    Description / Table of Contents: Zusammenfassung. Von Januar 1987 bis März 1994 wurden an drei Magdeburger Krankenhäusern insgesamt 103 subcutane Achillessehnenrupturen operativ versorgt. Unter Anwendung eines selbstentwickelten Scores konnten die verwendeten Operationsmethoden miteinander verglichen werden. In diesen Score flossen Auswertungsergebnisse aus Krankenunterlagen, histologischem Befund, klinischer Nachuntersuchung und isokinetischer Messung von Kraft und Ausdauer mittels eines KIN-TREX®-Dynamometers ein. Das Gesamtergebnis aller angewandten Operationsmethoden beträgt unter Nutzung des Scores 767 von 1000 möglichen Punkten. Dabei liegen die End-zu-End-Naht (781 Punkte), die Umkipplastik (772 Punkte) und die Fibrinklebung mit 754 Punkten in etwa gleichauf. Die Theorie von der Unabdingbarkeit der histologischen Degeneration in der Ätiologie der Ruptur ist wohl endgültig als nicht zutreffend zu bezeichnen. Zur Erarbeitung eines Regimes der postoperativen Belastbarkeit kann die isokinetische Messung sehr hilfreich sein.
    Notes: Summary. From January 1987 to March 1994 three hospitals in the city of Magdeburg treated a total of 103 subcutaneous Achilles tendon ruptures operatively. We created a special score to compare the outcome of the different operation techniques used. Important aspects of the follow-up examination were isokinetic measuring of power and staying power by means of a KIN-TREX dynamic measuring device and the clinical check-up in comparison with the histological results and the patients' history. Degeneration could only be proved histologically in 38,8 % of the cases. The average score of all OP methods applied was 767 of possible 1000 points. The adaptation suture (781 points), Silfverskiold's technique (772 points) and the fibrin adhesive technique (754 points) showed approximately the same results. The theory that all of the ruptures have a histological degeneration background seems to be wrong. Isokinetic measuring can be very helpful in preparing a postoperative regimen to find the optimum load.

    Notes: Abstract Objective: Inhalation of high concentrations of nitric oxide (NO) has been shown to improve gas exchange and to reduce pulmonary vascular resistance in individuals with ischemia-reperfusion injury following orthotopic lung transplantation. We assessed the cardiopulmonary effects of low doses of NO in early allograft dysfunction following lung transplantion. Design: Prospective clinical dose- response study. Setting: Anesthesiological intensive care unit of a university hospital. Patients and participants: 8 patients following a single or double lung transplantation who had a mean pulmonary arterial pressure (PAP) in excess of 4.7 kPa (35 mmHg) or an arterial oxygen tension/fractional inspired oxygen ratio (PaO2/FIO2) of less than 13.3 kPa (100 mmHg). Interventions: Gaseous NO was inhaled in increasing concentrations (1, 4 and 8 parts per million, each for 15 min) via a Siemens Servo 300 ventilator. Measurements and results: Cardiorespiratory parameters were assessed at baseline, after each concentration of NO, and 15 min after withdrawal of the agent [statistics: median (25th/75th percentiles: Q1/Q3), rANOVA, Dunnett's test, p 〈 0.05]. Inhaled NO resulted in a significant, reversible, dose-dependent, selective reduction in PAP from 5.5(5.2/6.0) kPa at control to 5.1(4.7/5.6) kPa at 1 ppm, 4.9(4.3/5.3) kPa at 4 ppm, and to 4.7(4.1/5.1) kPa at 8 ppm. PaO2 increased from 12.7(10.4/17.1) to 19.2(12.4/26.0) kPa at 1 ppm NO, to 23.9(4.67/26.7) kPa at 4 ppm NO and to 24.5(11.9/28.7) kPa at 8 ppm NO. All patients responded to NO inhalation (either with PAP or PaO2), all were subject to long-term inhalation (1–19 days). All were successfully weaned from NO and were discharged from the intensive care unit. Conclusion: The present study demonstrates that low-dose inhaled NO may be an effective drug for symptomatic treatment of hypoxemia and/or pulmonary hypertension due to allograft dysfunction subsequent to lung transplantation.

    Description / Table of Contents: Zusammenfassung. Zur Behandlung der habituellen Patellaluxation werden in der Literatur viele differente Behandlungskonzepte diskutiert. Von 1987 bis 1997 wurden aus unserem Patientengut 88 Patienten mit einer Medialisierung der Tuberositas tibiae ein- oder doppelseitig versorgt. Die Kombination mit einem lateralen Release erfolgte bei 21 Operationen. Nach einem durchschnittlichen Zeitraum von 4,9 Jahren konnten 79 Patienten klinisch, radiologisch sowie mittels isokinetischer Kraftmessung (Kintrex-Dynamometer) nachuntersucht werden. Die Durchführung der isokinetischen Kraftmessung im Vergleich mit der Gegenseite ergab in der Streckung ein durchschnittliches Defizit im maximalen Drehmoment von 19 nm. In der Betrachtung der klinischen Gesamtergebnisse konnten wir nach den Kriterien des Scores von Juliusson und Markhede [11] in 51 Fällen gute, in 18 Fällen befriedigende und in 10 Fällen schlechte Resultate feststellen. Eine geringe oder keine postoperative Änderung im Ausübungsverhalten beim Sport im Vergleich mit der Zeit vor der Operation beobachten wir in 79 % der Fälle. Für die Beurteilung des Operationserfolges stellt die isokinetische Kraftmessung mittels Kintrex, zur Analyse des Kraftentwicklungsverlaufs, eine probate Methode dar.
    Notes: Summary. Referring to relevant literature, various concepts are discussed in the treatment of habitual dislocation of the kneecap. Between 1987 and 1997, 88 patients have been treated surgically in our clinic by uni- or bilateral medial transfer of the tibial tuberositas. In 21 of the cases lateral release was also performed. In the follow-up, after an average of 4.9 years, 79 patients were examined clinically, radiologically and isokinetically (Kintrex-Dynamometer). Isokinetic measurement of the extension showed an average deficit of 19 nm compared to the contralateral side. Using the Juliusson and Markhede scores, 51 cases had a good, 18 a fair and 10 a poor clinical outcome. In 79 % of the cases, the postoperative sports activity level was the same as preoperatively. Isokinetic measurement with the Kintrex is a useful method of evaluating the development of muscle strength.

    Notes: An algorithm for solving large BEM equation sets arising when using the substructuring option is described. The solver is based on a frontal solution philosophy. This feature along with a special ordering scheme for the unknowns minimizes the effect of the fill-in. The condensation of unknowns is carried out in two loops running over all substructures. Within the first loop all unknowns associated with nodes at the external surface of the body are eliminated. The remaining interfacial unknowns are condensed within the second loop. The software has a built-in mechanism of recognizing whether the given step of condensation of unknowns can be carried out in-core or out-of-core. This limits the usage of slow direct access files to the necessary minimum. The implemented multiple right-hand side option makes the solver suitable for non-linear applications. Examples of such applications in non-linear heat transfer problems and plasticity are discussed. A parallelized version of the solver uses the parallel virtual machine (PVM) software and is portable to many computer systems. The parallelization is based on a concept following the block solver philosophy. The parallel solver might be run on computer clusters consisting of processors of different power. A special load equivalencing algorithm has been developed to assign proper task to computers of different power. The solver has been run in an industrial environment to solve problems of about 100 thousand unknowns. Numerical examples are included.
