    Description / Table of Contents: Summary Skin fibroblast cultures were treated with various components of the blood clotting system (thrombin, fibrinogen and fibrin) during the logarithmic growth phase. Fibrin as well as thrombin showed dose-dependent growth promoting activities as revealed by cell counting and3H-thymidine uptake. No effect was seen with fibrinogen. After entrapping in polymerizing fibrin enriched by complete culture medium the cells elongated, multiplied and formed net-like interconnecting cell strands throughout the clots. Nutritional deprivation appeared as a limiting factor for eventual growth cessation. The results demonstrate active growth of fibroblasts in fibrin clots such as present in healing wounds and thrombi. The production of thrombin by the coagulation cascade does not only result in the conversion of fibrinogen to fibrin but has also a long-lasting hormone-like effect on fibroblast proliferation which is of essential importance in wound healing, thrombus organization and progression of chronic atherosclerotic lesions.
    Notes: Zusammenfassung Fibroblastenkulturen wurden während der logarithmischen Wachstumsphase mit verschiedenen Komponenten des Gerinnungssystems (Thrombin, Fibrinogen, Fibrin) behandelt. Die Beeinflussung der proliferativen Aktivität der Fibroblasten erfolgte durch Messung der3H-TdR-Einbaurate sowie durch Zellzählung. Fibrin und Thrombin zeigten eine konzentrationsabhängige Steigerung des Zellwachstums. Fibrinogen in verschiedenen Konzentrationen war ohne Effekt. Durch Züchtung von Fibroblasten in polymerisierendem, mit Nährmedium angereichtertem Fibrin entstanden elongierte, sich im Gerinnsel netzförmig ausbreitende Zellverbände. Die Zellen vermehrten sich um das Achtfache, nach etwa 10tägiger Kulturdauer sistierte das Zellwachstum infolge nutritiver Deprivation. Die Ergebnisse zeigen, daß Fibroblasten sich in Fibringerinnseln ausbreiten und vermehren. Thrombin bewirkt neben der Fibrinogen-Fibrin-Conversion eine anhaltende Steigerung der Fibroblastenproliferation, die bei der Wundheilung, der Organisation von Thromben und der Ausbildung atheromatöser Veränderungen von Bedeutung zu sein scheint.

    Notes: Summary Thrombin, factor XIII and fobroncctin were incubated with cultures of mouse sarcoma cells, human cervix carcinoma cells (HeLa cells) and cells of an acute lymphoblastic leukemia. Thrombin induced a significant increase of3H-thymidine uptake into cells with a 1,5- to 2-fold increase of cell count. The cells of an acute lymphoblastic leukemia showed a similar response to the influence of thrombin. Factor XIII merely induced an increase of3H-thymidine uptake in tumor cells, the cell count remained constant. These results differ from experiments with cultures of normal fibroblasts. Decreased receptor density in tumor cells toward factor XIII and thrombin would play a role so that the mitogenic stimulus is impaired. The cells of an acute lymphoblastic leukemia showed under the influence of factor XIII a significant increase of cell count and thymidine uptake. HeLa cell growth was optimal at low fibronectin concentrations. Fibronectin concentrations of 1 mg/ml to 3 mg/ml inhibited HeLa- and mouse sarcoma cell growth.

    Notes: Summary Mesna (sodium-2-mercaptoethansulfonate) is used in the prophylaxis of cyclophosphamide (CYC)-induced hemorrhagic cystitis. Four patients being treated with “low dose” CYC and prednisone for vasculitis developed severe side effects to Mesna. Fever, arthralgia, myalgia, tachycardia, electrocardiogram changes consistent with perimyocarditis, erythroderma, bullous skin and mucous membrane lesions, and abdominal complaints with profuse diarrhea were noted approximately 3 weeks after the initiation of therapy for CYC-induced leukopenia and a conservatively reduced prednisone dosage. Positive reexposure tests confirmed the association to Mesna use, and hypersensitivity skin tests demonstrated a delayed hypersensitivity reaction.

    Description / Table of Contents: Summary. Necrolytic migratory erythema is considered to be one of five well-defined paraneoplastic syndromes of the skin. An association with a pancreatic A-cell glucagonoma is commonly postulated. However, during the last years 10 cases of necrolytic migratory erythema with no evidence for glucagonoma have been published. We report on a 59-year-old woman who presented with typical clinical and histological findings of necrolytic migratory erythema. Staging and laboratory investigations gave no evidence of any neoplasia. A liver biopsy revealed chronic persisting hepatitis. Hepatocellular dysfunction was recently reported to be present concomitantly in a number of cases of necrolytic migratory erythema, suggesting a relation to the skin changes described here.
    Notes: Zusammenfassung. Das Erythema necroticans migrans gilt als eine der fünf obligaten kutanen Paraneoplasien. Im allgemeinen wird von der Vergesellschaftung mit einem α-Zell-Glukagonom des Pankreas ausgegangen. Allerdings wurden in Abweichung davon im angelsächsischen Schrifttum in den letzten Jahren 10 Fälle von Erythema necroticans migrans ohne nachweisbares Glukagonom bekannt. Wir berichten über eine 59jährige Patientin mit den typischen klinischen und histologischen Befunden eines Erythema necroticans migrans, bei der wir in Staging- und Laboruntersuchungen keinen Hinweis auf das Vorliegen einer Neoplasie fanden. Die Leberbiopsie bewies eine chronisch-persistierende Hepatitis. Da auch bei anderen Fällen von Erythema necroticans migrans hepatozelluläre Störungen bestanden, mag ein Zusammenhang mit den Hautveränderungen vermutet werden.

    Notes: Summary Five patients with widespread plaque-type psoriasis were treated continuously with clobetasol under occlusion. Clinical healing was seen after 6–10 days of treatment. All plaques treated in this way clinically relapsed approximately 12 days later. During the period of remission, sequential biopsies were taken and prepared for light and electron microscopy. Histologically, the earliest indications of relapse were endothelial alterations (swelling, intercellular widening) followed by the appearance of mast cells around the postcapillary venules; these mast cells showed signs of degranulation. Hours later, activated macrophages showing pericellular edema were present, and these migrated into the epidermis soon after. Associated with the presence of macrophages, there was a complete loss of desmosome-tonofilament complexes. Later, lymphocytes and neutrophils were seen. Under these experimental conditions, the psoriatic-tissue alterations appear to have been initiated by degranulating mast cells as well as by macrophages which later invaded the epidermis.

    Notes: Summary In this report we investigate the simultaneous occurrence of psoriasis and atopic dermatitis (AD) as well as the association with infectious skin diseases. Among 29,159 patients hospitalized between 1953 and 1983, 8.5% (2,467 patients) were treated for psoriasis, while 1.6% (470 patients) were hospitalized for AD treatment. On the basis of incidence rates for both diseases, 36 patients (0.14%) with both psoriasis and AD were expected to be seen. However, the two conditions were simultaneously present in 2 patients only. Approximately 30% of the AD patients were suffering from either bacterial or viral infection, while this complication occurred in 6.7% of sporiatics. In addition, among 48 patients hospitalized for eczema herpeticatum 39 were atopics and none was psoriatic. The data demonstrate that the occurrence of psoriasis and AD in one and the same patient is quite rare and this may be related to conflicting immune defense patterns. Thus, increased sensitization against foreign protein together with high susceptibility to cutaneous infection present in AD is in contrast to high phagocyte responsiveness in psoriasis, where concurrent infections are rare.
