    Notes: We present calculations and measurements of current as a function of voltage I(V), and conductivity at low voltage as a function of temperature, G(T), for n+ -GaAs/i- Alx Ga1−x As/n+ -GaAs single-barrier tunneling structures. The current is modeled by its tunneling and thermionic emission components. The shape of the conduction-band edge is obtained by solving Poisson's equation self-consistently, together with two different models for the charge density. In the first model, the electron density is evaluated semiclassically, and in the second, the electron density is calculated from the electronic wave functions. Good agreement is obtained with both models between the experimental and the calculated I(V) and G(T) characteristics over the 4.2–200 K temperature range. The fits obtained are very sensitive to small variations (1%) in barrier width and effective barrier height, which allows one to determine these parameters within a narrower uncertainty range.

Among numerous applications, this yields an encompassing sufficient condition for the validity of the first finitistic dimensionconjecture, that is, for the little finitistic dimension of Λ to coincide with the big (this is well known to fail overfinite-dimensional algebras in general).

    Notes: Zusammenfassung Es wird über Erfahrungen mit der Corticoidbestimmungsmethode nachStaudinger undBauer berichtet. Mit diesem Verfahren werden die freien und conjugierten Corticoide gleichzeitig erfaßt. Ihr Ausscheidungsverhältnis beträgt etwa 1:9. Die Steroidwerte steigen mit dem Alter der Kinder kontinuierlich an. Nach den ersten Lebensjahren zeigen sie etwa gleichbleibende Relationen zu Körperoberfläche und -gewicht. Ein Vergleich der Corticoidwerte mit den ausgeschiedenen Harnmengen ergibt für die ersten Lebensjahre ein proportionales Verhalten der beiden Größen. Mit zunehmendem Alter vermindert sich diese Korrelation; die Steroidausscheidung wird unabhängiger vom Harnvolumen. Ursächlich kommen für diese Beziehungen sowohl eine gesteigerte adrenale Produktion wie eine verstärkte Clearance der Steroide unter dem Einfluß vermehrter Wasserzufuhr in Betracht. Wir möchten die zweite Erklärung für die wahrscheinlichere halten.

    Description / Table of Contents: Summary We report on an adolescent girl with secondary amenorrhea as first manifestation of a suprasellar germinoma. Absence of menstruation was evident for 13 months. However, only the occurrence of additional symptoms such as weight loss and lethargy caused diagnostic procedures. Cranial computerized tomography revealed a midline tumor involving the third ventricle and enlarged ventricles. Hormonal parameters gave evidence for hypogonadotropic hypogonadism, hyperprolactinemia, secondary adrenocortical insufficiency, and growth hormone deficiency. After drainage of the hydrocephalus, stereotactic biopsy and histologic verification, craniospinal irradiation was initiated. Discussion: Secondary amenorrhoea defined as loss of menstrual cycles for 3 to 6 months in an adolescent must be carefully evaluated.
    Notes: Zusammenfassung Wir berichten über eine jugendliche Patientin mit sekundärer Amenorrhö als erstes Symptom eines suprasellären Germinoms. 13 Monate lang ware keine Menstruationsblutungen aufgeteten, aber erst das Auftreten von zusätzlichen Symptomen wie Gewichtsabnahme und Inappetenz führte zur weiteren Diagnostik. Ein Computertomogram des ZNS zeigte eine Raumforderung im Bereich des 3. Ventrikels und erweiterte innere Liquorräume. Endokrinologisch fanden sich ein hypogonadotroper Hypogonadismus, eine Hyperprolaktinämie sowie Hinweise für eine Nebennierenrindeninsuffizienz und einen Wachstumshormonmangel. Nach Entlastung des Hydrozephalus, stereotaktischer Biopsie und histologischer Diagnosesicherung wurde eine perkutane Strahlentherapie eingeleitet. Diskussion: Jede sekundäre Amenorrhö, die nach zuvor regelmäßigen Zyklen länger als 3–6 Monate dauert, muß bei Adoleszenten abgeklärt werden.

    Description / Table of Contents: Summary Thrombolysis therapy in childhood and adolescence has brought about numerous case reports, but no controlled studies over dosage and duration of therapy have been published to date. We report about an adolescent female patient with massive pelvic and leg vein thrombosis that underwent thrombolysis therapy. Loss of the affected leg was imminent. A novelty in this field, two fibrinolytic agents (recombinant tissue plasminogen-activator, as well as streptokinase) were employed simultaneously with success. Discussion: In thrombolysis therapy without success the use of two fibrinolytic agents should be considered.
    Notes: Zusammenfassung Über Thrombolysetherapien bei der Behandlung von Thrombosen im Kindes- und Jugendalter gibt es bereits viele Fallberichte, jedoch noch keine kontrollierten Studien über Dosierung und Therapiedauer des Thrombolytikums. Wir berichten über eine Thrombolysetherapie bei einer massiven Becken- und Beinvenenthrombose einer jugendlichen Patientin, bei der erstmals bei drohendem Verlust des betroffenen Beins 2 Fibrinolytika (rekombinanter Gewebeplasminogenaktivator lokal und Streptokinase systemisch) zeitgleich erfolgreich eingesetzt wurden. Diskussion: Bei einer erfolglosen Lysetherapie einer bedrohlichen Bein- und Beckenvenenthrombose sollte der kombinierte Einsatz zweier Fibrinolytika erwogen werden.

    Notes: Abstract  Syndesmotic sutures of the skull are formed by dense connective tissue and called ”open”; they are ”closed” by formation of a synostosis between adjacent bones. Open sutures are considered as areas of growth and as hinges. The importance of open sutures during the period of skull growth is reflected by pathological situations in which premature closure of the sutures occurs. As alterations of the FGF receptor have been reported in genetical disorders accompanied by premature suture closure (Bellus et al. 1996), the role of fibroblasts and connective tissue in the development of the sagittal suture of mice has been investigated by light and electron microscopy. Morphological changes of the sagittal suture at the following stages are reported: at embryonic day 18, days 1, 5, 9, 14, 20, 26, 28 after birth and in adult mice. Two skulls per stage were investigated. Early osteogenesis appeared in a thin plate, followed by a second plate underneath the first one. Both were separated by blood vessels. In general, vascularization preceded desmoid mineralization; the space around blood vessels was occupied by non-bone-forming cells leaving cavities for the presumptive bone marrow. Mineralization of the collagen-rich osteoid at the mineralizing rim of the bone plates was accompanied by apoptoses and cell disintegration. Newly formed bone was immediately covered by osteoblasts forming a sheet of bone-lining cells. At day 9, the double-layered bone plates of both sides reached the median area of the skull but were separated by non-mineralizing, collagen-rich connective tissue. From day 14 onwards, the bone plates thickened. Bone apposition, recognizable by the formation of collagen-rich osteoid and proceeding from day 14 pp onwards, occurred mainly at the outer and inner surfaces of the calvariae, but neither at bone marrow surfaces nor at the medial edges of the parietal bones. These opposite bone faces showed fewer osteoblasts and bone-lining cells, but an increased number of fibroblasts. Tendon-like collagen bundles connected both bone plates of the open suture of day 26 pp as well as in the adult mice, whereby synostotically closed areas alternated. Formation of an open, syndesmotic suture can, therefore, be described as a transition of bone-forming tissue into a bone-tendon junction. The results indicate the importance of the replacement of osteoblasts by fibroblasts at the sutural front of the bone plates in order to prevent a premature suture closure.

    Notes: Summary The influence of all-trans retinoic acid, 13-cis retinoid acic and two aromatic retinoids (Ro 10-1670, Ro 11-1430) on the chondrogenic differentiation of mesenchymal cells in vitro was studied electronmicroscopically. Organ cultures of limb buds from mouse embryos (Day 11) and high-density cultures of embryonic-mouse mesenchymal cells (Day 12) were used as experimental models. After a 6-day culture in the control medium, the development of hyaline cartilage was observed in both systems. In cultures which were treated with retinoids from Day 1 through Day 3 and then incubated in the control medium for 3 more days, the mesenchymal cells still maintained the morphological features of the blastema stage; cartilage synthesis was reduced (low retinoid concentrations) or completely absent (high retinoid concentrations). These findings indicate that the treatment of embryonic-mouse mesenchymal cells with retinoids induces a persistent and dose-dependent inhibition of chondrogenic differentiation, which in quantitative terms, is variably expressed during treatment with different retinoids. These inhibitory effects of retinoids on chondrogenesis are probably implicated in the pathogenetic mechanisms of their teratogenic action in vivo.

    Notes: Abstract Objective: Polymorphonuclear neutrophils (PMN) are one of the major effector cells of pulmonary defence against bacterial infection. To determine whether neutrophil function is impaired in patients with severe pneumonia, we assessed the two main partial functions exocytosis and oxidative response (ROS production) in isolated neutrophils from the peripheral venous blood of pneumonia patients and healthy volunteers. In addition, pulmonary neutrophils and peripheral neutrophils were compared in pneumonia patients. Patients and methods: Twenty-one patients with severe pneumonia were enrolled in the study. Eleven patients were mechanically ventilated, ten patients breathed spontaneously. For comparison, ten healthy adults were studied. The release of two markers of neutrophil exocytosis, lactoferrin and myeloperoxidase (MPO), with and without stimulation by phorbol-myristate-acetate (PMA), was determined using immunoluminometric assays. ROS production was quantified using luminol-enhanced chemiluminescence. In addition, the clinical severity of pneumonia was correlated to neutrophil exocytosis. Results: With regard to blood neutrophils, both basal and PMA-stimulated exocytosis were significantly impaired in pneumonia patients compared to healthy volunteers (basal lactoferrin secretion in pneumonia patients: 0.25 ± 0.36 pg/PMN versus controls: 1.17 ± 0.78 pg/PMN, p 〈 0.01). In contrast, both basal and PMA-stimulated ROS production were increased in patients compared to controls (spontaneous chemiluminescence in pneumonia patients: 13.6 × 105 cpm versus controls: 5.5 × 105 cpm). In pneumonia patients, the pulmonary neutrophils released significantly more lactoferrin, MPO and ROS compared to blood neutrophils (basal lactoferrin secretion of pulmonary neutrophils: 1.19 ± 1.55 pg/PMN; p 〈 0,01). However, after stimulation with PMA the exocytosis of pulmonary and blood neutrophils was similar. The severity of pneumonia and prognostic indices like albumin were inversely correlated to the release of lactoferrin in blood neutrophils (p 〈 0,05). Conclusion: In patients with severe pneumonia, the exocytosis of blood neutrophils was significantly impaired. In contrast to this, the oxidative response was increased. Impaired bone marrow maturation of neutrophils during severe infection, perhaps due to shortened maturation time, could explain these findings.

    Notes: Abstract A new, long-path integrated optical (IO) sensor for the detection of non-polar organic substances is described. The sensing layer deposited on a planar multimode IO structure is built by a suitable silicone polymer with lower refractive index (RI). It acts as a hydrophobic matrix for the reversible enrichment of non-polar organic contaminants from water or air. Light from the near-infrared (NIR) range is coupled into the planar structure and the evanescent wave part of the light field penetrating into the silicone layer interacts with the enriched organic species. As a result, light is absorbed at the characteristic frequencies of the corresponding C-H, N-H or O-H overtone and combination band vibrations of the analytes. To perform evanescent field absorbance (EFA) measurements, the arc-shaped strip waveguide structure of 172 mm interaction length was adapted to a tungsten-halogen lamp and an InGaAs diode array spectrograph over gradient index fibers. Dimethyl-co-methly(phenyl)polysiloxanes with varying degrees of phenylation were prepared and used as sensitive coating materials for the IO structure. Light attenuation in the arc-shaped waveguides is high and typical insertion losses in the range of 14–18 dB were obtained. When the coated sensors were brought in contact with aqueous samples, the light transmission decreases, which is due to the formation of H2O micro-emulsions in the silicone superstrates. Nevertheless, after reaching constant light transmissions, absorbance spectra of aqueous trichloroethene samples were successfully collected. For gas measurements, where water cross sensitivity problems are absent, the sensitivity of the IO device for trichloroethene was tested as a function of the RI of the silicone superstrate. The slope of the TCE calibration function increases by a factor of 10 by using a poly(methylphenylsiloxane) layer with a RI of 1.449 instead of poly(dimethylsiloxane) (RI: 1.41). A comparison of the IO-EFA and an earlier developed fiber-optic EFA sensor for trichloroethene measurements in the gas phase showed an increase in sensitivity per unit length of the waveguide by a factor of up to 120.
