Zusammenfassung Es wird über einen Fall eines isolierten ACTH-Ausfalls mit Nebennierenrindeninsuffizienz bei einer 24jährigen jetzt zum zweiten Mal graviden Patientin berichtet, die während der Schwangerschaft mit ACTH substituiert und termingerecht spontan von einem gesunden Mädchen entbunden werden konnte. Die sehr niedrigen 11-OHCS im Plasma und die 17-OHCS im Urin ließen sich durch Depot-ACTH steigern. ACTH war im Plasma nicht nachweisbar. Nach Injektion von Lysin-Vasopressin stiegen die 11-OHCS im Plasma nicht an. Die thyreotrope und gonadotrope Funktion des Hypophysenvorderlappens waren normal.

Summary In a 24 years old patient presently pregnant for the second time, adrenal insufficiency resulted from isolated failure of adrenocorticotropin secretion. During pregnancy she was treated with exogenous ACTH, and, at the expected time she delivered spontaneously a healthy girl. After treatment with exogenous ACTH low adrenal function was restored to normal. There was an increase in 11-OHCS in plasma and in 17-OHCS in urine. No ACTH in plasma could be detected by radio-immunoassay. After injection of lysine-vasopressin there was no increase in 11-OHCS in plasma. Gonadal and thyroidal functions of the anterior lobe of the pituitary were normal.
