    Description / Table of Contents: Zusammenfassung Wenn die revidierteGram-Schmidt-Methode in der Orthonormalisierung einer gegebenen Menge magerer Vektoren angewendet wird, so kann die lokale Füllung von Nicht-Null-Elementen bei jedem Schrift leicht bestimmt werden. Dies ermöglicht eine Neuordnung der verbleibenden Vektoren bei jedem Schrift, die dann eine möglichst kleine lokale Füllung ergibt. Es wird hier ferner gezeigt, daß eine ähnliche Methode auch im Fall derHouseholder-Triangulisierungsmethode verwendet werden kann.
    Notes: Summary If the RevisedGram-Schmidt method is used for orthonormalizing a given set of sparse vectors, then it is shown that the local fill-in of non-zero elements at each stage can be easily determined. This makes it possible to rearrange the remaining vectors at each stage such that the local fill-in is minimized. It is also shown that a similar method can also be used in the case of theHouseholder triangularization method.

    Notes: Summary In this note, a cost criterion function for choosing the optimal pivot at each stage of the Gaussian elimination method is described. It takes into consideration both the fill-in and the number of arithmetical operations. Other known criterion functions byTewarson [4] andMarkowitz [6] are also discussed and compared with the new criterion function.

    Notes: Abstract Long-term effects of cornstarch (CS) therapy on biochemical values and physical growth in children with type I glycogen storage disease (GSD I) were compared to those of children receiving continuous nocturnal nasogastric glucose feedings (CNG). Only patients who had received more than 5 years of dietary therapy (either CS or CNG) were evaluated. Six patients (five female, age 13.5 years±1.3, range 11.7–16.5 years) received CS (1.75–2.5 g/kg, four times daily) and seven patients (five female, age 9.6±2.5 years, range 7.3–14.8 years) received CNG. Blood glucose, lactate, cholesterol and triglyceride levels were not significantly different between the two methods of treatment. All patients maintained linear growth rates normal for their age. The standard deviation score of height after 6.7±1.6 years (range 5–9 years) of CS treatment was −1.29±0.59 and after 8.8±2.4 years (range 7–14 years) of CNG was −1.24±0.63. These values did not differ significantly from each other or from the target height, an estimate of genetic potential for height as determined from parental heights. With the exceptions of diarrhea, increased flatulence and excess weight gain, there were no adverse effects of CS after 9 years of treatment. Our data suggests that cornstarch is a simple, effective and safe therapy for GSD I.

    Notes: Abstract In the construction of requirements specifications for a given problem, it is generally recommended that overspecification of the problem solution be avoided [1]. Such overspecification tends to distort the functionality required by suggesting implementation concepts at the outset. In order to avoid overspecification, and to provide a support environment to the specifier, a four-stage system design environment is proposed and has been partially implemented over a three-year period. The theoretical basis of this approach is explained in [2]. Our system-level specification language (DODAN) is developed in [3]. Based on this specification language, a prototype (DAA) [4, 5, 6] for supporting the requirements specification stage has been implemented. Our other protype (DARMS) addressing the module specification stage was developed in [7, 8]. In this paper, the current status of the project is described with an emphasis on the requirements specification stage.

    Notes: Summary In glycogen storage disease type III (glycogen debranching enzyme (DE) deficiency), the activities of serum alanine aminotransferase, aspartate aminotransferase and lactate dehydrogenase may be strikingly elevated during childhood but are low during adult life. To determine the pattern of the elevated serum enzyme activities in relationship to diet, the biochemical subtype and clinical symptoms, 13 patients with DE deficiency were studied. Activities of serum aspartate and alanine transaminases, lactate dehydrogenase, and alkaline phosphatase were markedly elevated during infancy. Continued elevation of enzyme activities during childhood appeared to be related to DE deficiency in liver, but unrelated to DE deficiency in muscle. Activity elevations correlated inconsistently with diet and poorly with childhood growth rate or the presence of hypoglycaemia. The serum enzyme activities declined around puberty concomitantly with a decrease in liver size. Although periportal fibrosis and micronodular cirrhosis indicated the presence of hepatocellular damage during childhood, the decline in serum enzyme activities with age and the absence of overt hepatic dysfunction suggest that the fibrotic process may not always progress.

    Notes: Summary Renal disease is a frequent and serious complication of type I glycogen storage disease. A type I glycogen storage disease patient with focal segmental glomerulosclerosis and progressive renal insufficiency underwent a renal allograft transplantation. Despite the same cornstarch therapy, the post-transplantation course was complicated by worsening of the metabolic control manifested by exacerbated lactic acidaemia and hyperlipidaemia. This lactic acidaemia was remarkable for its association with hyperglycaemia. Hyperglycaemia accompanied by lactic acidaemia is strikingly unusual in type I glycogen storage disease, since this is a disease characterized by hypoglycaemia and an inverse relationship between blood glucose concentration and lactate levels. Both fasting insulin and C-peptide levels in the patient were greater than similar age-matched type I glycogen storage disease controls, indicating hyperinsulinaemia. The most likely mechanism responsible for the combined hyperglycaemia and lactic acidaemia was insulin resistance due to glucocorticoid treatment, instituted for immunosuppression. The hyperglycaemia associated with the lactic acidaemia was transient and resolved with steroid tapering. The exacerbated hyperlipidaemia, however, persisted after renal transplantation. Type I glycogen storage disease patients may be prone to glucocorticoid-induced insulin resistance, since the cellular metabolism in these patients may already be compromised with ineffective insulin action and/or reduced insulin output.

    Notes: Abstract Tl-201 myocardial scintigraphy (Tl study) tends to be attenuated by soft tissues (such as the diaphragm) due to its low energy emission. 99mTc-sestamibi (2-methoxy isobutyl isonitrile) is a relatively new agent with a higher energy emission and this characteristic accounts for the higher quality of 99mTc-sestamibi images. The purpose of this study is to evaluate the ability of 99mTc-sestamibi in alleviating the inferior attenuation of Tl studies. 99mTc-sestamibi SPECT myocardial scintigraphy was performed on 13 patients with inferior wall perfusion defects as determined by Tl study (but with normal coronary artery as evidenced by cardiac catheterization). All patients underwent Tl SPECT study using a standard procedure. Same-day protocol (rest-stress sequence) was used for 99mTc-sestamibi SPECT imaging. All images were analyzed by two independent observers. The results of our study reveal that 99mTc-sestamibi produced better images. The inferior wall perfusion defects in the Tl study were noted in one case only (l/13)in the 99mTc-sestamibi study. Our study suggests that 99mTc-sestamibi can remarkably reduce the inferior attenuation of Tl study.

    Notes: By the differential operator representation of constitutive equations for linearly viscoelastic materials, general theoretical expressions are developed for the physical parameters of a composite system in relation to those of the constituents. The interfacial tension between the dispersed phase and continuous matrix is included in the derivation. The results show that it has effect only on the shear parameter but not on the bulk parameter of the composite system. The general expressions developed in this article represent a unified theory for composite systems. The results are shown to reduce to many special cases obtained by other investigators.
