    Notes: Abstract We studied three patients with myelomeningoradiculitis caused byBorrelia burgdorferi, herpes zoster virus or cytomegalovirus infection. All patients underwent MRI of the spinal cord with gadolinium-DTPA and showed enhancing lesions of the spinal cord or nerve roots that correlated with clinical signs. Gadolinium-DTPA enhancement may visualize lesions that suggest an inflammation associated with blood-brain-barrier alteration and indicate the diagnosis before serological results are available.

    Notes: Abstract  We have studied the effects of pursuit eye movements on the functional magnetic resonance imaging (fMRI) responses in extrastriate visual areas during visual motion perception. Echoplanar imaging of 10–12 image planes through visual cortex was acquired in nine subjects while they viewed sequences of random-dot motion. Images obtained during stimulation periods were compared with baseline images, where subjects viewed a blank field. In a subsidiary experiment, responses to moving dots, viewed under conditions of fixation or pursuit, were compared with those evoked by static dots. Eye movements were recorded with MR-compatible electro-oculographic (EOG) electrodes. Our findings show an enhanced level of activation (as indexed by blood-oxygen level-dependent contrast) during pursuit compared with fixation in two extrastriate areas. The results support earlier findings on a motion-specific area in lateral occipitotemporal cortex (human V5). They also point to a further site of activation in a region approximately 12 mm dorsal of V5. The fMRI response in V5 during pursuit is significantly enhanced. This increased response may represent additional processing demands required for the control of eye movements.

    Notes: Summary Vertebrobasilar ectasia is a rare cause of typical trigeminal neualgia. In a recently published large series only thirty-one (2%) of 1404 consecutive patients with vascularly determined neuralgia were found to have Vertebrobasilar compression. We present three patients with trigeminal neuralgia caused by Vertebrobasilar ectasia, who were evaluated with high resolution magnetic resonance imaging (MRI). MRI studies provided accurate information on the anatomical location and course of the ectatic vessel in the cerebel-lopontine angle and the caused mass effect on the brainstem. With gadolinium-enhancement vascular compression of the trigeminal nerve was demonstrated clearly. Surgery confirmed compression of the fifth nerve by an ectatic and tortuous Vertebrobasilar artery in two cases. Following microvascular decompression neither patient experienced further pain. The third patient was treated with a ventriculoperitoneal shunt, as he developed occlusive hydrocephalus caused by the Vertebrobasilar dolicho-ectasia. MRI is useful in the evaluation of trigeminal neuralgia as it excludes other aetiologies such as tumour or arteriovenous malformation, but also demonstrates cranial nerve compression by ectatic vertebral arteries.

    Description / Table of Contents: Zusammenfassung Visuelle, durch Schachbrettmusterumkehr evozierte Potentiale wurden bei 9 Patienten mit einer neuralen Muskelatrophie untersucht. Die Patienten wurden entsprechend genetischer, klinischer und elektromyographischer Befunde als Typ HMSN I klassifiziert. Die Latenzen des zweiten positiven Gipfels der visuell evozierten Potentiale waren in 7 Augen von 5 Patienten eindeutig pathologisch verlängert. Aufgrund dieser Ergebnisse ist es wahrscheinlich, daß der N. opticus in einem höheren Prozentsatz bei Patienten mit einer neuralen Muskelatrophie, Typ HMSN I, betroffen ist als klinisch vermutet werden kann.
    Notes: Summary Pattern shift visual evoked potentials were investigated in 9 subjects with Charcot-Marie-Tooth disease, who were classified as HMSN I on clinical, genetic and electromyographic criteria. The latency of the second positive peak was increased in 7 eyes of 5 subjects. These results suggest that patients with HMSN I the optic nerves may be affected more frequently than suspected on clinical examinations.

    Notes: Abstract. The aim of this study was to assess blood flow in the middle cerebral artery (MCA) according to age, gender, and side. Eighty-eight subjects without carotid obstruction were measured for mean velocity, vessel area, and volume flow rates of both MCA with phase-contrast MR. A high-resolution sequence with a matrix of 300 × 512 and a double oblique localizing strategy was used for measurement. A mean velocity of 33 ± 6.8 cm/s, a mean vessel area of 6.2 ± 1.2 mm2 and a mean flow rate of 121 ± 28 ml/min were measured in the MCA. Lower volume flow rates were seen in subjects aged over 50 years (p 〈 0.01). When comparing women with men, a lower vessel area (p 〈 0.05) of the MCA was counterbalanced by a higher velocity, resulting in no significant difference of the volume flow rate. No difference occurred between the right and the left side. Flow reduction occurs in the elderly. A lower vessel area of the MCA in women is compensated by a higher velocity.

    Notes: Summary Computed tomography (CT scanning) was carried out on 35 patients with proven occlusion of the carotid artery. There was a clear correlation of the clinical picture with the CT demonstration of cerebral infarct; 14 patients had no identifiable infarct. The findings were also correlated with angiographic data. The degree of cross or collateral filling of the middle cerebral artery on the affected side correlated better with the clinical features than with the CT data. Radioisotope studies were positive in only 50% of cases in which CT had shown infarction; this was related to the time interval between the onset of symptoms and scanning. The significance of these findings and their relationship to previous clinical necropsy data are discussed.

    Notes: Summary The maximal width of the third ventricle, the maximal distance between the outer tips of the anterior horns, and the number of enlarged cerebral sulci on the two highest CT slices were measured in 68 chronic schizophrenic patients on cranial computed tomograms in order to detect a possible enlargement of the cerebrospinal fluid (CSF) filled intracranial spaces. These results were compared with values obtained from a control group which was formed in accordance with definite exclusion criteria and matched-pair parameters (sex, age and maximal inner diameter of the skull). In a prolective trohoc study no difference was found in the size of the CSF spaces of schizophrenics and the controls. The psychopathological condition of the patients, which was classified in a semistandardized dialogue, also showed no correlation with the ventricular size or the number of enlarged cerebral sulci.

    Notes: Summary The characteristic findings on computed tomography (CT) in multiple sclerosis (MS) are discussed. In a series of 49 cases plain CT was normal in 21 (43%), cerebral atrophy alone was present in 17 (35%) and plaques were visible in 11 (23%). These were most often adjacent to the lateral ventricles (14 plaques) and in the parietal white matter (10 plaques). CT was performed after the intravenous administration of iodide in 16 of these cases. Three of the low attenuation and three additional isodense lesions enhanced. Two patients with low attenuation plaques were scanned with xenon enhancement; the plaques absorbed less xenon than the corresponding contralateral brain substance and additional, previously isodense plaques were revealed. In one case the white matter absorbed much less xenon than normal and its uptake relative to grey matter was reduced.

    Notes: Summary The case notes of 170 consecutive patients with a suggestive clinical history, or a proven final diagnosis of a posterior fossa lesion have been analyzed in order to determine the precise value of each radiological investigation. All had had CT scans. The patients have been grouped clinically according to their presenting signs and symptoms. The analysis suggests some well-defined regimes of investigation and indicates the successes and failures of radiology.
