    Notes: Abstract The presence of androgen receptors (AR) in neuroendocrine cells was investigated in benign tissue of 10 prostatectomy specimens, in 12 prostatic adenocarcinomas with focal neuroendocrine differentiation and in 1 case of a pure neuroendocrine small cell carcinoma of the prostate. Neuroendocrine cells were defined by their reactivity with an antibody to chromogranin A. Monoclonal antibody F39.4 directed against the amino-terminal domain of the AR molecule was used to detect AR. AR and chromogranin A were simultaneously visualized with a double immunofluorescence technique. The results indicate that chromogranin positive cells in both benign and malignant prostatic tissue lack detectable expression of AR. No effect of endocrine therapy was noted. These results are in agreement with the hypothesis that prostatic neuroendocrine tumour cells represent an androgen insensitive cell population, which incidentally may expand to replace the androgen-sensitive tumour cell population during androgen ablation therapy.

    Notes: Abstract Multidrug resistance (MDR) in a variety of human tumours such as renal cell carcinoma (RCC) is thought to be caused by expression of theMDR1 gene and may be reversed by applying modern chemosensitisers such as dexverapamil, which inhibit theMDR1 gene product P-glycoprotein. This preliminary report gives information on a clinical study complying with good clinical practice regulations in patients with advanced RCC. The final evaluation is pending. Vinblastine, if anything the most effective chemotherapeutic agent (5-day continuous regimen), was combined with oral dexverapamil (6 times per day) as a chemosensitiser and dexamethasone to increase dexverapamil tolerance. All patients had histologically proven RCC, which was metastatic and progressive at study entry. The statistical design featured a pre-study regimen of two cycles of vinblastine alone followed by evaluation. If no response was documented, with all patients thus serving as their own control, dexverapamil and dexamethasone were added for three cycles of combination therapy. Having obtained institutional permission from the ethical review committee, we enrolled patients of whom 25 qualified for the combined-treatment arm; 13 patients finished the study, 5 patients failed to complete all treatment cycles (1 because of treatment-related toxicity, 3 for personal reasons, not related to treatment, 1 for tumour-related reasons) and 7 patients were at too early a stage for evaluation. Altogether, 61% of all patients tolerated a dose of dexverapamil of at least 2400 mg/day with peak serum levels reaching, in some cases, approximately 8 μM (the sum of dexverapamil plus nordexverapamil levels). WHO grade 3 and 4 toxicities were mainly myelosuppression (5/18). The combination of 1.4 mg m−2 day−1 vinblastine plus dexverapamil was generally felt to be safe and well tolerated. One partial response and 7 stable diseases were noted in this heavily pretreated study population. Four-hourly administration of dexverapamil in combination with dexamethasone plus escalation to the individually tolerated doses have permitted increases in serum levels of dexverapamil.

    Description / Table of Contents: Summary A method of determining the shear deformations of the cover plates of T- and box-beams is developed, employing a variational principle of minimum potential energy. In assuming a convenient shape of deformation Bernoulli's hypothesis is rendered invalid. The Euler equations yield ordinary differential equations for the solution of stress, stability, and vibration problems. It is shown that the stresses, the buckling loads and the characteristic frequencies may differ significantly from the values obtained with the elemantary theory of elasticity. The accuracy of the solution is investigated and shown to be of a high degree by comparing the results with those obtained with the exact theory of disks.
    Notes: Übersicht Um die Schubverformungen in den Deckplatten von Plattenbalken und Hohlkästen zu berücksichtigen, wird mit Hilfe eines Variationsprinzips vom Minimum der potentiellen Energie ein Näherungsverfahren entwickelt. Bei der Annahme über den Formänderungszustand wird die Bernoullische Annahme vom Ebenbleiben der Querschnitte verlassen. Die Euler-Gleichungen des Variationsproblems ergeben einfache gewöhnliche Differentialgleichungen, die eine Behandlung von Spannungs-, Stabilitäts- und Schwingungsproblemen ermöglichen. Es zeigt sich, daß die Spannungen, Knicklasten und Eigenfrequenzen ganz erheblich von den bekannten Werten der elementaren Stabtheorie abweichen können. Die Genauigkeit der Lösung wurde durch Vergleich mit der exakten Scheibentheorie überprüft, wobei sich eine gute Übereinstimmung ergab.

    Description / Table of Contents: Summary We consider a bar, which is produced by a variable thin-walled cross-section, sliding along a given space curve. To determine the twisted curvilinear middle-plane of the shell and its deformations, we use the methods of differential-geometry. The connection between the axis of the centre of gravity of the deformed and undeformed cross-sectioni s given by two rotation-tensors with the help of the Bernoulli-hypothesis. The shear-centre, the hindered warped cross-section by torsion and the oblique fibres are taken in account. The equations of equilibrium are set up for the deformed element of the bar. The components of impuls and angular momentum, including nonlinear gyroscopic terms, are given.
    Notes: Übersicht Wir betrachten einen Stab, der durch das Entlanggleiten einer dünnwandigen Querschnittfläche längs einer gegebenen Raumkurve erzeugt wird, wobei der Stabquerschnitt auch veränderlich sein kann. Zur Bestimmung der entstehenden in sich gewundenen und gekrümmten Schalenmittelfläche und ihrer Verformungen werden die Methoden der Differentialgeometrie herangezogen. Mit Hilfe der Bernoullischen Hypothese wird der Zusammenhang zwischen den Schwerpunktachsen der unverformten und verformten Querschnittfläche durch zwei Drehtensoren hergestellt. Es wird die Lage des Schubmittelpunktes, die behinderte Querschnittverwölbung aus Torsion und die Schiefstellung der Stabfasern mitberücksichtigt. Die Gleichgewichtsbedingungen werden für das verformte Stabelement aufgestellt. Hierin werden die für eine Schwingungsuntersuchung erforderlichen Impuls- und Drallkomponenten angegeben, in denen auch die nichtlinearen Kreiseleffekte enthalten sind.

    Notes: Summary Serum acid phosphatase activity is known to be elevated in a number of patients with metastatic prostatic carcinoma. Prostatic acid phosphatase (PAP) can be specifically measured by immunological methods. In this study we evaluated an enzyme-immunoassay of the sandwich type utilizing monoclonal antibodies. Immunoassayable PAP decreased after storage at room temperature for 72 h and also after repeated freezing and thawing, although not evident in all samples. PAP levels were measured in 88 newly diagnosed prostatic carcinoma patients, in 124 patients with histologically proven benign prostatic hypertrophy (BPH) and in 124 elderly hospitalized male patients without urological complaints. The upper limit of normal, as determined in the control group, was 2.3 μg/l. Of the BPH patients, 17% had elevated PAP levels (range 2.4–27 μg/l). We found a positive correlation between prostate volume (grams of tissue removed during TUR) and preoperative PAP values (r=0.26, N=121, P〈0.01). Elevated PAP levels were found in 3% of BPH patients with a prostate volume of less than 20 g (N=63), in contrast with 55% of patients with a prostate volume above 50 g (N=18). The sensitivity of this PAP assay for detecting the clinical stages of prostatic carcinoma was 13% for category To (N=15), 30% for category T1–2 (N=20), 71% for category T3–4 (N=24), and 83% for category M1 or N1–4 (N=29). The upper limit of normal (2.3 μg/l) as calculated from a large group of controls with a mean age of 62 years is rather similar to the value found in younger controls. In BPH patients, however, elevated PAP levels may be found which are seen more frequently in patients with a large prostate. Thus, for the calculation of the normal range, it is not recommended to include the results from BPH patients. The sensitivity of this enzyme-immunoassay with monoclonal antibodies in detecting prostatic cancer, at a specificity of 98%, is comparable to other immunoassays for PAP.

    Notes: Summary The pharmacokinetics and endocrine effects of the LHRH analogue buserelin [D-Ser(TBU)6-LHRH], released from biodegradable implants, were studied in 14 patients with stage C and D prostate cancer. Six patients received a subcutaneous implant of 3.3 mg buserelin monthly, and 8 patients received a subcutaneous implant of 6.6 mg buserelin every two months. Serum levels of buserelin decreased rapidly immediately after implantation. After 1–2 weeks a more gradual decline occurred, while in the two-monthly treated group a third phase of the elimination curve started after 5 weeks. Mean serum buserelin levels just before the next implantation in the two groups were not different. Urinary excretion of buserelin followed the same pattern. Serum LH levels in both groups became non-detectable 2 weeks after the first implant. This decrease of LH levels was accompanied by a suppression of serum testosterone to concentrations below 1 nmol/l (castration level). Side effects were not different from those observed with the intranasal application of buserelin. It is concluded that the subcutaneous application of buserelin is an easily administered form of treatment which has more profound and more reliable endocrine effects when compared with the intranasal administration of the drug. The greatest advantage of the new preparation is that the intervals between applications may be prolonged to at least 2 months.
