    Notes: Abstract Molecules in the excited singlet and triplet state and photoproducts occuring after photoisomerization or proton transfer are investigated by time-resolved resonance Raman and resonance CARS spectroscopy with nanosecond time resolution. From the time-resolved spectra information on structures and bond strengths of these short-lived species and on their kinetics are obtained.

    Notes: Summary Percutaneous lumbar discectomy has gained growing interest during recent years as an alternative to open surgery for protrusions and non-sequestrated subligamentous intervertebral disc herniations. As a less invasive method it competes with chemonucleolysis. At least two modifications are known to date: automated percutaneous lumbar discectomy (APLD) with a 2-mm suction probe and non-automated, discoscopy-monitored percutaneous lumbar discectomy with a suction rongeur and a motor-driven shaver (NAPLD). In this study these two methods are compared for the amount of material extracted, dependence upon the degree of degeneration of the disc and intrinsic technical problems, using 20 human cadaver lumbar specimens for experimental testing. Total nucleotomy was not possible with either method. APLD yielded significantly less material and proved to be less effective in severely degenerated intervertebral discs than the NAPLD procedure, as preexisting gaps within the degenerated nucleus pulposus allowed the tissue to shift away from the tip of the 2-mm probe and facilitated displacement of the probe within the anulus fibrosus. By contrast, the rongeur, which first cuts the material to be removed and then carries it away by suction, was much more effective. Further data to support the advantages of non-automated percutaneous nucleotomy are discussed. Pretreatment of the disc with chymopapain did not result in a higher yield of nucleus material when combined with APLD.

    Notes: Summary In an experimental study with 18 minipigs, we have tried to establish a model for the standardized evaluation of mechanical, histological and radiological phenomena of degenerative and reparative processes within the lumbar motion segment. Comparing different operative techniques revealed that the intradiscal application of hyaluronic acid into the nuclear defect is likely to enhance the regeneration process. Using the measurement of disc compliance, a semiautomatic picture analyzer and a new semiquantitative disc score could make future studies more comparable. From this basis, the intradiscal application of hyaluronic acid deserves further evaluation.

    Description / Table of Contents: Summary Based on the observation of three clinical cases of late onset (after growth arrest) spondylolisthesis, the different pathogenetic factors for lumbar instability in adulthood are discussed. A causal classification based on functional and morphological criteria clearly separates the “genuine” traumatic spondylolisthesis from other forms causing instability (with spondylolysis) within a lumbar motion segment. The possibility of developing spondylolisthesis even after growth arrest is hereby proven. Consequences for appraisal procedures are discussed, and an example for possible surgical management is given.
    Notes: Résumé A partir de trois observations de spondylolisthésis d'installation tradive, postpubertaire, les auteurs discutent les divers facteurs pathogéniques de l'instabilité lombaire chez l'adulte. Une classification basée sur des critères fonctionnels et morphologiques permet de séparer clairement les véritables spondylolisthésis traumatiques des autres formes engendrant l'instabilité du segment mobile lombaire (avec spondylolisthésis). L'éventualité de la survenue d'un spondylolisthésis même après l'arrêt de la croissance est ici démontrée. Les auteurs discutent les conséquences de cette notion pour l'évaluation et proposent une possibilité thérapeutique chirurgicale.

    Description / Table of Contents: Summary We report the case of a delayed partial posttraumatic vertebral body necrosis (PVBN) 1 year after a spinal trauma. Operative treatment became necessary due to incipient paraplegia. PVBN, also known as Kümmell-Verneuil's disease, can occur as a rare but serious complication several months or even years after a spinal trauma. Radiologically, it often presents with an intravertebral vacuum phenomenon. The underlying reasons for the necrosis as mentioned in the literature seem to be an initial microtrauma, metabolic disturbances and primarily overlooked compression fractures. Histologically, it resembles a pseudarthrosis with only little bony reparation, as in an avascular femoral head necrosis.
    Notes: Résumé Nous rapportons le cas d'une nécrose posttraumatique partielle et tardive, un an après un traumatisme rachidien. Le traitement chirurgical devient nécessaire en raison de l'installation d'une paraplégie débutante. Cette affection également connue sous le nom de maladie de Kümmell-Verneuil, peut se présenter comme une complication rare mais sérieuse pouvant survenir plusieurs moís ou années après un traumatisme du rachis. Radiologiquement la nécrose post-traumatique du corps vertébral se traduit par un vide intravertébral. Les raisons de cette nécrose proposées dans la littérature semblent être à l'origine, un microtraumatisme répété, un trouble métabolique ou une fracture tassement initialement négligée du corps vertébral. Histologiquement elle ressemble à une pseudarthrose avec une reconstruction osseuse très pauvre, comme dans l'ostéonécrose de la tête fémorale.

    Notes: Abstract This study examined the role of central adrenergic receptors in the regulation of water absorption by the rat ileum. Clonidine, an α2 -adrenergic receptor agonist, increased water absorptionin vivo following intracerebroventricular but not intravenous injection. Intracerebroventricular injection of prazosin, an α1 -adrenergic receptor antagonist, did not alter basal water absorption. Prazosin did not prevent the clonidine effect. In contrast, the α2 -adrenergic receptor antagonist, yohimbine, reduced basal absorption and prevented the intracerebroventricular clonidine effect. Treatment with reserpine reduced ileal norepinephrine content by 98%, reduced basal water absorption consistent with a loss of sympathetic outflow to the mucosa, but did not prevent the increase in water absorption due to intracerebroventricular clonidine. These results suggest that central α2 receptors regulate water absorption by the rat ileum. The clonidine-induced increase in water absorption is not mediated by the sympathetic nerves innervating the intestine.

Nous comparons cette technique à la fixation par ciment, par autogreffe osseuse et par d'autres matériaux et nous en discutons les résultats.
    Notes: Summary From 1978 to 1989, we carried out 15 resections of single vertebral bodies, with homologous femoral head transplantation, for malignant and benign lesions of the spine. The advantages of this procedure are compared to fixation with bone cement, autologous bone grafts and other materials. The results are discussed.
