Summary A modified ventricular drainage for obstructive hydrocephalus is reported. The frontal horn of one lateral ventricle is connected to the cistern of the Sylvian fissure with a rubber catheter. This manoeuvre was performed to meet a desparate situation in a very frail patient. The effects of the operation were slight; the result to date, after 4 months, is satisfactory. The advantages and disadvantages of this method, as compared withTorkildsen's ventriculo-cisternostomy, were discussed briefly.

Résumé Il est rapporté une méthode de drainage ventriculaire au cours des hydrocéphalies bloquées qui met en communication la corne frontale d'un ventricule latéral avec la citerne de la fosse sylvienne par l'intermédiaire d'un tube de caoutchouc. Cette méthode a été appliquée chez un malade très grave. L'opération s'est déroulée normalement et le résultat actuel après quatre mois est bon. Les avantages et les inconvénients de cette méthode par rapport au drainage ventriculaire deTorkildsen sont exposés.

Zusammenfassung Berichtet wird über eine modifizierte Ventrikeldrainage bei Verschluß-hydrocephalus, bei der das Vorderhorn eines Seitenventrikels durch einen Gummikatheter mit der Cysterna fossae Sylvii verbunden wird. Dieser Eingriff wurde aus einer Notsituation heraus bei einer sehr hinfälligen Kranken ausgeführt. Die Operationsbelastung war gering, das bisherige Resultat nach 4 Monaten ist günstig. Vor- und Nachteile dieser Methode gegenüber der Ventrikeldrainage nachTorkildsen werden abschließend erörtert.
