    Notes: Zusammenfassung Bei 50 Patienten mit primärer Panmyelopathie wurde der Erythrocytenumsatz mit Hilfe der Cr51-Markierung untersucht. In den meisten Fällen ergab sich eine mäßige Verkürzung der Erythrocytenlebenszeit, während sich nur in der Minderzahl der Fälle eine absolute Senkung der Produktionsrate eindeutig nachweisen ließ. Die Anämie ist also im allgemeinen durch einen beschleunigten Erythrocytenabbau bei relativer Insuffizienz des erythropoetischen Knochenmarks bedingt. Aus den Reticulocytenzahlen können bei Panmyelopathien keine sicheren Rückschlüsse auf den peripheren Erythrocytenumsatz gezogen werden.

    Notes: Summary Thrombotic thrombocytopenic purpura (TTP) is a hematologic disorder which is clinically characterized by thrombocytopenia, microangiopathic hemolytic anemia, fever, neurologic symptoms, and cardiac and renal involvement. The pathogenic mechanisms of this disease are poorly understood. It is well known that TTP is associated with pregnancy and that prognosis for the mother and child is poor. We present the first case of a severe TTP diagnosed in the first trimester of pregnancy (13th week of gestation) with maternal survival and birth of a healthy child which required continuous and intensive treatment with plasmatherapy until delivery. During a period of 24 weeks several attempts to discontinue plasma therapy failed because of continuous active disease, and it became evident that plasma infusions were not as effective as plasma exchanges. The fact that the patient entered into remission soon after delivery of a healthy child by cesarean section in the 37th gestational week shows that in this case pregnancy activated an unknown factor which does not cross the placenta and which can be removed by plasmapheresis.

    Notes: Summary Aggressive chemotherapy of advanced myelodysplastic syndrome (MDS), acute myeloid leukemia (AML) evolving from MDS, subacute AML and secondary AML has usually been associated with low complete remission (CR) rates, a high incidence of early death, and low disease-free survival. We therefore have initiated a phase-III trial of aggressive chemotherapy consisting of idarubicin, cytosine arabinoside, and VP-16 to improve the CR rate. Each chemotherapy cycle is followed by G-CSF to accelerate neutrophil recovery and to reduce the incidence of infections. Until now, 19 patients with high-risk AML have been entered. The CR rate is 47%, with only one death during induction. Patients achieving CR are randomized to receive either high-dose or low-dose interleukin-2 to eliminate residual leukemic cells and to prolong the duration of remission.

    Notes: Summary High-titered spontaneous interferon (IFN) antibodies were detected in a patient with pure red cell aplasia (PRCA), a benign mediastinal tumor, and recurrent cutaneous carcinomas. The circulating IFN antibodies reacted broadly with various human IFN-α subtypes (20–140×103 neutralizing units/ml serum) but not with IFN-β or IFN-γ, and they neutralized the antiviral activity of the patient's endogenous IFN-α. The IFN-αbinding activity was restricted to the IgG1 subclass in a nonmonoclonal manner. Whereas the PRCA repeatedly responded to immunosuppression with high-dose cyclosporin A (CSA) and CSA plus plasmapheresis, IFN antibody production continued during treatment with cyclophosphamide and CSA. Serological analysis revealed past infection with parvovirus B19 and persistent B19 IgM titers. Antibody-mediated impairment of the IFN-α system might have favored the development of both PRCA and the various cutaneous carcinomas in this patient.

    Notes: Summary The therapeutic efficacy of interferon-alpha (IFN-α) in the treatment of chronic myeloid leukemia is currently being tested in a number of institutional, interinstitutional, and international trials. There is no doubt that responses are achieved in many patients, and in a small subset complete eradication of clonogenic cells may be possible. However, it has not yet been shown that overall survival of patients treated with IFN-α is better than that of those treated with conventional cytoreductive drugs. There are still controversial opinions on problems such as dosages and duration of treatment, combination with cytostatic agents, definition of responses, and relevance of cytogenic and molecular data. An international workshop discussed the data on interferon therapy and attempted to define the role of interferon today in the management of chronic myeloid leukemia.

    Notes: Summary The incidence of invasive fungal infections after bone marrow transplantation (BMT) was analyzed in 303 consecutive marrow graft recipients (allogeneicn=271, autologousn=27, syngeneicn=5). All patients received inhalations with amphotericin B (10 mg twice daily) during neutropenia. The overall incidence of invasive fungal infections within the first 120 days after transplant was 3.6% (11/303; aspergillosis: 6; yeast infection: 5). Four of the 11 cases occurred early, and seven cases were observed after neutrophil recovery and discontinuation of amphotericin B inhalation treatment. Late infection was significantly associated with the development of acute graft-versus-host disease. Four of the 11 infections (early 2/4; late: 2/7) were observed in patients with a history of previous fungal infection. Other patient and treatment characteristics were not helpful in defining potential risk factors. In particular, the incidence of invasive fungal infections did not differ between patients with more or less strict reverse isolation measures. Occasional side effects such as initial mild cough and bad taste were rare, usually disappeared during continued administration, and were in no case the reason for discontinuation of treatment. These data suggest that aerosolized amphotericin B may be a useful, convenient, and efficient prophylactic antifungal regimen in BMT.

    Description / Table of Contents: Summary In 20 cases of acquired haemolytic anaemia due to warm antibodies, measurements of surface activity after tagging of the erythrocytes was radio-chromium was carried out. In more than half the cases there were signs of increased sequestration of erythrocytes, mainly in the spleen. Some of the patients could be examined several times, using different experimental conditions. No definite relationship could be found between that distribution type of erythrocyte sequestration and the haematological and serological findings. In 7 cases splenectomy was performed and measurements of surface activity compared with the earlier ones.
    Notes: Zusammenfassung Bei 20 Patienten mit erworbener hämolytischer Anämie durch Wärmeautoantikörper wurden Oberflächenaktivitätsmessungen nach Radiochrommarkierung der Erythrozyten durchgeführt. In mehr als der Hälfte der Fälle zeigten sich Hinweise auf eine erhöhte Erythrozytensequestration überwiegend in der Milz. Einige Patienten konnten mehrmals unter verschiedenen Bedingungen untersucht werden. Zwischen dem Verteilungstyp der Erythrozytensequestration und den hämatologischen und serologischen Befunden ließ sich keine bestimmte Beziehung erkennen. Bei sieben Patienten wurde die Milz entfernt und das Ergebnis der Oberflächenaktivätsmessung mit dem Operationserfolg verglichen.

    Notes: Summary From a population-based study on the incidence of potentially drug-associated blood dyscrasias 28 cases were identified with pancytopenia. Who recovered within 90 days after diagnosis. Early recovery occured more frequently in patients showing normal or increased cellularity of the bone marrow than in patients with bone marrow hypoplasia. Median recovery times of leukocytes were 14 and 10 days and of platelets 21 and 9 days in patients with and without bone marrow hypoplasia, respectively. Age and sex distribution were similar in both groups. Of 28 patients, 11 reported a period of fever before onset of pancytopenia. Sixteen patients in whom information on drug use was available had taken a median of 4 drugs before the onset of symptoms that were related to pancytopenia. From these results we present the hypothesis that transient pancytopenia with or without marrow hypoplasia can be the expression of the same type of bone marrow injury and that drugs or viral infections should be considered as etiological factors.
