorderen Kreuzband nach Kniegelenkverdrehtrauma. Der femorale Anteil der Synovia retrahierte sich und klemmte zwischen dem femoralen Ansatz des vorderen Kreuzbands und der Femurkondyle im Sinne eines Impingements mit schmerzbedingter Streckhemmung ein. Klinisch imponierte der Befund wie eine akute Meniskuseinklemmung. Arthroskopisch wurden eine Meniskusläsion ausgeschlossen und die eingeschlagenen Weichteile entfernt. Bei sofort postoperativ wiedererlangter schmerzfreier Beweglichkeit blieben die Patienten beschwerdefrei.

    Notes: Abstract Two types of model catalysts are compared: thin film catalysts consisting of polyhedral noble metal nanocrystals (Rh and Pt) supported by reducible and non‐reducible oxides, and their inverted pendants, submonolayers of titania and vanadia deposited under UHV conditions on the respective metal surfaces (Pd and Rh(111) and Rh (polycrystalline)). The structure and composition of the inverse catalysts were examined in situ by LEED and AES and the nanoparticles were characterized by HRTEM. The activity of thin film and inverse catalysts was studied in a series of reactions, such as the ring opening of methylcyclopentane and methylcyclobutane, the dissociation of CO and the CO methanation. Reaction conditions comprise atmospheric pressure but also molecular beam experiments. The reaction rates are related to the oxidation state of the supporting oxide, to the free metal surface area and to the number of sites at the interface between metal and support.
