    Description / Table of Contents: Zusammenfassung Es wurde früher angenommen, daß der postanoxische Myoklonus durch eine Dysfunktion des ventro-lateralen thalamischen Nucleus bedingt ist. Diese Hypothese wurde durch verschiedene stereotaktische Studien in Frage gestellt. Neurochemische Untersuchungen, insbesondere die Messung von 5-Hydroxyindol-Essigsäure im Liquor öffnete den Weg zu neuen theoretischen und therapeutischen Möglichkeiten, Bahnen des serotoninergischen Systems zu beeinflussen. Ein typischer Fall wird vorgestellt, der unter einer Kombinationstherapie von 5-Hydroxytryptophan und einem Decarboxylase-Hemmer eine wesentliche Besserung zeigte. Die Pathogenese und therapeutische Möglichkeiten des postanoxischen Myoklonus werden diskutiert.
    Notes: Summary Postanoxic myoclonus was first accepted as being related to a dysfunction of the ventrolateral thalamic nucleus. Several stereotaxic studies have invalidated this hypothesis. The neurochemical approach, in particular the measure of 5-hydroxyindolacetic acid in the cerebrospinal fluid, has opened new theoretical and therapeutic possibilities involving serotoninergic pathways. A typical case is presented who improved markedly under a combined therapy with 5-hydroxytryptophan and a decarboxylase inhibitor. A review of the pathogenesis and therapeutic approach to postanoxic myoclonus is presented.

    Notes: Summary The specificity of immunoglobulin (Ig) for components of Borrelia burgdorferi was investigated in cerebrospinal fluid (CSF), serum and in CSF oligoclonal bands from nine patients with meningopolyneuritis Garin-Bujadoux-Bannwarth (MPN-GBB). All patients showed specific IgG and IgM antibodies in serum and CSF when incubated with a lysate of B. burgdorferi strain B 31. Specific antibody was detected in CSF but not in paired serum samples, indicating intrathecal synthesis. Investigation of the specificity of oligoclonal Ig in the CSF revealed oligoclonal bands with specificity for B. burgdorferi in one of the nine patients.

    Notes: Abstract We studied three patients with myelomeningoradiculitis caused byBorrelia burgdorferi, herpes zoster virus or cytomegalovirus infection. All patients underwent MRI of the spinal cord with gadolinium-DTPA and showed enhancing lesions of the spinal cord or nerve roots that correlated with clinical signs. Gadolinium-DTPA enhancement may visualize lesions that suggest an inflammation associated with blood-brain-barrier alteration and indicate the diagnosis before serological results are available.
