    Notizen: Abstract  Objective. To demonstrate the value of MR imaging in the diagnosis and differentiation of the various symptomatic complications of osteochondromas, providing pathological correlation with emphasis on the usefulness of MR imaging as a single imaging modality in these patients. Design. We retrospectively reviewed all MR examinations of clinically symptomatic osteochondromas (30 patients) performed at our institution between March 1990 and October 1997. Patients. Thirty patients had clinically symptomatic osteochondromas during the study period. Twenty patients were male and 10 were female. There were five cases of multiple osteochondromatosis. Pathological correlation was available in 24 patients. Results and conclusion . Symptomatic complications included fracture (7%), osseous deformity limiting range of motion (23%), vascular injury (7%), neurological compromise (10%), bursa formation (27%) and malignant transformation (27%). MR imaging was able to diagnose or suggest the etiology for the clinical symptomatology in all cases, demonstrating that it is an ideal imaging modality in the diagnostic evaluation of symptomatic complications of osteochondromas and often avoids the need for further imaging.

    Notizen: Abstract  The MRI features of two cases of spinal Langerhans’ cell histiocytosis with multilevel involvement are presented in which MRI was of help in differentiating active from inactive healing lesions by the demonstration of signal changes in the vertebral body marrow of the active lesion, manifest as low signal intensity on T1-weighted sequences and high signal intensity on T2-weighted sequences. This distinction could not be made by plain radiography or bone scintigraphy. In cases where biopsy is required for diagnosis, MRI is recommended to guide the biopsy towards levels suggestive of active involvement.

    Notizen: Abstract A total of 235 lesions in 187 patients were treated radiosurgically, including 71 primary malignant brain tumors, 113 metastases, 38 arteriovenous malformations, and 13 benign brain tumors. Postradiosurgery tissue was obtained from 36 lesions in 32 of these patients (17% of total) who developed radiographic or clinical progression/necrosis—entailing 27 resections/biopsies and 9 autopsies. Of these 32 patients, 18 had primary malignant brain tumors, 11 had metastasis, and 3 had benign brain tumors. Central or peripheral residual tumor was seen in 27 of 36 (75%) specimens, including 75% of metastases, 80% of malignant brain tumors, and 50% of benign brain lesions. Residual disease was designated to be central or peripheral to the radiosurgery target. Among primary brain malignancies, the 6- and 12-month actuarial risk for persistent central/peripheral tumor was 70/75% and 55/90%, respectively. For metastatic lesions, the 6- and 12-month actuarial risk for persistent central/peripheral tumor was 60/82% and 55/82% respectively. Radiographic changes consistent with progression were confirmed histopathologically in 75% of cases and viable tumor was most common at the periphery, correlating with lower isodose lines.
