    Notes: Rotationally resolved infrared spectra have been obtained for HCN–oH2, pH2–HCN, HCN–pD2, HD–HCN, and oD2–HCN binary complexes formed in liquid helium nanodroplets. The isolation provided by the droplets enables us to observe both ortho and para complexes, while in the previous gas phase infrared study only the more strongly bound J=1 complexes were observed. Of particular interest is the observation that the complexes correlating with J=0 and J=1 hydrogen bind to opposite ends of the HCN molecule. In an effort to understand this behavior we have carried out ab initio calculations at the CCSD(T) level, exploring the potential energy surface near the corresponding minima. The dramatic difference between the binding of ortho and para hydrogen to HCN can be at least partially understood by averaging the calculated surface over the appropriate rotational wave functions. © 2001 American Institute of Physics.

    Notes: Infrared spectra are reported for carbon dioxide and nitrous oxide solvated in superfluid helium droplets, corresponding to the vibrational excitation of the (0201)/(1001) Fermi diad. Although the rotational constants of these two molecules are similar in the gas phase, they are observed to be quite different in liquid helium, namely, 0.154 cm−1 for CO2 and 0.0717 cm−1 for N2O. In addition, solvation in helium results in shifts in the vibrational origin that are in the opposite directions, −0.42 cm−1, for CO2 and +1.2 cm−1 for N2O. The spectra also show strong droplet size dependence, indicative of the interactions between the molecule and the liquid. © 2001 American Institute of Physics.

    Notes: The amplitude and frequency of modes driven in the edge region of tokamak high mode (H-mode) discharges [type I edge-localized modes (ELMs)] are shown to depend on the discharge shape. The measured pressure gradient threshold for instability and its scaling with discharge shape are compared with predictions from ideal magnetohydrodynamic theory for low toroidal mode number (n) instabilities driven by pressure gradient and current density and good agreement is found. Reductions in mode amplitude are observed in discharge shapes with either high squareness or low triangularity where the stability threshold in the edge pressure gradient is predicted to be reduced and the most unstable mode is expected to have higher values of n. The importance of access to the ballooning mode second stability regime is demonstrated through the changes in the ELM character that occur when second regime access is not available. An edge stability model is presented that predicts that there is a threshold value of n for second regime access and that the most unstable mode has n near this threshold. © 2000 American Institute of Physics.

    Notes: By using an atomic-force-microscope-based technique, we image the vibration of high-frequency, bulk-mode, thin-film resonators. Our experimental technique is capable of monitoring the vibration of these devices over a broad frequency range, from 1 MHz to beyond 10 GHz, allowing us to obtain quantitative measurements of the piezoelectric properties of thin-film materials in that frequency range. This technique allows us to map the complex vibration modes of a new generation of high-frequency bulk piezoelectric resonators, revealing the presence of vibration patterns of very different characteristic lengths. © 2000 American Institute of Physics.

    Notes: High-resolution infrared spectra are reported for the "free" and "hydrogen bonded" H–F stretches of the hydrogen fluoride dimer solvated in helium nanodroplets. These rotationally resolved spectra provided detailed information concerning the effect of the helium solvent on the vibrational frequencies, rotational constants and tunneling dynamics of the dimer. The end-over-end rotation of the dimer is slowed by a factor of 2.2 by the helium, while the faster rotation about the a axis remains essentially unaffected. The interchange tunneling is reduced significantly (∼40%) in both the ground and vibrationally excited states. The effective tunneling barrier is higher than in the gas phase, making it easier to quench the tunneling motion with a large dc (direct current) electric field. Rapid rotational relaxation is observed from the Ka=1 state, resulting in significant broadening of the corresponding subband. Combination bands are observed for the intermolecular F–F stretch and trans-bend vibrations, providing further insights into the nature of the solvent effects. © 2000 American Institute of Physics.

    Notes: Fifty isolates of Crinipellis perniciosa originating from Theobroma cacao, Heteropterys acutifolia and Solanum lycocarpum, from six states within Brazil, were characterized through ERIC-PCR, representing the first application of this method for molecular characterization within C. perniciosa. Phenetic analysis of banding patterns revealed a separation of isolates on the basis of host of origin, with T. cacao-derived isolates showing only a 0·2 similarity level to a cluster comprising the isolates from H. acutifolia and S. lycocarpum. Considerable intraspecific variability was observed within C. perniciosa isolates from T. cacao, with distinct groups observed correlating with geographical origin. Given that a number of isolates from T. cacao from the Amazon region grouped with isolates from Bahia state, this work discusses the possibility that current C. perniciosa populations pathogenic on T. cacao in Bahia originated from the Amazon region, rather than from alternative host plants.

    Notes: High-resolution infrared laser spectroscopy is used to study hydrogen fluoride solvated in helium nanodroplets. The results clearly show that the vibrationally excited HF (v=1) does not relax on the time scale of the experiments (0.5 ms) and that the large linewidth of the R(0) transition (0.43 cm−1) results from rotational relaxation. A large dc electric field is applied to induce a Q(0) transition, providing an accurate value for the rotational constant of HF in solution (19.47 cm−1), only 2% smaller than in the gas phase. © 2000 American Institute of Physics.

    Notes: Summary.  This study explores the rate of psychosocial dysfunction in affected and unaffected children from families with haemophilia or β-thalassaemia, as part of a cross-sectional, multicentre study into the resilience of 115 families with blood disorders. Sociodemographic and developmental data were collected from the parents using a standardized and semi-structured interview format, and medical data were obtained from the clinician. The children's social functioning over the year prior to the assessment was assessed with The Social Adjustment Scale adapted for school-aged children. Children with β-thalassaemia showed significantly higher rates of social dysfunction than their unaffected siblings or children with haemophilia and their siblings. Older children showed significantly higher social dysfunction at school. The high rate of social dysfunction in children with β-thalassaemia compared with unaffected siblings is likely to have a basis in the negative experiences associated with their medical problems. In contrast, the therapeutic advances in haemophilia allows boys to lead an almost normal life. Overall, the rates of social dysfunction in families with both these disorders proved commoner than reported in population surveys, but with the unavailability of local population controls, caution needs to be exercised in the interpretation of this finding.

    Notes: The purpose of this study was to assess the relationship between health care and utilization of that health care, and to provide a base measurement of health status in patients with haemophilia. Provider interview and retrospective chart review of 336 patients with haemophilia treated during 1995 at one of five comprehensive haemophilia treatment centres was conducted to measure patient health status characteristics and utilization of health care. Two health status scales were included. The first, the Self-Care Measure, was a four-point single item scale measuring the patient's ability for basic self-care, which was scored by a chart review and an interview with the health-care provider. The second, the Haemophilia Utilization Group Study (HUGS) Functional Status Measure, is a four-item, 10-point scale developed specifically for patients with haemophilia. Our sample represents 27% of actively treated patients in region IX. The mean score on the HUGS Functional Status Measure was 8.7 (SD=2.4). The HUGS scale exhibited a ceiling effect across all four scales: attitude (n=269, 80.1%), overall wellbeing (n=263, 78.3%), working (n=254, 75.6%) and orthopaedic status (n=195, 58.0%). Both higher total health-care costs and factor VIII annual costs were significantly associated with lower scores on the HUGS Functional Status Measure. Health status is a critical component in the assessment of the utilization and outcomes of care. In the absence of the availability of a patient interview, the HUGS Functional Status Measure can be used as one characteristic that explains the variation in the utilization of health care by patients with haemophilia.

    Notes: Summary.  Objective: The main objective of this study was to examine factors associated with utilization and costs for persons with haemophilia. Study design: Utilization data and patient characteristics were collected through medical record review of 336 patients receiving treatment for at least 90% of their haemophilia care at one of five comprehensive haemophilia treatment centres in California. Principal findings: The range of factor VIII deficiency in our sample was similar to the distribution among haemophilic patients in the Western United States; 215 (64%) had severe FVIII deficiency. The mean age in our sample was 21.4 (SD = 16.2) years old and 114 (34%) were HIV-positive. In the multivariate model predicting the total cost of health care during 1995 (adjusted R2 = 0.40), total annual costs were significantly (P 〈 0.05) associated with being HIV-seropositive, infusing FVIII concentrate through a port vs. i.v. infusion, the number of comorbidities, moderate arthropathy (compared with no arthropathy), mild arthropathy, history of inhibitor to FVIII, and current prophylactic FVIII concentrate infusion. Conclusion: As expected, total health-care costs were correlated with comorbid medical conditions, such as HIV and sequelae of haemophilia such as arthropathy. Health policy should consider risk adjustment for the presence of complications such as arthropathy and HIV infection in the financing of haemophilia treatment to promote more equitable delivery of these services.
