    Description / Table of Contents: Zusammenfassung Fortschritte der Chirurgie führten dazu, daß die Patienten heute im Durchschnitt einer schärferen Aseptik als vor 2–3 Jahrzehnten bedürfen. Die Suche nach Lücken in der Aseptik sollte sich nicht auf die Klimaanlagen beschränken und auch nicht bei ihnen beginnen. Zahlreiche Lücken in der Aseptik finden sich im täglichen Arbeitsablauf. Sie zu beseitigen bedarf es einer intensiven Schulung des Personals und zum Teil der Entwicklung praktikabler und auch bei Daueranwendung für das Personal unbedenklicher Desinfektionsmittel bzw. Arbeitsverfahren.
    Notes: Summary Advances in surgery over the last 2–3 decades have led to a situation whereby patients now usually need more thorough asepsis than in earlier times. The search for deficiencies in asepsis should, however, not be limited to air conditioners or even start with them. Many deficiencies in asepsis in day-to-day clinical routine have been pointed out. To overcome them intensive training of the staff and to an extent the development of effective disinfectants and procedures that are harmless to the staff even after constant use are necessary.

Bei Operationen am Dickdarm konnten in Blutproben aus Venen des Mesocolons nach gewisser Dauer der Präparationsarbeiten in jedem zweiten und im venösen Blut des großen Kreislaufes in jedem dritten Fall Bakterien nachgewiesen werden, die ihrer Art nach aus der Dickdarmflora stammen konnten. 2. Die nachgewiesenen Bakteriämien könnten teilweise verantwortlich sein für eine auffällige Häufung von Wundinfektionen nach Eingriffen, die von der Operationstechnik her kaum Möglichkeiten zu Verstößen gegen die Asepsis bieten, bei denen jedoch das Colon stumpfen Traumati sierungen ausgesetzt ist.

In operations on the large intestine we found bacteria in blood samples from mesocolon veins after a certain time of preparation in each second case, and also in every third case bacteria in the veins of the large circulatory system. These different types of bacteria apparently come from the large intestine. 2. These bacteriaemias could be partly responsible for the often recorded wound infections after interventions with trauma of the colon although the method of operation in itself does not offer increased chances of sepsis..

    Description / Table of Contents: Zusammenfassung Aus der bakteriell kontaminierten Wunde (Bauchchirurgie, Unfallchirurgie) den Übergang in die infizierte Wunde zu verhindern, wird seit langem durch Antibioticaprophylaxe versucht. Die bisherigen Ergebnisse waren mit allen Präparaten schlecht, Nebenwirkungen nicht selten. Für die in den letzten Jahren propagierte prä- bzw. perioperative Prophylaxe kann unterstellt werden, daß die Nebenwirkungen unbedeutend sein werden; ihre Wirksamkeit kann aber bisher nicht als wissenschaftlich exakt bewiesen gelten. Letzteres gilt auch für die Propagierung einer Gammaglobulinprophylaxe zur Verhütung postoperativer Infektionen.
    Notes: Summary Continuous prophylactic therapy with antibiotics has been used for a long time to try to prevent the step from bacterial contamination to clinical infection of surgical wounds. Earlier results of clinical trials were poor and side-effects common. For those used in recent years, however, the side-effects of pre- or perioperative prophylactic antibiotics have been minimal, although the therapeutical benefits have not yet been proven. This has also been the case with prophylactic infusion of γ-globulin for prevention of postoperative wound infections.
