    Notes: A total population survey was made of all 10-yr-old children in an inner London borough. As part of this study, systematic standardised interviews were held with random samples of parents of children from West Indian families and of children from non-immigrant families. In contrast to the situation in the Caribbean, the nuclear family was the norm for West Indian families living in London, just as it was non-immigrant families. In many respects the West Indians and non-immigrant families were closely similar. No differences were found in rates of parental mental disorder or criminality and the quality of family relationships was the same in the two groups. However, the West Indian parents were more likely to hold semiskilled or unskilled manual jobs and to live in poor quality overcrowded housing. In spite of this, they were also more likely to own their own house. West Indian families had many more children and differed somewhat in their patterns of child rearing. The significance of these findings is discussed in terms of both likely effects on child development and also the future implications for West Indian families living in London.

    Notes: A systematic study was made of a representative group of 21 same-sexed twin pairs (11 MZ and 10 DZ) in which at least one twin showed the syndrome of infantile autism. There was a 36 per cent pair-wise concordance rate for autism in MZ pairs compared with o per cent concordance in DZ pairs. The concordance for cognitive abnormalities was 82 per cent in MZ pairs and 10 per cent in DZ pairs. It was concluded that there were important hereditary influences concerning a cognitive deficit which included but was not restricted to autism. In 12 out of 17 pairs discordant for autism, the presence of autism was associated with a biological hazard liable to cause brain damage. It was concluded that brain injury in the infancy period may lead to autism on its own or in combination with a genetic predisposition. Uncertainty remains on both the mode of inheritance and exactly what is inherited.

    Notes: Summary The concept of adolescent turmoil is considered in the context of findings from a total population epidemiological study of Isle of Wight 14–15-yr-olds. It is concluded that parent child alienation is not a common feature unless the adolescents are already showing psychiatric problems. Inner turmoil, however, as represented by feelings of misery and self-depreciation is quite frequent. Psychiatric, conditions are only slightly commoner during adolescence than in middle childhood but the pattern of disorders changes in terms of an increase in depression and school refusal. Many adolescent psychiatric problems arise in early childhood but conditions arising for the first time during adolescence differ in important respect from those with an earlier onset.

    Notes: The study was designed to examine the effects on the performance of autistic children of varying the cognitive demands being made upon the children. The Board Form of the Raven Coloured Progressive Matrices was administered to 30 children and then, if necessary, either a range of easier, “matrices-type” problems, or the more difficult Standard Progressive Matrices. Distribution and type of errors suggested that for most of the children tested, success or failure on any item was best predicted by the intrinsic difficulty of that item rather than by the child's lack of co-operation. However, it did appear that for some of the lower functioning children, early experience of failure did interfere with subsequent performance. The implications of the findings are discussed in the context of the clinical assessment of autistic children.

    Notes: Using the data from five epidemiological studies of the total child population, the traditional distinction between general reading backwardness (i.e. reading below average for age, regardless of I.Q.) and specific reading retardation (i.e. a disability in reading relative to the child's age and general intellectual level) is put to the test. The concept and measurement of underachievement are discussed and it is shown that severe degrees of specific reading retardation occur at a rate above that predicted on a statistical basis, suggesting a “hump” at the lower end of the distribution. Reading retardation is shown to differ significantly from reading backwardness in terms of sex ratio, neurological disorder, pattern of neuro-developmental deficits and educational prognosis. It is concluded that the concept of specific reading retardation is valid, but evidence is discussed which shows that this does not support the view of a genetically distinct syndrome of dyslexia.

    Notes: A great deal has still to be learned on the various ways in which brain damage predisposes children to psychiatric disorder. Research has shown that many of the stereotypes of “the brain damaged child” were mistaken and misleading. But, equally, studies have clearly shown the major importance of brain damage as a factor contributing in a substantial way to the genesis of child psychiatric disorder. Treatment methods have still to be evaluated but already there are pointers as to how psychiatric disorders in neurologically impaired or brain damaged children should be treated. In dismissing over simplified catch phrase concepts concerning brain damage syndromes it is important not to overlook the reality of the problems to which they refer.
