    Notes: A self-adjoint, gauge invariant stability functional δWTG for determination of a sufficient condition of tearing mode stability is obtained for a plasma with a uniform pressure. The plasma is stable if there is no free energy available to support energy flux into the singular layer. The Δ' concept, valid in simple geometries, is generalized to reflect variation of equilibrium quantities on the flux surface. The relationship of δWTG to the ideal stability functional and other possible forms of tearing mode stability functionals is clarified through gauge transformations.

    Notes: The fast Fourier transform enables us to transform time series data efficiently between the time, frequency, ‘complex trace’ and ‘complex demodulate’ domains. The complex demodulate domain is related to the complex trace domain via a time-dependent phase rotation and optimal resampling. Data manipulation in any one domain has equivalents in the other domains, which may be more computationally efficient. Thus if we wish we can process seismic reflection data entirely in the complex demodulate (CD) domain. Working in the CD domain allows us to apply certain approximations which in turn allow us to carry out some involved operations very much more efficiently than in the time or frequency domains. We present some basic theory, some simple examples and an application to residual static estimation on crystalline bedrock. For this data a non-linear stackpower optimization in the CD domain had run times significantly less than those of similar processing in the time domain, and produced essentially identical results. Using a more narrow frequency band degrades the results only slightly, and decreases run times by a factor of up to 20 compared to the time domain processing.

    Notes: The influence on the adrenocortical function of high-dose topical glucocorticoids inhaled via conventional devices and via a Nebuhaler®, was studied in ten children with asthma. In patients treated with dosages of beclomethasone dipropionate or budesonide, in excess of 2100 μg/1.73 m2 body surface, the 24-h urinary excretion of free cortisol increased when the inhaler device was changed from a conventional one to a Nebuhaler. This suggests a decreased systemic influence of the inhaled glucocorticoids when these were administered via a Nebuhaler.

    Notes: The adrenocortical function was investigated in 18 children treated with high-doses of inhaled glucocorticoid aerosol (mean: 1965 μg/1.73 m2 body surface a day). Basal serum cortisol was only below the normal range in patients treated with doses exceeding 2500 μg/1.73 m2 body surface. 15 of 18 children had normal 24 h urinary free cortisol excretion, compared with 27 normal children matched for age, sex and body surface. Three patients taking more than 2400 μg/1.73 m2 body surface showed excretion values below the range for the normal controls. 10 of 12 patients showed a normal response to a short ACTH stimulation test. One patient treated with 3300 μg/1.73 m2 body surface showed no response and one patient gave a borderline response to ACTH. We concluded that doses up to 2000 μg/1.73 m2 body surface/24 h can be administered by pressurized aerosol with little risk of adrenocortical suppression.

    Notes: This study was undertaken to clarify the mechanism behind the severely decreased lymphocyte proliferative response upon stimulation with mitogens and antigens seen after allogeneic bone marrow transplantation (BMT) in man. We investigated eight BMT patients and eight controls and found that the proliferative response of patient cells was reduced both when the cells were stimulated with phytohaemagglutinin (PHA) and when they were stimulated with a combination of phorbol myristate acetate (PMA), which is an activator of protein kinase C (PKC), and the calcium ionophore A23187, which irreversibly opens for calcium transport into the cell (median relative responses were 41 and.37%, respectively). However, the PHA-induced increase in the concentration of intracellular tree calcium in post-BMT cells was not significantly different from the values found in control cells and the expression of interleukin 2 (IL-2) receptors (CD25) was only slightly decreased. However, the production of IL-2 was severely decreased in patient cells after stimulation with A23187/PMA (median 3541 units), although it was higher than in PHA-stimulated control cells (median 354 units). These results show that a direct activation of PKC by PMA combined with an increase in intracellular free calcium by A23187 cannot overcome the lymphocyte proliferation deficiency in cells from patients after allogeneic BMT. The data suggest that the defect is affecting the diacylglycerol pathway considerably more than the inositol triphosphate pathway.

    Notes: Summary The thermal cutaneous evoked tail flick response was measured both before and after intrathecal injection of R-apomorphine in spinal rats and in rats with an intact neuraxis pretreated with either long-term haloperidol or intrathecal 6-hydroxydopamine. Intrathecal R-apomorphine produced a dose-dependent increase of tail flick latency in spinal rats but not in intact rats. Long-term haloperidol treatment increased the effect of R-apomorphine in spinal rats while 6-OHDA decreased it. The present findings provide further evidence for the modulatory role of spinal DA receptor populations on spinal reflex activity. Pretreatment with long-term haloperidol or 6-OHDA may alter the conformational status of spinal DA receptors in opposite directions and thereby change the responsiveness of receptors to R-apomorphine.

    Notes: Summary Type 1 (insulin-dependent) diabetic patients with clinical nephropathy have a more than ten-fold increase in mortality of cardiovascular diseases compared with diabetic patients without nephropathy. The risk factors for cardiovascular disease, plasma concentrations of lipoproteins and fibrinogen, were investigated in 74 long-term diabetic patients: 37 with normal urinary albumin excretion, 20 with incipient nephropathy and 17 with overt clinical nephropathy based on urinary albumin excretion. The groups were matched according to sex, age and diabetes duration. The concentration of plasma cholesterol, very low density lipoprotein cholesterol, low density lipoprotein cholesterol, triglyceride and fibrinogen rose with increasing urinary albumin excretion. The plasma concentrations of these lipoproteins and fibrinogen were 11–14% higher in the patients with incipient nephropathy and 26–87% higher in the patients with overt clinical ne phropathy compared with the patients without nephropathy. The plasma concentration of high density lipoprotein cholesterol was unaffected by albuminuria. Patients with normal urinary albumin excretion and HbA1c〉8.0% had significantly higher very low density lipoprotein- and lower high density lipoprotein cholesterol concentrations compared with patients with HbA1c〈8.0%. Simple addition of the described risk factors can only account for a minor part of the greatly increased cardiovascular mortality in patients with diabetic nephropathy. An additional and possibly more decisive factor might be a change in the arterial wall, a change which promotes lipid accumulation and/or facilitates thrombus formation.

    Notes: Summary Albuminuria in Type 1 (insulin-dependent) diabetes is not only an indication of renal disease, but a new, independent risk-marker of proliferative retinopathy and macroangiopathy. The coincidence of generalised vascular dysfunction and albuminuria, advanced mesangial expansion, proliferative retinopathy, and severe macroangiopathy suggests a common cause of albuminuria and the severe renal and extrarenal complications associated with it. Enzymes involved in the metabolism of anionic components of the extracellular matrix (e.g. heparan sulphate proteoglycan) vulnerable to hyperglycaemia, seem to constitute the primary cause of albuminuria and the associated complications. Genetic polymorphism of such enzymes is possibly the main reason for variation in susceptibility.
