    Notizen: Twelve patients had their arterial oxygen saturation measured pre-operatively and on days 1, 4 and 7 after laparotomy. Measurements were performed in the supine, sitting and standing positions on each day. Arterial oxygen saturation was significantly higher during sitting and standing on days 1 and 4 after operation compared with the supine position (p 〈 0.05). These results give further evidence for the benefits of patient mobilisation after major surgery.

    Notizen: Thirteen patients were monitored for nocturnal body position (supine vs. side) and arterial oxygen saturation pre-operatively and on the second postoperative night after major abdominal surgery. The number of positional changes were significantly decreased after operation (p 〈 0.05) with a trend towards more time spent in the supine position (p = 0.1). Individual mean arterial oxygen saturation decreased postoperatively (p 〈 0.05) but without a difference between the supine and side positions (p = 0.9). Pre-operatively, episodic desaturations were significantly more frequent in the supine position than on the side (p 〈 0.05) but not postoperatively. Pain was the most frequent reason for decreased nocturnal movements.

    Notizen: Concomitant hypoxaemia and tachycardia in the postoperative period is unfavourable for the myocardium. Since hypoxaemia per se may be involved in the pathogenesis of postoperative tachycardia, we have studied the effect of oxygen therapy on tachycardia in 12 patients randomly allocated to blinded air or oxygen byfacemask on the second or third day after major surgery. Inclusion criteria were arterial hypoxaemia (oxygen saturation ≤ 92%) and increased heart rate (〉 90 beat.min-1). Each patient responded similarly to oxygen therapy: an increase in arterial oxygen saturation and a decrease in heart rate (p 〈 0.002). Thus, postoperative supplementary oxygen has a positive effect on the cardiac oxygen delivery and demand balance.

    Notizen: Episodic oxygen desaturation is frequent in the late postoperative period and seems most pronounced on the second and third postoperative nights. However, the ventilatory pattern has not been described systematically during this period. We studied the ventilatory pattern and associated arterial oxygenation using the Edentrace II equipment (impedance pneumography and pulse oximetry) on the second and third postoperative nights in 28 patients undergoing major abdominal surgery. Ventilatory disturbances were common and included periods of hypopnoea, and obstructive, central and mixed apnoeas. Overall, the median (range) respiratory disturbance index (apnoeas + hypopnoeas per h) was 12 (0–121), with the patients spending 6% (0–65%) of the night in some kind of ventilatory disturbance. It was not possible from pre-operative snoring habits to predict patients who developed postoperative ventilatory disturbances. Overall, 23% (0–100) of the hypopnoeas and 7% (0–100) of the apnoeas were associated with episodic hypoxaemia. In conclusion, ventilatory disturbances were common in the late postoperative period in the general surgical ward and often associated with episodes of oxygen desaturation.

    Notizen: New measurement principles in pulse oximetry have been introduced to decrease the incidence of false movement alarms. Experimental studies have shown that the new Nellcor Symphony N-3000 may reduce the incidence of false alarms when monitoring during different activities. We compared the Nellcor Symphony N-3000 with the Nellcor N-200 pulse oximeter, when monitoring patients in the general surgical ward. Twenty-two patients were monitored during unrestricted ward activities for a total of 275 h with a N-3000 and a N-200 pulse oximeter simultaneously. Data were analysed for lack of concordance between the two pulse oximeters with respect to frequency of registered hypoxaemic episodes and thus the amount of time spent in the alarm state. The median number of desaturation episodes with the N-200 was 18 (range 0–511) compared with four (range 0–476) with the N-3000 (p 〈 0.0001). The median number of drop-outs (loss of signal) was 13 (range 1–46) with the N-200 compared with nine (2–41) with the N-3000 (p = 0.06). The N-200 registered saturation values of 85% or below for 23% of the observation time compared with 6% of the observation time with the N-3000 pulse oximeter (p 〈 0.0001). The different working principles of the two generations of oximeters were reflected in the present results derived from a clinical setting. The Nellcor Symphony N-3000 may offer an advantage compared with the Nellcor N-200, because of the reduced frequency of alarms and total time in alarm when monitoring patients in the general surgical ward.

    Notizen: Twenty-six healthy volunteers were monitored simultaneously with the Nellcor N-200 and N-3000 pulse oximeters during nonhypoxaemic simulated postoperative activity. The overall number of registered events (hypoxaemic episodes or loss of signal) was fewer with the N-3000 than with the N-200 (8 vs. 32, p 〈 0.00005). Episodes of ‘desaturation’ of ≥5% from baseline were significantly fewer with the N-3000 than with the N-200 (5 vs. 19, p =0.0001), and lowest values below 90% occurred nine times on the N-200, but were not seen with the N-3000 (p 〈0.00005). Furthermore, episodes owing to loss of signal were significantly rarer with the N-3000 than with the N-200 (3 vs. 13, p =0.001). The Nellcor N-3000 oximeter may offer an advantage over the N-200 model when monitoring patients in the postoperative period.

    Notizen: A full understanding of the sequence specificity of EcoRI endonuclease requires structural information on complexes where the DNA contains one `incorrect' base pair; historically, these sites are referred to as EcoRI* sites. They are inherently asymmetric, unlike the canonical EcoRI site, GAATTC, which possesses a twofold axis of rotational symmetry. All previously determined DNA–EcoRI complexes incorporated this symmetry axis into the space group, requiring the design of `new' oligonucleotides to produce an asymmetric unit appropriate to an EcoRI* complex. The incomplete factorial approach of Carter & Carter [Carter & Carter (1979). J. Biol. Chem. 254, 12219–12223.] was used to design the DNA sequence. Factors included the location and type of EcoRI* substitution and the length and AT richness of the sequences on both sides of the RI site. Co-crystals were obtained with several sequences, including one with TCGTGGACTTCGTG. Diffraction data were collected from one crystal of this complex to 3.2 Å resolution; the unit-cell parameters are a = b = 123.8 and c = 148.9 Å and the space group is P3221. Unit-cell and space-group information was also obtained for the EcoRI* sites AAATTC, GGATTC and GAGTTC. These experiments demonstrated the need for a rapid, economical method that would distinguish DNA-protein co-crystals from crystals of protein only. This can be readily achieved with a single small crystal and a staining method based on methylene blue and methyl violet, which stain DNA and protein, respectively.

    Notizen: Abstract To assess the preliminary efficacy of rituximab therapy in Waldenstrom's macroglobulinemia (WM), we examined the clinical and laboratory data for all patients with WM treated on IDEC Pharmaceuticals sponsored trials and one patient treated at Walter Reed Army Medical Center. Seven symptomatic patients with WM were treated with four (n = 6) or eight (n = 1) weekly infusions of rituximab (375 mg/m2). Patients had received a median of three prior therapies (range 1–4) which included alkylator therapy in all (five patients refractory) and fludarabine in four (all refractory). Therapy was tolerated well in all patients without decrement in cellular immune function or significant infectious morbidity. Partial responses were noted in three of these patients, including two with fludarabine-refractory disease. The median progression-free survival for these patients was 6.6 months (range 2.2–29+ months). These data suggest that rituximab has clinical activity in heavily pre-treated patients with Waldenstrom's macroglobulinemia. Based on these data, clinical studies of Rituximab in previously untreated and treated WM appear indicated.

    Notizen: Abstract Objective: A randomized double-blind controlled trial of doxapram versus placebo against late postoperative hypoxaemia was planned to include 40 patients (2 × 20). Results: After inclusion of 18 patients a serious adverse event was encountered with development of a brain stem infarction in a 90-year-old woman receiving doxapram. At this point the randomization code was broken and we decided to terminate the trial. Three of nine patients receiving doxapram had had an adverse event whereas none of the patients receiving placebo had adverse events (P = 0.2). In the 18 patients studied, there was an insignificant trend towards higher mean oxygen saturation in the doxapram group, and a significantly higher minimum oxygen saturation and reduced number of hypoxaemic events on the first postoperative night. Conclusion: Although these preliminary data on the effect of doxapram on postoperative hypoxaemia seem promising, further studies on the effect of continuous nocturnal postoperative doxapram infusion on levels of arterial oxygen saturation should be postponed until more knowledge about the pharmacokinetics of doxapram in this particular clinical situation has been gathered.
