    Notes: Abstract 51 patients suffering from Hodgkin's disease or non-Hodgkin's lymphoma participated in this double-blind, cross-over study in which 2 antiseptic mouthwashes were tested for their effects on various periodontal index scores and salivary microbial counts. All patients were receiving combination cytostatic treatment based on methotrexate and doxorubicin. The patients (49±14 years old, 28 men, 23 women) were allotted at random to 2 groups. One rinsed with a 0.12% chlorhexidine gluconate (CHX) solution, the other with a 0.025% amine-stannous fluoride (AmF + SnF) solution 2× daily for 2 weeks. Both groups then continued rinsing with a 0.05% sodium fluoride (F) solution for 2 weeks, before switching over to AmF + SnF or CHX, respectively. All solutions had been prepared in such a way that they had the same colour and taste. Visible plaque index and gingival bleeding index scores were significantly reduced after periods of rinsing with CHX solution (P 〈 0.001) and AmF + SnF solution (P 〈 0.05). Microbiological cultivations of saliva specimens revealed significant reductions in mutans streptococci immediately after commencing rinsing, while lactobacilli and yeast counts were not affected.

    Description / Table of Contents: Zusammenfassung Infektionen sind die unmittelbare Todesursache bei vielen Krebspatienten. Bei neutropenischen Patienten mit Verdacht auf Infektionen ist es üblich, empirisch mit einer Kombination aus einem modernen β-Laktamantibiotikum mit einem Aminoglykosid zu behandeln. Die neuen Chinolone sind jedoch offensichtlich wichtige Kombinationspartner für β-Laktamantibiotika geworden. Nach einem blinden Randomisierungsverfahren erhielten 87 Patienten mit Verdacht auf eine schwere Infektion entweder 2 g Cefotaxim i.v. plus 200 mg Ofloxacin per os, zweimal täglich oder 2 g Cefotaxim i.v. zweimal täglich und Tobramycin dreimal täglich in einer der Nierenfunktion und dem Körpergewicht angepaßten Dosierung. Die Ansprechrate war in der mit Cefotaxim plus Ofloxacin behandelten Gruppe mit 71% signifikant besser als in der Vergleichsgruppe (47%). Die Therapie mit Cefotaxim plus Ofloxacin erwies sich als sicher und sehr viel weniger aufwendig für das Pflegepersonal.
    Notes: Summary Infection is the immediate cause of death in many patients with cancer. Traditionally, combinations of modern beta-lactam antibiotics and aminoglycosides are empirically used in the treatment of patients with neutropenia and presumed infection. However, the new quinolones appear to have become potent combination partners for beta-lactams. Eighty-seven patients with presumed serious infection were blindly randomised to receive either 2 g cefotaxime i.v. plus 200 mg ofloxacin twice daily (group 1) or 2 g cefotaxime i.v. twice daily plus tobramycin i.v. three times daily with dosage adjustment according to renal function and body weight (group 2). The response rate was significantly higher in group 1 (71%) compared to group 2 (47%). The cefotaxime/ofloxacin combination proved to be safe and represented a considerable reduction of workload on the nursing staff.
