Summary. Necrolytic migratory erythema is considered to be one of five well-defined paraneoplastic syndromes of the skin. An association with a pancreatic A-cell glucagonoma is commonly postulated. However, during the last years 10 cases of necrolytic migratory erythema with no evidence for glucagonoma have been published. We report on a 59-year-old woman who presented with typical clinical and histological findings of necrolytic migratory erythema. Staging and laboratory investigations gave no evidence of any neoplasia. A liver biopsy revealed chronic persisting hepatitis. Hepatocellular dysfunction was recently reported to be present concomitantly in a number of cases of necrolytic migratory erythema, suggesting a relation to the skin changes described here.

Zusammenfassung. Das Erythema necroticans migrans gilt als eine der fünf obligaten kutanen Paraneoplasien. Im allgemeinen wird von der Vergesellschaftung mit einem α-Zell-Glukagonom des Pankreas ausgegangen. Allerdings wurden in Abweichung davon im angelsächsischen Schrifttum in den letzten Jahren 10 Fälle von Erythema necroticans migrans ohne nachweisbares Glukagonom bekannt. Wir berichten über eine 59jährige Patientin mit den typischen klinischen und histologischen Befunden eines Erythema necroticans migrans, bei der wir in Staging- und Laboruntersuchungen keinen Hinweis auf das Vorliegen einer Neoplasie fanden. Die Leberbiopsie bewies eine chronisch-persistierende Hepatitis. Da auch bei anderen Fällen von Erythema necroticans migrans hepatozelluläre Störungen bestanden, mag ein Zusammenhang mit den Hautveränderungen vermutet werden.
