    Notes: In the presence of high concentrations of exogenous arachidonic acid (≥ 10 μm). eosinophils produced 15-hydroxycicosatetraenoic acid (15-HETE) in the absence of stimuli. The calcium ionophore A23187, as well as the chemotaxins used in this study-complement split product C5a, platelet-activating factor (PAF). and,N-formyl-methionyl-leucyl-phenylalanine(FMLP)–failed to increase 15-HETE production, indicating that eosinophil 15-lipoxygenase is already active Production of 15-HETE from eosinophils increased with increasing concentrations of arachidonic acid, exogenously added. Maximal 15-HETE production was observed to he 1111 ± 380 ng per 106 eosinophils at the concentration of 100μm of arachidonic acid. With low concentrations of exogenous arachidonic acid (below 2;μm). eosinophils were considered to incorporate exogenous arachidonic acid into their cell membrane, and did not produce 15-HETE. In contrast, 15-HETE formation in highly purified neutrophils (eosinophils 〈 1%) was negligible compared with that in eosinophils (.300-fold less), suggesting that 15 f HETE-forming activity in granulocytes is derived from the eosinophil 15-lipoxygetiase pathway and that neutrophils may lack 15-lipoxygcnase activity.

    Notes: In this study we report on functional characteristics of pustule as well as blood polymorphonuclear neutrophils (PMN) in patient suffering from relapsing bullous staphyloderma. Large numbers of viable PMN from newly formed pustules as well as from the peripheral blood were investigated. During the course of disease chemotactic migration, enzyme degranulation, superoxide-anion generation and leukotriene B4 production were determined simultaneously.The results revealed C5a- and NAP-1/IL-8-specific dysfunction of pustule PMN as compared with blood PMN. In contrast, FMLP-elicited functional activities of pustule PMN were only slightly affected.Our findings provide evidence that in inflamed tissue invading PMN are regulated by in situ generated mediators. C5a produced by staph. aureus-induced activation of the alternative pathway of the complement cascade represents predominant regulatory factor in situ. Furthermore, the results substantiate previous observations concerning different modulation of C5a and f-met-peptide receptors on human PMN.

    Notes: Summary Anthralin is a well-established and widely used compound for topical treatment of psoriasis. In recent years attention has been focused on the anti-inflammatory properties of anthralin, with particular reference to psoriasis. In this study the effect of anthralin on human monocyte chemotaxis, superoxide-anion generation, and enzyme degranulation, were investigated. For comparison, the effect of the clinically inactive anthralin derivative danthrone and the solvent (acetone) were also studied. The results show that anthralin potently inhibits stimulated human monocyte superoxide-anion generation and enzyme degranulation, with a half-maximal inhibitory concentration (IC50) of as low as 0.02 μg/ml. Chemotactic migration of monocytes, however, was only affected when very high doses of anthralin (10 μg/ml) were used for pretreatment of the cells. Danthrone, up to a concentration of 10 μg/ml, or acetone alone (0.1%, v/v), did not inhibit the monocyte functions tested. Our results indicate that anthralin at pharmacological concentrations is a potent and selective inhibitor of human monocyte pro-inflammatory activities, by inhibiting respiratory burst activity (e.g. superoxide-anion generation) and enzyme degranulation, without affecting chemotactic migration.

    Notes: Summary Mesna (sodium-2-mercaptoethansulfonate) is used in the prophylaxis of cyclophosphamide (CYC)-induced hemorrhagic cystitis. Four patients being treated with “low dose” CYC and prednisone for vasculitis developed severe side effects to Mesna. Fever, arthralgia, myalgia, tachycardia, electrocardiogram changes consistent with perimyocarditis, erythroderma, bullous skin and mucous membrane lesions, and abdominal complaints with profuse diarrhea were noted approximately 3 weeks after the initiation of therapy for CYC-induced leukopenia and a conservatively reduced prednisone dosage. Positive reexposure tests confirmed the association to Mesna use, and hypersensitivity skin tests demonstrated a delayed hypersensitivity reaction.

    Notes: Summary The history of 188 Caucasian patients with atopic dermatitis (AD) and of 2,151 family members has been analyzed. Of the AD patients 48% suffered from respiratory atopy (36% rhinitis, 28% asthma, and 15% both). AD showed by far the earliest onset of all atopic diseases: 50% of our patients had skin lesions before the age of 2 years and 60% before the age of 5 years. In contrast, symptoms of allergic asthma developed in 40% of AD patients before the age of 5 years in comparison with only 25% who had allergic rhinitis. AD affects females more frequently than males (male to female ratio 1∶1.5), regardless of whether additional respiratory atopies are present or not. In contrast, respiratory atopies develop more frequently in males than in females (male to female ratio 1.5∶1). Mothers with respiratory atopy more often have atopic children (26%) than do fathers with respiratory atopy (13%). Finally, risk figures for genetic counselling are given. In short, the general risk of developing AD (3%) and atopy (7%) increases by a factor of two with each first-degree family member already suffering from atopy.

    Notes: Summary The neutrophil activating peptide NAP-1/IL-8 has in the past been shown to be secreted by diverse cell-types involved in inflammatory processes. Furthermore, potent biological effects on both neutrophilic granulocytes and lymphocytes enforce its role in inflammation. Recently, immunohistochemical studies using monoclonal anti-NAP-1/IL-8 antibodies have been performed on dermal inflammatory conditions like psoriasis vulgaris. These have demonstrated epidermal IL-8 immunoreactivity in a pattern inversely related to the degree of inflammatory infiltration. Based on these results, in the present study biopsies from patients with contact eczema as well as atopic dermatitis were examined. The same patterns of immunoreactivity were found with either homogenous epidermal staining, focally negative staining or overall decreased or even absent staining. As in psoriasis, these patterns were related to the degree of inflammatory infiltration. These results prove NAP-1/IL-8 to be involved not only in psoriasis vulgaris, but more likely to be a marker of different inflammatory processes. Future work will have to examine the kinetics as well as stimuli causing these effect.

    Description / Table of Contents: Summary. Necrolytic migratory erythema is considered to be one of five well-defined paraneoplastic syndromes of the skin. An association with a pancreatic A-cell glucagonoma is commonly postulated. However, during the last years 10 cases of necrolytic migratory erythema with no evidence for glucagonoma have been published. We report on a 59-year-old woman who presented with typical clinical and histological findings of necrolytic migratory erythema. Staging and laboratory investigations gave no evidence of any neoplasia. A liver biopsy revealed chronic persisting hepatitis. Hepatocellular dysfunction was recently reported to be present concomitantly in a number of cases of necrolytic migratory erythema, suggesting a relation to the skin changes described here.
    Notes: Zusammenfassung. Das Erythema necroticans migrans gilt als eine der fünf obligaten kutanen Paraneoplasien. Im allgemeinen wird von der Vergesellschaftung mit einem α-Zell-Glukagonom des Pankreas ausgegangen. Allerdings wurden in Abweichung davon im angelsächsischen Schrifttum in den letzten Jahren 10 Fälle von Erythema necroticans migrans ohne nachweisbares Glukagonom bekannt. Wir berichten über eine 59jährige Patientin mit den typischen klinischen und histologischen Befunden eines Erythema necroticans migrans, bei der wir in Staging- und Laboruntersuchungen keinen Hinweis auf das Vorliegen einer Neoplasie fanden. Die Leberbiopsie bewies eine chronisch-persistierende Hepatitis. Da auch bei anderen Fällen von Erythema necroticans migrans hepatozelluläre Störungen bestanden, mag ein Zusammenhang mit den Hautveränderungen vermutet werden.
