    Description / Table of Contents: Zusammenfassung Es gibt gute Gründe auch sogenannte „stumme” Steine zu operieren: 1. Die operative Letalität junger Patienten ist gering, unter 40 praktisch null. 2. 50 aller Patienten mit stummen Steinen bekommen Symptome, oder müssen innerhalb von 10–20 Jahren operiert werden. 3. Das Gallenblasencarcinom wird selten diagnostiziert und ist in den meisten Fällen bei Laparotomie inoperabel. Die Zahl der Gallenblasenkrebssterbefälle hat sich in den letzten 10 Jahren in Deutschland nicht geändert. Sie liegt unverändert hoch bei 3000 Menschen jährlich. Die Risikofaktoren zur Cholecystektomie steigen mit dem Alter des Patienten. Die Frühcholecystektomie wird daher empfohlen. Dennoch ist die Indikation durch Cholecystektomie beim stummen Gallenstein eine individuelle Entscheidung.
    Notes: Summary There are several sound arguments to operate on “silent” gallstones: 1) The operative mortality following simple cholecystectomy in patients under 40 is zero. 2) 50 % of all patients with silent gallstones will be operated upon or develop symptoms within 10–20 years after the initial diagnosis. 3) Carcinoma of the gallbladder is very rarely diagnosed in time and in most cases is incurable at the time of operation. The number of deaths in the FRG due to a carcinoma of the gallbladder has been consistently high in the last decade. Risk factores for cholecystectomy increase with the patients age. Therefore early cholecystectomy is recommended. Nevertheles indication for cholecystectomy is an individual decision.
