    Notes: Abstract We report six cases of segmental agenesis of the internal carotid a. discovered in adult patients: one case of cervical segmental agenesis (no. 1), one case of cervical and petrosal segmental agenesis (no. 2), two cases of vertical cavernous segmental agenesis (nos. 3 and 4) and two cases of distal segmental agenesis, one unilateral (no. 5) and the other bilateral (no. 6). The collateral pathways observed were: the ascending pharyngeal a. which constitutes an “intratympanic course” of the internal carotid a. (no. 1), an intercarotid anastomosis (no. 2), an arterial network at the base of the skull, the so-called “rete mirabile” (nos. 3 and 4) and the posterior communicating a. (nos. 5 and 6). Recognition of these rare dysgeneses relies upon the following radio-anatomic characteristics: reduced caliber of the a., inconsistent sparing of the carotid body, reduced diameter or even absence of the carotid canal and above all, the presence of collateral pathways. The collateral pathways allow an understanding of the segmental nature of carotid a. development and a distinction between congenital and acquired stenoses.

    Notes: Abstract We report on the case of a 25-year-old female with severe systemic lupus erythematosus (SLE) who presented with pancytopenia, fever, arthralgia and abdominal pain. After antibiotic treatment, the patient was afebrile for 3 days before her temperature rose again. Dyspnoea and cough pointed towards pneumonia which was confirmed by X-ray. Different antibiotics and the antimycotic agent fluconazol were given. The lupus flare was treated with high-dose prednisolone. After a couple of days, the dyspnoea increased and mechanical ventilation became necessary. Bronchoscopy and transbronchial biopsy revealed the diagnosis of invasive aspergilloses. Despite of an immediate treatment with amphotericin B, the patient died because of respiratory insufficiency. The literature on aspergillosis in SLE is reviewed and prophylactic, diagnostic and therapeutic options are discussed for this infectious complication which has an 80% mortality in patients with SLE.

    Notes: Abstract The MAGE-1 gene encodes a protein encompassing a HLA-A1-restricted target epitope for cytolytic T lymphocytes. Monoclonal antibodies directed against the MAGE-1 protein were tested for usage in immunohistology of routine pathology material. Seven formalinfixed, paraffin-embedded malignant melanomas were studied by the Avidin-Biotin complex (ABC) method with or without different antigen retrieval methods. Native, frozen tissues from the same tumours were used to validate the results by immunohistochemistry on frozen sections, by PCR for mRNA and by protein demonstration in tissue extracts using western blotting. Of 4 monoclonal antibodies tested, mAB 34B and mAB 77B were highly efficient in detecting MAGE-1 protein in deparaffinised sections with the regular ABC method after microwave pretreatment. In a series of an additional 28 patients 75% expressed MAGE-1, 50% in a substantial proportion. Follow-up studies in 6 patients indicate that the expression pattern remains stable but may change substantially within a short range. Immunohistology is thus a rapid and well-established method that might be used to select and monitor HLA-A1 positive patients with malignant melanoma and other candidate tumours for MAGE-1-directed immuno-therapy.

    Description / Table of Contents: Résumé Six observations d'agénésies segmentaires de l'artère carotide interne, découvertes chez l'adulte, sont rapportées : un cas d'agénésie du segment cervical (no 1), un cas d'agénésie des segments cervical et pétreux (no 2), deux cas d'agénésie du segment caverneux vertical (no 3 et 4) et deux cas d'agénésie distale de l'artère, unilatérale (no 5) et bilatérale (no 6). Les voies de suppléance observées sont : l'artère pharyngienne ascendante réalisant un “trajet intratympanique” de l'artère carotide interne (no 1), une anastomose intercarotidienne (no 2), un réseau artériel de la base du crâne, dit “réseau admirable” (no 3 et 4) et l'artère communicante postérieure (no 5 et 6). La reconnaissance de ces rares dysgénésies repose sur les caractéristiques radio-anatomiques suivantes : réduction de calibre de l'artère, respect inconstant de sa portion bulbaire, réduction des dimensions voire absence du canal carotidien et surtout voies anastomotiques utilisées. Les anastomoses observées permettent en effet de comprendre le caractère segmentaire du développement de l'artère et de distinguer les sténoses congénitales et les sténoses acquises.

    Notes: Abstract Objective To clarify the influence of an elevated carboxyhemoglobin (COHb) blood level on the course of acute myocardial infarction and to evaluate the administration of supplemental oxygen on the COHb level and the incidence of complications. Design Prospective clinical study with randomized, unblinded intervention. Setting Coronary Care Unit of a university hospital. Patients 78 consecutive patients with acute myocardial infarction. Excluded were patients with severe dyspnea, pulmonary edema or any other medical indication for supplemental oxygen therapy. Interventions Randomized therapy with 4l/min oxygen in 35 patients.Measurements and results: COHb was measured at admission and 4 h later. The incidence of serious arrhythmias and the maximal creatine kinase (CK) values were recorded. In patients with initial COHb≧5%, there were significantly more arrhythmias and significantly higher maximal CK values than in those with normal COHb at admission (89 vs 33%,p〈0.001; and 1897±1602 u/l vs 960±1097 u/l,p=0.05). This effect was seen only in patients with Q-wave infarction, not in those with non-Q-wave infarction. Supplemental oxygen had no effect on the incidence of arrhythmias. Conclusions We conclude that myocardial infarction patients with acute Q-wave infarction and increased COHb levels at admission suffer a more severe course of the disease. This outcome was not influenced by oxygen therapy. Whether this finding indicates a causal relationship and whether higher oxygen concentrations would favorably alter the course of acute myocardial infarction remain to be determined.

    Notes: Summary Reported is the case of a 60-year-old female who was referred to our rheumatology center because of a high titer of antinuclear antibodies (ANAs). Initially several lupus-like features suggested a diagnosis of undifferentiated connective tissue disease (UCTD). Immunofluorescence of the patient's serum showed a nuclear dot pattern. Counterimmunoelectrophoresis for antibodies to Sm, RNP, SS-A and SS-B were negative. Immunoblotting revealed a band at 53 kD which did not correspond to known autoantigens. Several months later the patient presented to the emergency room with symptoms due to a hypertensive crisis. Check-up finally revealed a cortisone-producing tumour of the right adrenal gland. The adenoma was removed by surgery and the patient did well again.

    Notes: Abstract In 1999 after a 3-year transit, the Near-Earth Asteroid Rendezvous (NEAR) spacecraft will enter a low-altitude orbit around the asteroid, 433 Eros. Onboard the spacecraft, five facility instruments will operate continuously during the planned one-year orbit at Eros. One of these instruments, the NEAR Laser Rangefinder (NLR), will provide sufficiently high resolution and accurate topographical profiles that when combined with gravity estimates will result with quantitative insight into the internal structure, rotational dynamics, and evolution of Eros. Developed at the Applied Physics Laboratory (APL), the NLR instrument is a direct-detection laser radar using a bistatic arrangement. The transmitter is a gallium arsenide (GaAs) diode-pumped Cr:Nd:YAG (1.064-µm) laser and the separate receiver uses an extended infrared performance avalanche-photodiode (APD) detector with 7.62-cm clear aperture Dall–Kirkham telescope. The lithium-niobate (LiNbO3) Q-switched transmitter emits 15-ns pulses at 15.3 mJ pulse-1, permitting reliable NLR operation beyond the required 50-km altitude. With orbital velocity of 5 m s-1 and a sampling rate of 1 Hz, the NLR spot size provides high spatial sampling of Eros along the orbital direction. Cross-track sampling, determined by the specific orbital geometry with Eros, defines the resolution of the global topographic model; this spacing is expected to be 〈500 m on the asteroid's surface. Combining the various sources of range errors results with an overall range accuracy of 6 m with respect to Eros' center-of-mass. The NLR instrument design, perfomance, and validation testing is decribed. In addition, data derived from the NLR are discussed. Using altimetry data from the NLR, we expect to estimate the volume of 433 Eros to 0.01% and its mass to 0.0001% accuracies; significantly greater accuracies than ever possible before NEAR.
