    Notes: Abstract The present study evaluated the ability of the anti-GD2 ganglioside monoclonal antibody 3F8 to target tumor sites in patients with small-cell lung cancer (SCLC). Of 12 patients entered into the trial, ten received intravenous 3F8 labeled with 2 or 10 mCi iodine-131. The first five patients had recurrent or progressive disease after chemotherapy. Subsequent patients were studied before starting chemotherapy. Radionuclide scans were performed on days 1, 2, and 3 post-infusion and once between day 5 and day 7. Four patients underwent single-photon emission tomography (SPET) imaging. Radionuclide scans demonstrated localization to all known sites of disease, other than small brain metastases in one patient. SPET/CT scan fusion images confirmed precise localization. No significant toxicity was observed. Mean serum half-life was 64.2 h. Analysis of specimens from one patient who died of unrelated causes 6 days post-infusion confirmed the scan results. The present study demonstrates that 3F8 targets SCLC sites in patients. Further studies of anti-GD2 antibodies with higher doses of antibody and radionuclide are warranted to evaluate their role in SCLC.

    Notes: Abstract Iodine-131 3F8, a murine IgG3 monoclonal antibody that targets to GD2-bearing tumors, was administered intravenously to 12 patients with brain tumors. Six patients received 2 mCi (0.74 Bq) of131I-3F8, five patients 10 mCi (3.7 Bq)/1.73 m2 of131I-3F8, and one patient 2.6 mCi (0.96 Bq) of124I-3F8, with no side-effects. Nine of 11 malignant gliomas and the single metastatic melanoma showed antibody localization, with the best tumor delineation on single-photon emission tomography (SPET) following 10 mCi (3.7 Bq)/1.73 m2 dose. No nonspecific uptake in the normal craniospinal axis was detected. There was no difference in the pharmacokinetics of low-dose versus the higher-dose antibody groups; plasma and total-body half-lives were 18 h and 49 h, respectively. Surgical sampling and time-activity curves based on quantitative imaging showed peak uptake in high-grade glioma at 39 h, with a half-life of 62 h. Tumor uptake at time of surgery averaged 3.5×10−3 %ID/g and peak activity by the conjugate view method averaged 9.2×10−3 %ID/g (3.5–17.8). Mean radiation absorption dose was 3.9 rad per mCi injected (range 0.7–9.6) or 10.5 cGy/Bq (range 1.9–26). There was agreement on positive sites when immunoscintigraphy was compared with technetium-99m glucoheptonate/diethylene triamine penta-acetic acid planar imaging, thallium-201 SPET, and fluorine-18 fluorodeoxyglucose positron emission tomography. Taken together, these data suggest that quantitative estimates of antibody targeting to intracranial tumors can be made using the modified conjugate view method.

    Notes: Abstract. This review discusses the application of alpha particle-emitting radionuclides in targeted radioimmunotherapy. It will outline the production and chemistry of astatine-211, bismuth-212, lead-212, actinium-225, bismuth-213, fermium-255, radium-223 and terbium-149, which at present are the most promising alpha-emitting isotopes available for human clinical use. The selective cytotoxicity offered by alpha particle-emitting radioimmunoconstructs is due to the high linear energy transfer and short particle path length of these radionuclides. Based upon the pharmacokinetics of alpha particle-emitting radioimmunoconstructs, both stochastic and conventional dosimetric methodology is discussed, as is the preclinical and initial clinical use of these radionuclides conjugated to monoclonal antibodies for the treatment of human neoplasia.

    Notes: Abstract Epidural neuroblastoma xenografts in nude rats causing paraparesis were treated with intravenous injection of an anti-GD2 monoclonal antibody 3F8. Metastatic or primary epidural tumors in humans cause rapid neurologic compromise. Treatment is often unsatisfactory. An animal model was established to study antibody targeted therapy of epidural tumor. Human neuroblastoma was xenotransplanted into the thoracic epidural space of nude rats. When paraparesis developed, animals were treated intravenously with an anti-GD2 monoclonal antibody, 3F8, either alone or radiolabeled with131Iodine. Improvement in neurologic function occurred in 2 of 20 (10%) animals receiving no treatment or control antibody, 14 of 17 (82%) animals receiving 3F8 alone and all 9 animals receiving131I-3F8 (p〈0.0001 for 3F8 or131I-3F8 vs. control). Six animals treated with 3F8 alone recovered normal neurologic function and remained well until sacrifice 10 days later. Four animals treated with 3F8 alone had no tumor evident on pathologic examination. The percent injected dose of131I-3F8/g tumor in 5 samples ranged from 0.73% to 3.8%. These observations demonstrate that neoplastic epidural compression of the spinal cord in the rat can be treated successfully with intravenous unmodified monoclonal antibody and that signs of neurologic dysfunction can be reversed. The potential of this approach in treating patients with epidural tumors and other neoplasms, especially those that are not sensitive to chemotherapy or radiotherapy, deserves to be explored.

    Notes: Abstract A mechanical drainage system, the “artificial lymphatic system” (ALS), consisting of a vacuum source and drain, is evaluated for its ability to aspirate the interstitial fluids responsible for the elevated interstitial fluid pressure (IFP) observed in solid tumors. IFP, pH, and pO2 radial profiles were measured before and after aspiration using wick-in-needle (WIN) probes, needle pH and oxygen electrodes, respectively. Laser Doppler flowmetry measured temporal changes in blood flow rate (BFR) at the tumor surface during aspiration. The WIN probe and IFP profile data were analyzed using numerical simulation and distributed mathematical models, respectively. The model parameter, p E reflecting central tumor IFP, was reduced from 15.3 to 5.7 mm Hg in neuroblastoma and from 13.3 to 12.1 mm Hg in Walker 256, respectively, following aspiration. The simulation demonstrated that spatial averaging inherent in WIN measurements reduced the calculated magnitude of the model parameter changes. IFP was significantly lower (p 〈 0.05), especially in regions surrounding the drain, and BFR was significantly higher (p 〈 0.05) following 25 and 45 min of aspiration, respectively; pH and pO2 profiles increased following aspiration. The experimental and mathematical findings suggest that ALS aspiration may be a viable way of reducing IFP and increasing BFR, pO2 and pH and should enhance solid tumor chemo and radiation therapy. © 2000 Biomedical Engineering Society.     Notes: Abstract This paper presents findings from uptake studies to evaluate the ability of an “artificial lymphatic system” (ALS) to enhance large and small molecular weight drug transport into solid tumors and the therapeutic effect of the additional drug on their growth. These studies also served to test the effectiveness of an implantable multidrain ALS. Walker 256, Neuroblastoma, and Sarcoma dual-tumor models were used to evaluate the effect of ALS aspiration on the uptake of 3F8 monoclonal antibody, and doxorubicin. A tumor shrinkage experiment using Walker 256 dual tumors was used to evaluate the efficacy of an implantable ALS with cyclophosphamide chemotherapy. Drug uptake significantly increased in all aspirated tumors; 3F8 uptake was enhanced 37.4% in the Walker and 93.1% in the Neuroblastoma tumor lines (p 〈 0.05). Doxorubicin uptake increased 23.2%; in Sarcoma tumor (p 〈 0.05). The shrinkage study demonstrated that one-drain aspirated tumors shrank 90% faster (p 〈 0.01) than control tumors, while three-drain aspirated tumors shrank 123% faster than control tumors (p 〈 0.01). © 2000 Biomedical Engineering Society.
